UNLABELLED: We retrospectively reviewed 10 patients with spinal brucellosis of the thoracic and lumbar spine who were treated successfully with a combination of surgery and antibiotics. All patients had back pain; six patients had radiculopathy and one patient had paraparesis. Patients with spondylodiscitis without epidural abscesses (n = 3) had transpedicle discectomy and drainage. Epidural abscesses in the lumbar area caused by spondylodiscitis (n = 3), spondylitis (n = 2), and discitis with infected disc herniation (n = 1), were drained using a posterior approach combined with posterolatera

BACKGROUND Percutaneous transforaminal endoscopic discectomy (PTED) has been widely applied for lumbar disc herniation (LDH) in recent years; however, the achievement of bilateral decompression through a conventional unilateral PTED approach remains a key challenge. This retrospective study evaluated outcomes after unilateral PTED among patients with lumbar stenosis due to a large central LDH and concomitant bilateral neurological symptoms. MATERIAL AND METHODS From January 2018 to December 2020, 34 patients aged 22 to 54 years with large central LDH and concomitant bilateral neurological symp

BackgroundSymptomatic same-level recurrence remains a concern after percutaneous endoscopic lumbar discectomy (PELD). We compared PELD plus endoscopic annular suture with PELD alone.MethodsThis single-center prospective observational cohort enrolled patients from January 2023 through October 2024. Treatment was selected through shared clinician-patient decision-making. Propensity scores based on eight baseline variables were matched 1:1, yielding 51 patients per group. Clinical scores were assessed through 12 months; symptomatic recurrence was followed for up to 23.9 months. Kaplan-Meier and e

Percutaneous lumbar nucleoplasty (PLN) and intradiscal electrothermal therapy (IDET) are effective treatment options for discogenic low back pain (D-LBP). We evaluated the effectiveness of PLN and IDET and the positive predictive factors associated with intradiscal procedures. We reviewed the medical records of 205 patients who underwent IDET or PLN in patients with D-LBP followed by positive provocation discography. A successful outcome was defined as ≥ 50% pain relief on the numerical rating scale (NRS) pain score at the 6-month follow-up visit. The relationship between the outcome of the in

The objective of this study was to investigate the risk factors associated with surgical site infection (SSI) after percutaneous endoscopic lumbar discectomy (PELD) in patients with lumbar disc herniation (LDH). A retrospective analysis was performed on a cohort of 335 patients who underwent PELD between January 2016 and January 2023. Data were derived from the Hospital Information System (HIS), and a comprehensive statistical assessment was performed using IBM SPSS Statistics version 25.0. Both univariate and multivariate logistic regression analyses assessed a range of risk determinants, suc

BACKGROUND: Transcorporeal percutaneous endoscopic cervical discectomy (TcPECD) destroys the integrity of the vertebral body. We herein discuss its long-term risks and avoidance measures. Case presentation: A 44-year-old woman underwent TcPECD. Although her upper limb symptoms were relieved after the operation, the bone channel did not heal and the endplate of the segment was altered. She consequently developed chronic neck and shoulder discomfort.
CONCLUSION: Careful preoperative planning is needed to avoid non-healing of the bone channel following TcPECD. The diameter of the bony channel sh

BACKGROUND: The long-term complications of traditional discectomy and fusion surgery have led to the need for minimally invasive procedures that do not require a complete discectomy and fusion. Jho developed an anterior unco-foraminotomy that we have modified, with the approach being more medial than that of Jho, into an anterior transcorporeal tunnel approach which we use for cervical spondylotic unilateral radiculopathy.
METHODS: A retrospective analysis was carried out in 30 patients who underwent a transcorporeal "tunnel" anterior micro-foraminotomy for unilateral radicular symptoms with

The purpose of this article is to present the clinical results of the prospective series of studies on percutaneous nucleotomy (PN) for lumbar disc herniation and to introduce current criteria of patient selection for PN. Since 1983, 107 patients were treated with PN (Hijikata's method), 85 of whom had a follow-up period of more than 2 years. Of these 85 patients, 54 had successful results. Clinical findings and various imaging evaluations (eg, magnetic resonance imaging and computed tomography after discography) before PN, as well as operative findings, were used to study the surgical outcome

PURPOSE: The spine is an important source of pain and disability, affecting two thirds of adults at some time in their lives. Treatment in these patients is mainly conservative medical management, based on medication, physical therapy, behavioral management, and psychotherapy, surgery being limited to elective cases with neurologic deficits. This study was carried out to evaluate the efficacy of percutaneous nucleoplasty in patients affected by painful diskal protrusions and contained herniations.
METHODS: From February 2004 to October 2005, 72 patients (48 men, 24 women; mean age 48 years) a
