[Progressive dysarthria and bilateral sensory disturbance in a case of bilateral ventrolateral pontine infarction].

A rare case of bilateral ventrolateral pontine infarction in a 70-year-old man who developed progressive dysarthria and bilateral sensory disturbance is reported with literature review. He had been diagnosed with hypertension, dyslipidemia, and impaired glucose tolerance 10 years earlier. Ten days before admission, he was aware of the difficulty in walking and speaking, which gradually worsened. On admission he showed bilateral thermal hypoalgesia of face and lower extremities, dysarthria, dysphagia, and ataxic gait. High resolution three-dimensional MRI revealed bilateral ventrolateral pontin

A Cochrane review of treatment for dysarthria following acquired brain injury in children and adolescents.

BACKGROUND: The expression ''acquired brain injury'' (ABI) incorporates a range of etiologies including cerebrovascular accident, brain tumour and traumatic brain injury. ABI is a common cause of disability in the pediatric population, and dysarthria is a common and often persistent sequelae associated with ABI in children. OBJECTIVES: The aim of this study was to assess the efficacy of intervention delivered by Speech and Language Pathologists/Therapists targeting dysarthric speech in children resulting from acquired brain injury. METHODS: Several electronic databases were searched up to Ja

The cognitive competence of speakers with acquired dysarthria: judgements by doctors and speech and language therapists.

BACKGROUND: Studies have shown that listeners make negative attributions towards people with communication impairments. This appears to be the case for health care professionals as well as non-professional listeners. AIMS: This study extends this line of research to speakers with acquired dysarthria. These clients often complain that listeners treat them differently after the onset of their speech impairment. The study examines judgements of the cognitive status of speakers with acquired dysarthria made by health care professionals. METHODS AND PROCEDURES: Doctors, speech and language therap

A fuzzy cognitive map hierarchical model for differential diagnosis of dysarthrias and apraxia of speech.

This paper presents a novel soft computing system for differential diagnosis of the dysarthrias and apraxia of speech based on well accepted dysarthrias' classification system used by speech and language pathologists. The dysarthrias and apraxia are complex disorders of speech because they represent a variety of neurological disturbances that can potentially affect every component of speech production. Since an accurate diagnosis is a very challenging task for the clinician, the under development system based on hierarchical Fuzzy Cognitive Maps (FCMs) will be used as a "second opinion" or tra

Adapting the Bogenhausen Dysarthria Scales (BoDyS) to Chilean Spanish Speakers: Face and Content Validation.

Background: Dysarthria is a neuromotor speech disorder that significantly impacts patients' quality of life. In Chile, there is a lack of culturally validated instruments for assessing dysarthria. This study aimed to cross-culturally adapt the Bogenhausen Dysarthria Scales (BoDyS) into Chilean Spanish and to conduct face and content validation. Methods: The adaptation process included translation and back-translation, followed by validation by a panel of experts. Clarity, format, and length were evaluated, and the Kappa index (KI), content validity index (CVI), and content validity ratio (CVR)

Dysarthria Subgroups in Talkers with Huntington's Disease: Comparison of Two Data-Driven Classification Approaches.

Although researchers have recognized the need to better account for the heterogeneous perceptual speech characteristics among talkers with the same disease, guidance on how to best establish such dysarthria subgroups is currently lacking. Therefore, we compared subgroup decisions of two data-driven approaches based on a cohort of talkers with Huntington's disease (HD): (1) a statistical clustering approach (STATCLUSTER) based on perceptual speech characteristic profiles and (2) an auditory free classification approach (FREECLASS) based on listeners' similarity judgments. We determined the amou

Clinical Reasoning: A 73-Year-Old Woman With Episodic Dysarthria and Horizontal Binocular Diplopia.

A 73-year-old woman presented with transient episodes of dysarthria and horizontal diplopia. She had stereotactic radiosurgery 18 years prior for a retroclival meningioma. Neurologic examination was notable for right-sided tongue deviation, tongue fasciculations, and intermittent impaired abduction of the right eye. MRI ruled out recurrence or progression of the retroclival meningioma. EEG failed to reveal electrographic seizures. EMG showed spontaneous depolarizations in bursts that sounded like "marching soldiers" in the right hemitongue, consistent with myokymia. Focal myokymia is an unusua

Is Neuromuscular Electrical Stimulation Effective in Treating Severe Dysarthria: Insights from a Case Study.

BACKGROUND: Dysarthria refers to a group of disorders resulting from disturbances in neuromuscular control over the speech mechanisms due to damage of the central nervous system (CNS) or peripheral nervous system (PNS). Rehabilitation outcomes of dysarthria significantly depend on the collaboration skills of the patients. This case study aimed to investigate the potential role of neuromuscular electric stimulation (NMES) in improving severe dysarthria. An 18-year-old man affected by severe dysarthria following postanoxic brain injury underwent two different intensive rehabilitation trainings:

What speech can tell us: A systematic review of dysarthria characteristics in Multiple Sclerosis.

IMPORTANCE: Multiple sclerosis produces neurological impairments that are variable in duration, severity and quality. Speech is frequently impaired, resulting in decreased communication skills and quality of life. Advancements in technology now makes it possible to use quantitative acoustic assessment of speech as biomarkers of disease progression. OBSERVATIONS: Four domains of speech have been identified: articulation (slow articulation and imprecise consonants), voice (pitch and loudness instability), respiration (decreased phonatory time and expiratory pressure) and prosody (longer and fre

Non-speech oro-motor exercises in post-stroke dysarthria intervention: a randomized feasibility trial.

BACKGROUND: There has been little robust evaluation of the outcome of speech and language therapy (SLT) intervention for post-stroke dysarthria. Non-speech oro-motor exercises (NSOMExs) are a common component of dysarthria intervention. A feasibility study was designed and executed, with participants randomized into two groups, in one of which NSOMExs were a component of the intervention programme. AIMS: To examine (1) operational feasibility of the programme; (2) participants' views of the programme; and (3) speech intelligibility, communication effectiveness and tongue and lip movement at f

Disorders of nasality in subjects with upper motor neuron type dysarthria following cerebrovascular accident.

The nasality of 19 subjects with upper motor neuron (UMN) damage following cerebrovascular accident (CVA), and 19 control subjects matched for age and sex, was investigated using both perceptual judgements of nasality and a modified version of the nasal accelerometric procedure described by Horii (1980). Nasality indices were calculated for each subject during the production of a series of nasal and non-nasal sounds, words, and sentences. Statistical comparison of the two groups revealed that the CVA subjects had significantly higher nasality indices on the production of nonnasal speech tasks

Gelastic dysarthria: Speech-triggered pathological laughter with evidence for a selective pontine gating mechanism.

We report a highly selective manifestation of pathological laughter in which speech initiation consistently triggers laughter, resulting in disrupted articulation. A 55-year-old male with bilateral pontine infarction exhibited laughter that was time-locked to speech, most prominent during speech production and absent during non-speech tasks. Functional MRI during speech demonstrated activation of language regions, medial prefrontal cortex, amygdala, and ventral pons, with concurrent subjective laughter. We propose that ventral pontine lesions disrupt inhibitory pathways, leading to disinhibiti

Interventions for dysarthria due to stroke and other adult-acquired, non-progressive brain injury.

BACKGROUND: Dysarthria is an acquired speech disorder following neurological injury that reduces intelligibility of speech due to weak, imprecise, slow and/or unco-ordinated muscle control. The impact of dysarthria goes beyond communication and affects psychosocial functioning. This is an update of a review previously published in 2005. The scope has been broadened to include additional interventions, and the title amended accordingly. OBJECTIVES: To assess the effects of interventions to improve dysarthric speech following stroke and other non-progressive adult-acquired brain injury such as

Optic neuritis and palatal dysarthria as presenting features of post-infectious GQ1b antibody syndrome.

A 31-year-old man had optic neuritis 2 weeks after a diarrheal illness, followed by several deficits including palatal dysarthria, diplopia, ataxia, sensory dysfunction, and mild dysautonomia. Brain MRI and CSF were normal. Nerve conduction studies were initially normal and subsequently showed mild reduction in sensory amplitudes. Anti-GQ1b IgG titer was positive. Deficits resolved after treatment with IVIg. This clinical constellation represents an overlap between Miller Fisher syndrome (MFS) and the pharyngeal-cervical-brachial (PCB) variant of Guillain-Barre syndrome (GBS), along with the i

Behavioral Therapy for Tremor or Dystonia Affecting Voice in Speakers with Hyperkinetic Dysarthria: A Systematic Review.

INTRODUCTION: Hyperkinetic dysarthria is characterized by atypical involuntary movements within the speech mechanism that may affect the respiratory, laryngeal, pharyngeal-oral, or velopharyngeal-nasal subsystems and may alter speech production. Although articulatory impairments are commonly considered in hyperkinetic dysarthria, speakers with hyperkinetic dysarthria may also present with changes in voice quality, pitch, and loudness. In approximately 70% of speakers with hyperkinetic dysarthria, these voice alterations are associated with tremor or dystonia. The purpose of this systematic rev
