BACKGROUNDS AND PURPOSE: Microtia is a congenital malformation characterized by abnormal development of the external ear. Auricular reconstruction is the primary treatment for microtia. Because of the unique 3-dimensional anatomical structure of the auricle, the fabrication of patient-specific scaffolds remains one of the most technically demanding aspects of the reconstruction process. This review summarizes the literature and provides a comprehensive guide of the latest advances and breakthroughs in traditional autologous cartilage procedures and tissue-engineered ear scaffold technologies.

The authors describe their experience with the correction of prominent ears using a modified Chongchet technique. This method is based on the investigations of GIBSON and DAVIS (1958), who showed the tendency of cartilage to warp when one surface is cut. The prominent ear is folded back to produce the desired antihelix fold, which is outlined with methylene blue. An elliptical piece of retroauricular skin is excised. An incision is made through the whole thickness of the cartilage, following the posterior marked linea of the new antihelical fold, from the superior crus to the tail of antihelix

BACKGROUND: The remnant ear is a very important material in auricular reconstruction surgery; its dimension, position, and shape determine how it is used. However, the local conditions of microtia patients are complex and variable. Situations may be encountered where a series of abnormal remnant ears cannot be utilized in clinical practice. Currently, there are no literature that elaborates on this type of microtia and provides systematic treatment methods. The purpose of this article is to systematically classify them and optimize the two-stage method auricular reconstruction, to provide an e

Various materials such as fascia, perichondrium, and cartilage have been used for reconstruction of the tympanic membrane in middle ear surgery. Because of its stiffness, cartilage is resistant to resorption and retraction. However, cartilage grafts result in increased acoustic impedance, the main limitation to their use. The aim of this study was to compare the hearing results after cartilage tympanoplasty versus fascia tympanoplasty. This study included 114 patients without postoperative tympanic membrane perforation who underwent tympanoplasty type I between 2007 and 2010, 31 with fascia an

Perichondritis of the pinna is a serious late infection of burns involving the ear. The avascular nature of this injury renders systemic antibiotic therapy ineffective in delivering adequate levels of drug to the area to provide antimicrobial activity. Treatment has traditionally been surgery in some form, resulting in further disfigurement of the damaged cartilagenous framework. Iontophoresis, a method of dispersing charged particles through a relatively impermeable membrane by a light electrical field, appears to be a feasible method of antibiotic delivery. This initial pilot study investiga

In an experimental study the cartilaginous protential of the rabbit ear perichondrium has been compared with that of the rib in vivo and in vitro. Perichondrium was transferred as free autologous grafts to the subcutaneous tissue on the scalp and as loose bodies into the knee joint. The presence of cartilage in the grafts was examined after six weeks. In vitro explants of rabbit perichondrium from the ear and the rib were maintained in an organ culture system. The presence of cartilage was analyzed after one to three weeks. Rabbit perichondrium from the rib appeared to have a greater cartilagi

The authors report their experience in the surgical treatment of 78 patients with lop, prominent, or protruding ears. Four patients had a unilateral deformity. The procedure used in 110 ears consisted of a cartilage excision-incision and horizontal mattress suture technique. In 42 ears, horizontal mattress sutures only, with or without scoring of cartilage, were used. The complications and unsatisfactory cosmetic results are reported. No major complications such as perichondritis, severe infection or unsightly postoperative deformities occurred. In conclusion, 94,7% of all cases had a good cos

BACKGROUND: The adhesive perforation could be the sequela of adhesive otitis media, that partial tympanic membrane remnant is bound completely to the medial wall of the middle ear by fibrous adhesions. However, few studies have reported on the repairing of adhesive perforation.
OBJECTIVES: To evaluate the long-term outcome of endoscopic full-thickness cartilage-perichondrium double graft myringoplasty for adhesive perforation.
MATERIALS AND METHODS: In total, 26 patients with unilateral adhesive perforation associated with chronic otitis media who underwent full-thickness cartilage-perichond

Transplantation of bioengineered elastic cartilage is considered to be a promising approach for patients with craniofacial defects. We have previously shown that human ear perichondrium harbors a population of cartilage progenitor cells (CPCs). The aim of this study was to examine the use of a rotating wall vessel (RWV) bioreactor for CPCs to engineer 3-D elastic cartilage in vitro. Human CPCs isolated from ear perichondrium were expanded and differentiated into chondrocytes under 2-D culture conditions. Fully differentiated CPCs were seeded into recently developed pC-HAp/ChS (porous material

Acute external ear lesions: clinical aspects, assessment and management. We reviewed the literature concerning the assessment and the management of the external ear traumas, which is not very rich. Nevertheless, we outlined the practical attitudes in the four major conditions met: the auricular haematoma, the auricular perichondritis, the auricular laceration and auricular bums. All these pathologies must be promptly treated because there is a risk of perichondritis, which can destroy the cartilage and will result in a severely deformed ear. Auricular haematomas must be drained as soon as poss

Tissue-engineered ear cartilage scaffold based on three-dimensional (3D) bioprinting technology presents a new strategy for ear reconstruction in individuals with microtia. Natural hydrogel is a promising material due to its excellent biocompatibility and low immunogenicity. However, insufficient mechanical property required for cartilage is one of the major issues pending to be solved. In this study, the gelatin methacryloyl (GelMA) hydrogel reinforced with bacterial nanocellulose (BNC) was developed to enhance the biomechanical properties and printability of the hydrogel. The results reveale

Paediatric Chronic Suppurative Otitis Media (CSOM) remains a challenge for the treating otologists. Usually, concurrent foci of infection lie in the oropharynx or nasopharynx and treating these eventually leads to resolution of ear discharge. However, refractory cases can be threatening in view of complications secondary to CSOM. A two year and 10-month-old child presented with recurrent profuse mucopurulent ear discharge which had been refractory to multiple medical treatment. Recently she had recurrent otitis external episodes with excoriations of canal skin. Child underwent bilateral tympan

BACKGROUND: Total ear amputation is a relatively rare trauma with an absolute indication for surgical treatment. Numerous techniques for auricular reconstruction have been described. When local and general conditions allow microsurgical replantation, this must be the first choice. We propose the association of microsurgical techniques with some modification (modified Baudet technique) to obtain higher survival rate of the reimplanted stump.
METHODS: This study included cases of 3 male patients with total ear amputation, the injuries and their mechanism (workplace accident) being identical. Ch

BACKGROUND: The pterional incision is usually performed near the course of the superficial temporal artery (STA), which carries a risk of injury to a branch or even the main trunk of the STA (mSTA). In this study, we assessed the usual course of the mSTA and its parietal branch of the STA (pSTA) and evaluated the efficacy of a modified pterional scalp incision for the preservation of all STA branches.
MATERIALS AND METHODS: Sixteen sides of cadaveric heads were dissected to study the location and paths of the mSTA and pSTA in the vicinity of the ear cartilage and the oculomeatal (OM) line. We

After the auricular cartilage of 5-week-old rabbits was bent and fixed by suture, the cartilage increased in thickness in a short period of time. The thickness was maximal 6 weeks later and slightly greater than that of the control cartilage 12 weeks later. At this stage, the auricle was fixed in curved form. There was no difference in the morphology of cartilage cells between the bent group and the control. Tape attachment therapy is very simple. We propose that surgical treatment should be designed after a few months of tape attachment therapy. In the present work, the effect of taping was g

Cystic chondromalacia of the auricle is an uncommon condition in which a degenerative process occurs within the cartilage. The disorder affects young and middle-aged people. Clinically, it manifests as a painless, fluctuant swelling that frequently relapses despite various therapeutic approaches. In this article we report a typical case of cystic chondromalacia of the auricle that was successfully treated by surgery-specifically, dual-plane dissection-and we briefly review the literature.
