The syndrome of male pseudohermaphroditism secondary to 5 alpha-reductase deficiency is reviewed, as are hormonal evaluation and tissue studies documenting the enzyme deficiency. These studies reveal that the 5 alpha-metabolite dihydrotestosterone is essential for differentiation of the external genitalia and prostate. Studies of male rat fetuses treated with 5 alpha-reductase inhibitors during the critical period of sexual differentiation in utero reveal incomplete masculinization of the external genitalia and impaired prostate growth and development. Pineal acid phosphatase (ACP) activity was examined in seven experiments involving young intact or castrated male and female Syrian hamsters. Nine-week old female hamsters had a 3-fold (p < 0.001) higher ACP activity in their pineal glands than did males. After three weeks of castration, a significant increase (p < 0.001) in ACP activity occurred in castrated male pineal glands. To determine the effect of estrogenic and progesteronic activity on lower esophageal sphincter (LES) circular muscle, studies were performed on 20 adult opossums. Does-response curves on circular smooth muscle strips from the LES were constructed for gastrin and acetylcholine alone, and with 17beta-estradiol and/or progesterone added. Each female hormone significantly decreased the maximal LES muscle responses to gastrin and acetylcholine. A combination of 17beta-estradiol and progesterone abolished the response to gastrin.
