Treatment with electromagnetic fields improves dual-task performance (talking while walking) in multiple sclerosis.

Multiple sclerosis (MS) is associated with an increased risk of falling resulting from visual disturbances, difficulties with gait and balance, apraxia of gait and peripheral neuropathy. These factors often interact synergistically to compromise the patient's gait stability. It has long been recognized that walking involves a cognitive component and that simultaneous cognitive and motor operations (dual-task) such as talking while walking may interfere with normal ambulation. Talking while walking reflects an example of a dual-task which is frequently impaired in MS patients. Impaired dual-tas

Imaging gait analysis: An fMRI dual task study.

INTRODUCTION: In geriatric clinical diagnostics, gait analysis with cognitive-motor dual tasking is used to predict fall risk and cognitive decline. To date, the neural correlates of cognitive-motor dual tasking processes are not fully understood. To investigate these underlying neural mechanisms, we designed an fMRI paradigm to reproduce the gait analysis. METHODS: We tested the fMRI paradigm's feasibility in a substudy with fifteen young adults and assessed 31 healthy older adults in the main study. First, gait speed and variability were quantified using the GAITRite© electronic walkway. Th

Effect of a dual task on quantitative Timed Up and Go performance in community-dwelling older adults: A preliminary study.

AIM: The Timed Up and Go test (TUG) is used as a measure of functional ability in older adults; however, the method of measurement does not allow us to determine which aspects of the test deficits occur in. The aim of the present study was to examine the ability of the quantitative TUG (QTUG) to measure performance during the TUG test under three different conditions - single task, motor task and cognitive dual task - and to compare performance between fallers and non-fallers in high-functioning community-dwelling older adults. METHODS: A total of 37 community-dwelling older adults, 16 with a

Differential effects and discriminative validity of motor and cognitive tasks varying in difficulty on cognitive-motor interference in persons with multiple sclerosis.

BACKGROUND: Cognitive-motor interference (CMI) has been well recognized in persons with multiple sclerosis (pwMS); however, there are limited data on effects of task difficulty. OBJECTIVE: Examine (1) the effects of motor and cognitive tasks varying in difficulty on the magnitude of CMI and (2) the discriminative validity of CMI between pwMS and healthy controls (HC). METHODS: Nine cognitive-motor dual-task (DT) conditions (combinations of three cognitive and three walking tasks) were examined. Outcome measures were DT-performance and dual-task cost (DTC) of gait parameters and correct answe

Visual but not verbal working memory interferes with balance in patients after optic neuritis suggestive of multiple sclerosis.

BACKGROUND: Subtle cognitive deficits are present in almost half of the patients with clinically isolated syndrome (CIS) suggestive of multiple sclerosis (MS). Similarly, subtle balance deficits can be detected at the earliest stages of the disease. To assess cognitive-motor interference (CMI) in nondisabled CIS patients, we studied postural performance using dual task paradigm in CIS patients presenting with optic neuritis. METHODS: We prospectively included 20 patients with visual acuity of 0.8 or more within the 3 months from unilateral ON. We also included 20 age, weight, height and educa

The effects of cognitive versus motor concurrent task on gait in individuals with transtibial amputation, transfemoral amputation and in a healthy control group.

BACKGROUND: Lower limb amputation causes difficulties in mobility together with motor and sensory loss. Challenging situations such as concurrent tasks cause gait parameters to deteriorate. Understanding the effect of concurrent tasks on gait is important for the rehabilitation of amputees. RESEARCH QUESTION: Are the effects of concurrent cognitive and motor tasks on gait parameters at fixed speed different in individuals with transtibial amputation, or transfemoral amputation compared to healthy individuals? METHODS: The gait parameters were evaluated of 20 individuals with transtibial ampu

Multiple Object Tracking While Walking: Similarities and Differences Between Young, Young-Old, and Old-Old Adults.

OBJECTIVE: Walking while simultaneously engaged in another activity becomes more difficult as one grows older. Here, we address the issue of changes in dual-task behavior at different stages of life, particularly in the latter stages. METHODS: We developed a dual task that combined walking along an 8-m walkway with a multiple object tracking (MOT) task of increasing difficulty. This secondary cognitive task imitates visuospatial daily activities and provides reliable quantitative measurements. Our dual-task paradigm was tested on 27 young adults (23.85 ± 2.09 years old) and two groups of olde

Dual-Task Gait Performance Following Head Impact Exposure in Male and Female Collegiate Rugby Players.

BACKGROUND: Gait impairments have been well-studied in concussed athletes. However, the sex-specific effect of cumulative head impacts on gait is not well understood. When a cognitive task is added to a walking task, dual-task gait assessments can help amplify deficits in gait and are representative of tasks in everyday life. Dual-task cost is the difference in performance from walking (single-task) to walking with a cognitive load (dual-task). PURPOSE: The objectives of this study were to explore the differences between sexes in 1) dual-task gait metrics, 2) gait metric changes from pre-seas

Reliability and Minimal Detectable Change for a Smartphone-Based Motor-Cognitive Assessment: Implications for Concussion Management.

Our purpose was to investigate the reliability and minimal detectable change characteristics of a smartphone-based assessment of single- and dual-task gait and cognitive performance. Uninjured adolescent athletes (n = 17; mean age = 16.6, SD = 1.3 y; 47% female) completed assessments initially and again 4 weeks later. The authors collected data via an automated smartphone-based application while participants completed a series of tasks under (1) single-task cognitive, (2) single-task gait, and (3) dual-task cognitive-gait conditions. The cognitive task was a series of continuous auditory Stroo

Recall and Recognition in Alzheimer's Disease and Frontotemporal Dementia.

BACKGROUND: It has long been debated whether performance on recall and recognition tests depends on the same or different memory systems and whether performance on these two tasks is dissociated in clinical populations. According to Dual process theories of recall, performance on recall and recognition tests dissociates in the relative reliance on frontal lobe related activities; in fact, the recall test requires more strategic retrieval of memoranda than the recognition task. By contrast, Dual process theories of recognition posit that performance on these tests differs in the relative contri

Technical skill training improves the ability to learn.

BACKGROUND: In the initial phases of surgical training, attentional resources are monopolized by the execution of novel tasks. This consideration can result in overwhelming attentional resources, leaving few for other tasks. Practicing a technical task decreases the attentional resources required for its execution. These resources are then free for redistribution to other, clinically important tasks. This pilot study investigated how laboratory-based surgical training on a technical procedure affects concurrent acquisition of other, nontechnical information. METHODS: After watching an instruc

Dual-task performance is associated with brain MRI Morphometry in individuals with mild cognitive impairment.

BACKGROUND AND PURPOSE: Cognitive impairment is a critical health problem in the elderly population. Research has shown that patients with mild cognitive impairment (MCI) may develop dementia in later years. Therefore, early identification of MCI could allow for interventions to help delay the progression of this devastating disease. Our objective in this study was to detect the early presence of MCI in elderly patients via neuroimaging and dual-task performance. METHODS: Brain MRI scans from 21 older adult volunteers, including cognitively healthy adults (HA, n = 9, age = 68-79 years) and mi

Turning as a single and dual task-associations with Alzheimer's and vascular pathologies in cognitively healthy older people.

BACKGROUND: Turning performance is an important and challenging motor activity, with potential relevance in several brain diseases. This study aimed to determine how Alzheimer's and vascular pathologies are associated with turning (360° while standing and 180° while walking), both as a single task and during dual-tasking in cognitively healthy older people. METHODS: Cognitively healthy older people (n = 297, mean age 77.7 years, 61.6% women) were included. A 180° turn was assessed using a wearable sensor; a 360° turn was assessed both clinically and using a sensor. Dual-tasking included a sim

Dual-task speech-in-noise performance in children with unilateral cleft lip and palate and minimal conductive hearing loss.

PURPOSE: Children with cleft lip and palate (CLP) frequently experience middle-ear dysfunction and conductive hearing loss, which may affect functional listening in noise. This study examined speech-in-noise recognition and visual-task latency cost during concurrent auditory-visual task performance in children with unilateral CLP and minimal conductive hearing loss. METHODS: Twenty-five children with unilateral CLP and minimal conductive hearing loss and 28 age-matched typically developing controls, aged 7-15 years, completed audiological assessment, a single-task speech-in-noise condition (S

Pupillometry provides a psychophysiological index of arousal level and cognitive effort during the performance of a visual-auditory dual-task in individuals with a history of concussion.

Research shows that concussions cause long-term deficits in executive functions when tested using challenging tasks with high cognitive load. The neurophysiological mechanism(s) associated with executive dysfunction are not well understood. Pupillometry provides a non-invasive index of arousal and cognitive load; therefore, the current study investigated whether pupillometry could help explain the persistent deficits in dual-task performance in individuals with a history of concussion (n = 14) compared to controls (n = 13). Participants were tested using a computerized Corsi block task which i

Affective theory of mind impairments linked with callous-unemotional traits implicate cognitive control: A pre-registered double-blind randomized controlled trial with a dual-task paradigm.

INTRODUCTION: Youth antisocial behavior strongly associates with conduct problems (CP) and callous-unemotional (CU) traits. While CP has links to broad cognitive impairments, CU traits have specific links with cognitive control and affective theory of mind (ToM) difficulties. Evidence suggests cognitive control limitations impact affective processing in ToM amongst youth with elevated CU traits. Here we sought to improve on those initial findings by leveraging a randomized dual-task (within-trial) design to replicate and extend prior findings. METHODS: In this double-blind, randomized control

Objective Dual-Task Turning Measures for Return-to-Duty Assessment After Mild Traumatic Brain Injury: The ReTURN Study Protocol.

Determining readiness for duty after mild traumatic brain injury (mTBI) is essential for the safety of service members and their unit. Currently, these decisions are primarily based on self-reported symptoms, objective measures that assess a single system, or standardized physical or cognitive tests that may be insensitive or lack ecological validity for warrior tasks. While significant technological advancements have been made in a variety of assessments of these individual systems, assessments of isolated tasks are neither diagnostically accurate nor representative of the demands imposed by

Between gait and cognition: an exploratory mixed-methods study of cognitive-motor dual-task profiles and lived experience.

BACKGROUND: Cognitive-motor dual tasking is widely studied. However, the results remain heterogeneous depending on the type of tasks and individual psychological factors. To address this variability, dual-task cost (DTC) frameworks allow classification of behavioral profiles. This study aimed to propose a method designed to identify individual determinants of dual-task (DT) behavior by combining quantitative gait and cognitive measures with qualitative analyses of lived experience. METHODS: Thirty-three young healthy adults performed Raven's Progressive Matrices Test (RPMT) and treadmill walk
