BACKGROUND: Over 100,000 Abbott Riata® were implanted in the United States before they were recalled in 2010. There are still a significant number of Abbott Riata® leads in use, and it is unclear how these leads should be managed at the time of generator change or lead malfunction. Although data comparing both Sprint Fidelis® and Abbott Riata® leads in this setting is available, there are no multicenter comparative studies of outcomes for various lead management strategies, including lead extraction (LE), lead abandonment/revision (LA), and generator change (GC) only at the time of device at e

BACKGROUND: Management of cardiac perforation caused by the lead of a cardiac implantable electronic device (CIED) is currently unclear. This study evaluated the outcomes of transvenous lead extraction (TLE) in patients with cardiac perforation caused by a transvenous lead.
HYPOTHESIS: Removal of perforated lead by transvenous approach is safe and effective.
METHODS: The medical records of all patients diagnosed with cardiac perforation by a pacing or defibrillator lead in Peking University People's Hospital from January 2008 to January 2019 were reviewed. We included patients who were manag

AIMS: In parallel with increasing implantation rates and patients' longer life expectancy, the need for transvenous lead extraction (TLE) as a specialized procedure has shown a significant growth over years. Herein, we aimed to present our initial experience in TLE by using a novel TightRail™ Rotating Mechanical Dilator Sheath.
METHODS AND RESULTS: Between October 2014 and March 2015, a total of 42 leads in 23 patients were removed at our tertiary referral centre. All of the extracted leads were >12 months old and indications for extraction were based on the recommendations of the Heart Rhyth

Identification and determination of leachable components are essential for the safety assessment of implantable medical devices. The safety concern threshold (SCT) for leachable components is 0.15 μg/day for genotoxic or carcinogenic compounds and 1.5 μg/day for others. Regulatory agencies require extraction of a whole medical device using an extraction media that simulates in vitro conditions. Large-sized medical devices therefore require large volumes of aqueous media, leading to extracts of very low concentrations of the targeted analytes. Analysis of these dilute solutions is often challen

BACKGROUND: Limited data exists for outcomes in patients undergoing cardiovascular implantable electronic device (CIED) transvenous lead extraction (TLE) without clear indications for device reimplantation. The implantable loop recorder (ILR) may be an effective strategy for continuous monitoring in select individuals.
OBJECTIVE: This retrospective analysis aims to investigate patients who have undergone ILR implant following TLE without CIED reimplantation.
METHODS: Clinical data from consecutive patients who have undergone TLE with ILR implant and without CIED reimplantation from October 2

INTRODUCTION: Current guidelines advocate reviewing peri-procedural anticoagulation on individual case basis for transvenous lead extraction (TLE). We investigated the safety of TLE on uninterrupted warfarin with therapeutic INR.
METHODS: Retrospective registry of consecutive patients undergoing TLE on uninterrupted warfarin (Warfarin Group) across two centres. Age and sex matched controls not on anticoagulation (No-Warfarin Group) and undergoing TLE over the same time-period were included. Both groups were compared over one-year.
RESULTS: 121 TLEs over 18-months. 22 patients on uninterrupte

BACKGROUND: Venous thrombosis occurring after TLE procedures is a fairly common but poorly described complication and requires further investigation into its causes.
AIM: The aim of this study was to identify independent predictors of UEDVT occurrence after TLE procedures.
METHODS: We analyzed data from 504 transvenous lead extraction procedures performed in the hybrid surgery room of the Department of Electrocardiology and Heart Failure at the Medical University of Silesia, Katowice, Poland, between January 2016 and May 2022. All procedures were performed using mechanical-only techniques.

BACKGROUND AND AIMS: Transvenous lead extraction (TLE) is increasingly performed for malfunction or infection of cardiac implantable electronic devices (CIED). Active mechanical rotational sheaths facilitate extraction and are available in short and long sizes, but it is unclear how many patients short active mechanical extraction sheaths (SAMES) are sufficient for TLE, and in which patients they may facilitate TLE. This study aimed to evaluate the incidence and predictors of successful lead extraction using only SAMES.
METHODS: We retrospectively analyzed patients who underwent TLE at the Un

INTRODUCTION: Cardiac tamponade (CT) is one of the most common and dangerous complications of transvenous lead extraction (TLE). So far, however, there has been little discussion about the problem.
METHODS: We analyzed the occurrence of CT in a group of 1226 patients undergoing TLE at a single reference center between June, 2015 and February, 2021. Using standard mechanical devices as first-line tools, a total of 2092 leads had been extracted.
RESULTS: CT occurred in 18 patients (1.47%): due to injury to the wall of the right atrium in 14 patients (1.14%) and other cardiac walls in four pati

BACKGROUND: Conventional cardiac device infections are increasing in incidence, causing significant morbidity and mortality. Leadless pacemaker (LP) therapy may provide new opportunities for the management of pacemaker (PM) infections as it does not require implantation of transvenous leads and a pectoral pocket.
OBJECTIVE: We sought to evaluate the effect of early and late LP implantation in patients diagnosed with device infection.
METHODS: Patients receiving an LP at our center after conventional PM lead extraction due to infection between December 1, 2013 and November 30, 2017 were inclu

BACKGROUND: Lead perforation, though an uncommon complication of cardiac device implantation, is associated with significant morbidity, especially when leads migrate to extracardiac structures. Lead migration into the abdominal cavity is exceedingly rare, and management in such cases can be complex.
CASE SUMMARY: We present the case of an 82-year-old woman with known dementia who underwent single-chamber pacemaker implantation for symptomatic Mobitz Type II atrioventricular (AV) block. Two weeks post-implantation, the nursing home staff observed that the patient had bradycardia. Electrocardio

Chronic superior vena cava obstruction is a recognized complication of transvenous pacemaker and implantable cardioverter-defibrillator leads. A 39-year-old woman with genetically confirmed left ventricular non-compaction cardiomyopathy and a transvenous implantable cardioverter-defibrillator placed for primary prevention developed progressive superior vena cava syndrome. Computed tomography angiography demonstrated chronic occlusion of the superior vena cava and left brachiocephalic vein with extensive collateralization. Endovascular recanalization was attempted from femoral, bilateral intern

Cardiac resynchronization therapy device with defibrillator (CRT-D) implantation is indicated for patients with a history of malignant ventricular arrhythmias, symptomatic heart failure, wide QRS, or high-degree atrioventricular block. A 67-year-old patient with dilated cardiomyopathy received a CRT-D with the conventional method but 1 month later skin necrosis was diagnosed above the device. The complete system was extracted from the patient and we utilized negative pressure wound therapy for the treatment of the remaining tissue. We decided to perform surgical reimplantation of the device us

Background and Objectives: The nature of multilevel lead-related venous stenosis/occlusion (MLVSO) and its influence on transvenous lead extraction (TLE) as well as long-term survival remains poorly understood. Materials and Methods: A total of 3002 venograms obtained before a TLE were analyzed to identify the risk factors for MLVSO, as well as the procedure effectiveness and long-term survival. Results: An older patient age at the first system implantation (OR = 1.015; p < 0.001), the number of leads in the heart (OR = 1.556; p < 0.001), the placement of the coronary sinus (CS) lead (OR = 1.2

Accurate non-invasive monitoring of blood glucose (BG) is a challenging issue in the therapy of diabetes. Here near-infrared (NIR) photoplethysmography (PPG) sensor based on a vapor-deposited mixed tin-lead hybrid perovskite photodetector is developed. The device shows a high detectivity of 5.32 × 1012 Jones and a large linear dynamic range (LDR) of 204 dB under NIR light, guaranteeing accurate extraction of eleven features from the PPG signal. By a combination of machine learning, accurate prediction of blood glucose level with mean absolute relative difference (MARD) as small as 2.48% is rea

BACKGROUND: The insertion of cardiac implantable electronic devices has become increasingly common, resulting in a growing need for lead extraction, most often due to infection and malfunction. The majority of the research indicated that the lead indwell time was the most important factor in determining the procedural success of extraction. In this study, we examined the factors affecting complete radiological success.
METHODS: A total of 192 lead extractions were performed on 102 patients at Başakşehir Çam and Sakura City Hospital. These procedures were compared based on lead indwell time an

Incorrect implantation of a ventricular pacemaker (PM) lead into the left ventricle (LV) is a known problem associated with permanent pacing. The optimal management of such cases identified late has not been clearly established. Generally acceptable management options are: open-chest cardiac surgery using cardio-pulmonary bypass, chronic anticoagulation and antiplatelet-drugs therapy. Rarely, the problem is solved by percutaneous LV lead extraction. We present a case of a patient with DDD pacing and ventricular lead implanted incorrectly into the LV apex region via an atrial septal defect eigh

Arteriovenous fistula (AVF) maturation failure is a possible barrier to establishing permanent hemodialysis access. While overt stenosis is the most commonly cited cause, a substantial proportion of non-maturing fistulas lack angiographically apparent lesions, suggesting that subclinical hemodynamic and geometric factors play a critical role. This case describes a 65-year-old man with end-stage renal disease on hemodialysis, coronary artery disease status post right coronary artery (RCA) stent placement, ischemic cardiomyopathy (left ventricular ejection fraction (LVEF) 40-45%), and a history
