Homelessness among problem drug users: prevalence, risk factors and trigger events.

The present paper uses data from a prospective cohort study of 877 problem drug users entering treatment in Scotland to extend knowledge of homelessness and drug misuse in three important respects: First, the prevalence of homelessness among problem drug users is investigated; secondly, key risk factors for homelessness among problem drug users are identified; and thirdly, trigger events associated with movements into or out of homelessness by problem drug users over time are explored. Data were collected during two waves of interviewing which were conducted 8 months apart. Thirty-six per cent

HIV incidence among injection drug users in Baltimore, Maryland (1988-2004).

BACKGROUND: We examined recent trends (1999-2004) in HIV incidence among a cohort of injection drug users (IDUs) followed since 1988 in Baltimore, Maryland. METHODS: One thousand eighty-three HIV-seronegative individuals with a history of injection drug use were recruited between 1988 and 1998 and returned for >or=1 semiannual follow-up visit, where they underwent HIV antibody testing and interviews eliciting risk behaviors. Person-time methods were used to calculate HIV incidence rates per 100 person-years (PYs). RESULTS: Over 14,770 PYs, 304 individuals seroconverted to HIV (2.06 per 100 P

Values and beliefs of psychedelic drug users: a cross-cultural study.

Psychedelic drugs such as LSD and psilocybin are often claimed to be capable of inducing life-changing experiences described as mystical or transcendental, especially if high doses are taken. The present study examined possible enduring effects of such experiences by comparing users of psychedelic drugs (n = 88), users of nonpsychedelic illegal drugs (e.g., marijuana, amphetamines) (n = 29) and non illicit drug-using social drinkers (n = 66) on questionnaire measures of values, beliefs and emotional empathy. Samples were obtained from Israel (n = 110) and Australia (n = 73) in a cross-cultural

HIV among injection drug users in large US metropolitan areas, 1998.

This article estimates HIV prevalence rates among injection drug users (IDUs) in 95 large US metropolitan areas to facilitate social and policy analyses of HIV epidemics. HIV prevalence rates among IDUs in these metropolitan areas were calculated by taking the mean of two estimates: (1) estimates based on regression adjustments to Centers for Disease Control and Prevention (CDC) Voluntary HIV Counseling and Testing data and (2) estimates based on the ratio of the number of injectors living with HIV to the number of injectors living in the metropolitan area. The validity of the resulting estima

Memory-related hippocampal dysfunction in poly-drug ecstasy (3,4-methylenedioxymethamphetamine) users.

RATIONALE: 3,4-Methylenedioxymethamphetamine (MDMA, ecstasy) is neurotoxic in animal studies and its use has been associated with cognitive impairments in humans. OBJECTIVE: To study hippocampal activation during the retrieval from episodic memory in polyvalent users of ecstasy. METHODS: Twelve polyvalent ecstasy users and twelve matched controls were examined by means of functional magnetic resonance imaging (fMRI) while they retrieved face-profession associations from episodic memory. RESULTS: Ecstasy users had a normal structural MRI scan without focal brain lesions or anatomical abnorma

Club drug use among minority substance users in New York City.

Surveillance data suggests that club drug use (Ecstasy, GHB, ketamine, LSD, methamphetamine, PCP and flunitrazepam) has been a predominantly White adolescent and young adult phenomenon in the United States. The authors investigated the use of club drugs among 323 street-recruited minority substance users in northern New York City (66.3% were Hispanic, 23.8% were Black, and 9.9% were White/other race; median age = 32 years old). While Whites were more likely than others to have used club drugs, club drug use among Hispanics and Blacks was not uncommon; 45.3% Hispanics and 56.4% of Blacks report

Targeting HIV prevention on African American crack and injection drug users.

The use of drugs in the African American community, particularly crack cocaine, has been linked to sexual risk-taking behavior, which increases the likelihood that persons will become infected with Human Immuno Virus. In order to more fully understand risk-taking behavior and to target interventions among African American men and women, this study used data collected from 1277 individuals residing in Lexington and Louisville, Kentucky, who were recruited into National Institute on Drug Abuse (NIDA) Cooperative Agreement Project from 1993 to 1998. The study compared African Americans treated fo

[Causes of death in HIV-infected French drug users, 1995-2000].

Since the decline in mortality among HIV-infected persons after introduction of highly active antiretroviral therapy, concerns related to co-morbidities have increased as they may impair vital prognosis, particularly in intravenous (IV) drug users. We describe firstly the changes in the distribution of the causes of death between 1995 and 2000 among IV drug users in the "Aquitaine Cohort" based on hospital information system, and secondly the distribution of the causes of death among IV drug users in the French national survey "Mortalité 2000" specifically set up in 2000 for optimal exhaustive

Incentives vs outreach workers for latent tuberculosis treatment in drug users.

BACKGROUND: Drug users are at increased risk for latent tuberculosis infection (LTBI) and also at increased risk for noncompletion of medication regimens for treatment of LTBI or tuberculosis disease. Directly observed therapy (DOT) provided by outreach workers, the use of incentives, or both have been suggested as a means to increase adherence. OBJECTIVE: To compare the independent and combined effects of monetary incentives and outreach worker provision of DOT for LTBI treatment in a sample of active drug users. METHODS: The research design was a randomized controlled trial in a community

Partner notification with HIV-infected drug users: results of formative research.

The authors conducted formative research on the use of partner notification with HIV-infected drug users (i.e. those who use/abuse injectable drugs, crack or cocaine) in order to guide the development of an effective intervention for this population in New York City. Structured focus group and personal interviews were conducted with 25 in- and out-of-treatment drug users, 23 counsellors from a sexually transmitted disease (STD) clinic and a methadone maintenance treatment programme (MMTP), and nine experts in the field of HIV partner notification and/or substance abuse prevention and treatment

Wound botulism associated with black tar heroin among injecting drug users.

CONTEXT: Wound botulism (WB) is a potentially lethal, descending, flaccid, paralysis that results when spores of Clostridium botulinum germinate in a wound and elaborate neurotoxin. Since 1988, California has experienced a dramatic increase in WB associated with injecting "black tar" heroin (BTH), a dark, tarry form of the drug. OBJECTIVE: To identify risk factors for WB among injecting drug users (IDUs). DESIGN: Case-control study based on data from in-person and telephone interviews. PARTICIPANTS: Case patients (n=26) were IDUs who developed WB from January 1994 through February 1996. Con

Geographic variation in HIV infection among injecting drug users with Barcelona.

In response to a high incidence of AIDS among injecting drug users in Barcelona, Spain, the city established a program in four geographically distinct centers for assistance and surveillance. The centers provided testing, counseling, and monitoring of infectious diseases. In 1991-92, HIV rates were 33% in the northwest center, 39% in the western center, 39% in the east, and 71% in the south. Differences between the rates in the southern center when compared with those in the other centers were very significant (P < or = .001). These data show geographic variation of HIV infection within one ci

Inadequate bleach contact times during syringe cleaning among injection drug users.

Objectives were to measure syringe cleaning strategies used by injection drug users (IDUs) and to assess syringe contact with bleach during cleaning demonstrations. IDUs were interviewed about cleaning activities during their most recent injection episode; they demonstrated these activities on videotape. Coders reviewed the videotapes, categorized activities, and used stop watches to record bleach exposure. Of 161, 146 subjects reported cleaning at last injection, 85 (58%) of 146 used full strength bleach. Of bleach users, 20% had total contact time (duration of bleach inside syringe) of > or

HIV risks for women drug injectors: heroin and amphetamine users compared.

The incidence of HIV infection among women in Europe and the US is growing rapidly. Women who inject drugs are particularly vulnerable--they may acquire the infection through sharing injecting equipment and through sexual contact with an infected male. Opioids and stimulants are reputed to have different effects upon sexual activity and a sex life enhanced by drugs may increase the risk further. In the context of two larger studies of HIV-related risk behaviours among opioid and amphetamine users in the North West of England, the sexual behaviour and sharing of two groups of female injectors w

HIV prevalence and risk behaviour in injecting drug users in Berlin.

In injecting drug users (IDU) newly entering drug treatment centres in Berlin, HIV seroprevalence continuously decreased from 1985 through 1992. A recent cross-sectional study with multi-site sampling (n = 472) showed significantly differing HIV prevalences by site of recruitment (treatment centres 6%, storefront units 20%, infectious disease clinic 56%). In multivariate analysis, the borrowing of syringes in prison was the strongest independent predictor of HIV infection. A majority of the IDU have reduced risks of infection. However, current risk behaviour is common. Of all IDU, 48% reported

Seroepidemiology of viral infections among intravenous drug users in northern California.

Intravenous drug users are frequently exposed to parenterally transmitted viral infections, and these infections can spread to the general population through sexual activity. We investigated the prevalence of serologic markers for human immunodeficiency virus type 1 (HIV-1), human T-cell lymphotropic virus type I/II (HTLV-I/II), hepatitis B virus (HBV), and hepatitis C virus (HCV) in intravenous drug users and their sexual contacts. Of 585 drug users from northern California tested for these serologic markers, 72% were reactive for the antibody to HCV, 71% for the antibody to hepatitis B core

Trends in HIV infection among intravenous drug users in Innsbruck, Austria.

The first HIV test among intravenous drug users (IVDUs) at the AIDS clinic and/or the drug dependency clinic of the University of Innsbruck was the basis for the calculation of the proportions of those testing HIV seropositive annually over the period 1985-1990. The numbers testing HIV seropositive at the drug dependency clinic declined drastically, from 72.2% in 1985 to 12.5% in 1990 (chi 2 = 29.62, p less than 0.0001), whereas they rose at the AIDS clinic during this period, from 48.4% to 100% (chi 2 = 5.82, p = 0.016). Overall 132 of the 268 (49.2%) individuals examined tested HIV seroposit

Prescribing patterns for older heavy drug users living in the community.

We report an analysis of prescription drugs claimed under a government-sponsored, universal Pharmacare program for community-dwelling adults aged greater than or equal to 50 years in Manitoba during 1975, 1978, 1981, and 1984. We limited our analysis to claimants who reported over six drugs in a year in order to control for effects of inflation and a changing deductible. The median number of prescribed drugs and the percent of claimants over age 50 years receiving prescriptions from multiple physicians decreased during the course of our study. In this sample of heavy drug users, age and sex di

Drug use, health, family and social support in "crack" cocaine users.

The purpose of this study was to explore and describe "crack" cocaine use, health, family and social support in adult users. A nonprobability, convenience sample was used comprising 100 inpatients, 77 men and 23 women, from a drug treatment program. Gender differences, though not found in areas related to drug consumption, were found with respect to where and with whom subjects smoked, as well as perceptions of significant others. This initial, exploratory study should provide a foundation for future investigations concerning crack cocaine users.

Benign lymphoepithelial lesion of the parotid gland in intravenous drug users.

Fifteen partial parotid salivary gland resection specimens interpreted as benign lymphoepithelial lesion (BLL) were accessioned by our surgical pathology service between January 1983 and December 1986. Twelve of the specimens were removed from 11 prison inmates referred to our hospital, a patient subgroup constituting the source of only 2% of surgical pathology specimens in general. All prison inmates with BLL had a history of intravenous drug use, had developed unexplained lymphadenopathy concurrently with the parotid gland enlargement, and had suspected human immunodeficiency virus (HIV) inf
