Enhancing Distress Tolerance Skills in Adolescents With Anorexia Nervosa Through the BALANCE Mobile App: Feasibility and Acceptability Study.

BACKGROUND: Anorexia nervosa is a severe psychiatric disorder with high morbidity and mortality, particularly among adolescents. Family-based treatment (FBT) is the leading evidence-based intervention for adolescent anorexia nervosa, involving parents in renourishment and behavior interruption. Despite its effectiveness, challenges in distress tolerance and emotion regulation during high-stress situations, such as mealtimes, contribute to suboptimal treatment outcomes, with only 35% to 50% of adolescents achieving full recovery. Enhancing distress tolerance skills during FBT may improve treatm

Five-Year Outcomes of a Dialectical Behaviour Therapy Skills Training Group Versus Supportive Group Training for Individuals With Nonsuicidal Self-Injury: A Randomized Controlled Study.

BACKGROUND: Nonsuicidal self-injury (NSSI) is common among adolescents and adults and is linked to depression, impulsivity and borderline traits, with increased suicide risk. Dialectical behaviour therapy (DBT) is widely used for NSSI, yet long-term evidence across age groups remains limited. OBJECTIVE: To compare the long-term effectiveness of DBT skills groups (DBT-ST) versus social support group training (SSGT) for individuals with NSSI, and to characterize the durability of changes over up to 5 years. METHODS: In this prospective randomized controlled trial, 47 individuals with NSSI were

Personality Disorders in Older Age.

Personality disorders are among the most common mental disorders in older age and are associated with a range of negative outcomes in physical and emotional health as well as in interpersonal functioning. Several screening tools have been validated with older patients and can aid in diagnosis. The presence of a personality disorder is associated with increased risk of cognitive decline, and cognitive disorders may mimic personality disorders. As a result, an evaluation of cognitive function is an essential part of assessing for a personality disorder. Emerging evidence points to the promise of

Impact of DBT-based counselling program on emotional eating and emotional regulation among overweight women.

BACKGROUND: This study investigates the impact of a Dialectical Behavior Therapy (DBT)-based counseling program on emotional regulation (ER) and emotional eating (EE) among overweight female students. The program incorporated four core DBT modules: mindfulness, emotion regulation, distress tolerance, and interpersonal effectiveness. METHOD: A sample of 68 female students at a public university in Saudi Arabia was selected for a quasi-experiment. The participants were divided equally into two groups: an experimental group (n = 34) and a control group (n = 34). The dependent variables (ER and E

Borderline personality disorder across the lifespan.

Borderline personality disorder is characterized by affective dysregulation, intense, unstable interpersonal relationships, impulsivity and unstable identity. It overlaps considerably with both PTSD and bipolar spectrum disorders. Research on true late-life BPD is limited, but suggests that some of the core features of BPD including interpersonal difficulties, unstable affect and anger remain relatively unchanged, while impulsivity and identity disturbance decline or change their mode of expression in late life. Diagnosis of BPD in late life requires both flexible application of the standard d

The economic burden of Borderline Personality Disorder: direct and indirect costs, and evidence from evaluations of interventions.

BACKGROUND: Borderline Personality Disorder (BPD) is characterized by longstanding patterns of difficulty in functioning, and when left untreated, BPD can impose a substantial financial burden on individuals and society, in terms of higher healthcare costs as well as broader societal costs. AIMS: To conduct a critical review of the direct and indirect costs related to BPD. Additionally, to critically evaluate the economic evidence on interventions for BPD. METHODS: A comprehensive search strategy was conducted across PsycINFO, PubMed, and SCOPUS. Studies were screened in two phases: an initi

Development of an Evidence-based Violence Rehabilitation Program for Offenders With Intellectual Disability.

A review of the current literature on evidence-based violence prevention programs developed for individuals with an intellectual disability (ID) reveals a paucity of direct evidence for this population. In addition, the existing offence-specific programs are primarily grounded in adapted cognitive behaviour therapy (CBT) programs designed for the mainstream offender population, which may not be suitable for offenders with co-morbid mental health and personality disorders. The current paper discusses the development of a violence rehabilitation program for offenders with an ID. The focus of the

Digital Approach to Improving Mental Health.

This chapter focuses on investigating various digital approaches to improving mental health. Due to the increasing prevalence of various issues related to mental health worldwide, there is increasing interest in various approaches to improve mental well-being. We open this chapter with an overview of the current statistics about mental health problems and a short description of each of the most common mental disorders. This is followed by a description of various toolkits for designing mental health support applications, i.e., cognitive behavioral therapy and dialectical behavioral therapy. We

Misophonia: A scoping review of research.

OBJECTIVE: To scope the literature describing misophonia populations, management, and research opportunities. METHOD: Literature searches for research studies describing patients diagnosed with misophonia, defining a patient profile, or outlining development or testing of an intervention for misophonia. A data extraction form was developed and piloted before data from each article were independently charted by two researchers. Researchers then agreed on a final data set for each article. RESULTS: Thirty-one records were included. The misophonia population was described in terms of onset age,

Hypnosis applications to the treatment of posttraumatic stress disorder.

Current evidence-based therapies considered efficacious for posttraumatic stress disorder (PTSD) include Prolonged Exposure Therapy (PE), Cognitive Processing Therapy (CPT), Eye Movement Desensitization Reprocessing (EMDR), and medication treatments. Hypnosis, one of the first mind-body therapies known to Western medicine, has been used for mind-body problems such as conversion reactions, medication-less anesthesia, and wound healing. Because PTSD is a mind-body problem, hypnosis seems especially suited for its treatment. Hypnosis is a state-based treatment that can move PTSD patients into a s

Highly Aggressive Women in a Forensic Psychiatric Hospital.

In this study, we compared three groups of women admitted to a public forensic inpatient facility over the course of a two-year period. Detailed and systematic examination of social and psychiatric histories revealed that the group with the most persistent levels of aggression differed from the other two groups with respect to frequency of self-harming behavior, intellectual impairment, hypothyroidism, a childhood diagnosis of attention deficit-hyperactivity disorder (ADHD), and age of onset of psychiatric and behavioral symptoms. The high-aggression group also had the highest rate of childhoo

Treating adults with a dual diagnosis of borderline personality disorder and posttraumatic stress disorder related to childhood abuse: Results from a randomized clinical trial.

OBJECTIVE: About half of individuals seeking treatment for borderline personality disorder (BPD) present with co-occurring posttraumatic stress disorder (PTSD). However, therapies that have been proven efficacious for simultaneously treating the full spectrum of core symptoms in patients with a dual diagnosis of BPD + PTSD are lacking. METHOD: This is a subgroup analysis from a randomized controlled trial (registration number DRKS00005578) which compared the efficacy of two treatment programs, dialectical behavior therapy for PTSD (DBT-PTSD) versus cognitive processing therapy (CPT). Factors predicting early dropout from dialectical behaviour therapy in individuals with borderline personality disorder.

BACKGROUND: Dialectical behaviour therapy skills training (DBT-ST) has proven effective to treat individuals with borderline personality disorder (BPD). However, therapy still faces the problem of early dropout. The aim of the present study is to examine which factors are associated with early dropout from DBT-ST in a sample of subjects with BPD. METHOD: 118 subjects with BPD diagnosis were included in the study. Apart from socio-demographic and clinical variables, childhood trauma history, personality dimensions, and comorbidities with other psychiatric disorders were collected. Differences

Therapeutic and Preventive Interventions in Adolescents with Borderline Personality Disorder: Recent Findings, Current Challenges, and Future Directions.

BACKGROUND: Borderline personality disorder (BPD) has long suffered from overshadowing in adolescents and hopelessness from the psychiatrists themselves. Comprehensive guidelines for this age group are lacking. AIMS: This narrative review aims to describe current recommendations for BPD and recent empirical evidence on effective treatments (both pharmacological and non-pharmacological) and preventive approaches. Innovative approaches, based on recent and original research on BPD adolescents, are also discussed. RESULTS: Very low-certainty evidence has supported that medication has a positive

Lifetime and acute risk of suicidality by profiles of early life adversity: an observational cohort study in a high-risk population.

Early life adversity in the form of child maltreatment (CM) is a transdiagnostic risk factor for suicidal ideation and behavior (SIB), yet its heterogeneity and relevance for lifetime versus acute SIB and psychotherapy outcomes remain unclear. In an observational cohort of 240 high-risk adults with borderline personality disorder (42.5%) or persistent depressive disorder (57.5%), lifetime and acute SIB were assessed using the Columbia-Suicide Severity Rating Scale. CM profiles were derived using a data-driven clustering approach based on the Childhood Trauma Questionnaire. Associations between

Does More Mean Better? Treatment Intensity and Predictors of Clinically Significant Change in Suicidal Ideation During Dialectical Behaviour Therapy.

This study examined whether DBT treatment intensity predicts clinically significant change (CSC) in suicidal ideation, whether maladaptive personality traits moderate this relationship and whether early symptom improvements in suicidal ideation from baseline to midtreatment predicted CSC at post-treatment, above and beyond the effects of treatment intensity and personality traits. Adults (n = 105) in a hospital-based DBT day treatment programme were triaged into three DBT intensity tracks over 12 weeks: low intensity (weekly DBT skills group), mid intensity (weekly skills group plus individual

Clarifying conceptualization of emotional dysregulation: Differences with emotional lability during 4-month DBT skills training - A naturalistic study.

INTRODUCTION: Emotion dysregulation is common in many different psychiatric disorders and it can be effectively treated with the well-established Dialectical Behavioral Therapy (DBT). Despite its clinical relevance and increasing scientific interest, emotional dysregulation (ED) is sometimes conflated with emotional lability (EL). However, these constructs differ: ED involves top-down neurobiological processes, while EL involves bottom-up processes. As psychotherapy essentially influences top-down processes compared to pharmacotherapy, this study aimed to investigate whether ED is more sensiti

Randomized controlled trials confirm the efficacy of psychological interventions and medications: Commentary on Jobes and Barnett (2025).

Jobes and Barnett (see record 2024-78987-001) decided to emphasize and recommend only psychological interventions (e.g., dialectical behavior therapy) in the clinical treatment for suicidal risk and behavior. Such interventions have been, in fact, proven effective in research emphasizing randomized controlled trials. Jobes and Barnett, however, decided not to include a section dealing with medications because they concluded that, with some exceptions (e.g., clozapine), the majority of medications have "little to no evidence" in randomized controlled trials. The article by Jobes and Barnett not

Why Is It Still So Difficult to Acknowledge the Reality of Borderline Personality Disorder?

Borderline personality disorder (BPD) has emerged as a major clinical and societal challenge since its mid-twentieth-century conceptualization. This paper addresses four persistent obstacles to effective care: diagnostic complexity, interpersonal dysfunction and risk behaviors, high societal burden, and pervasive stigma. We argue that despite recent critiques, many of which propose replacing the diagnosis with trauma-related formulations, BPD remains a valid, coherent, and clinically indispensable construct. Misdiagnosis contributes to iatrogenic harm, while accurate diagnosis enables targeted

The Mediating Roles of Hostility and Dissociation in the Relationship Between Sexual Assault and Suicidal Thinking in College Students.

Sexual assault and suicide are two serious public health concerns. Research has documented the relationship between sexual assault and suicidal thinking and attempts; however, limited research explores the more multifaceted relationships between posttraumatic stress reactions and suicidal ideation in college students through unsuccessful modulation of emotion. The authors hypothesized a mediation model where the relationship between sexual assault and suicidal ideation is mediated by dissociation and hostility. In total, 1,677 undergraduate students were administered modified versions of the T
