Design of an Early Exercise Program for Hospitalized Patients: "Let's Move".

BACKGROUND: Hospital-associated deconditioning (HAD) affects up to 30% of older adults during acute hospitalization, leading to functional and cognitive decline and adverse long-term outcomes. PURPOSE: To develop and pilot "Let's Move," an implementation-oriented early exercise program designed to support mobility in hospitalized patients. METHODS: An action-research study was conducted at a tertiary hospital in two stages: (i) program design based on an electronic health record (EHR) needs assessment and targeted protocol review; and (ii) pilot implementation among clinically stable adults

Early Out-of-Bed Rehabilitation After Endovascular Thrombectomy: A Randomized Controlled Trial.

OBJECTIVES: To determine whether out-of-bed rehabilitation within 48 hours after endovascular thrombectomy (EVT) for large-vessel occlusion improves functional outcomes and motor recovery and to evaluate the safety of neurologic and immobility-related complications. DESIGN: A randomized controlled trial. SETTING: A tertiary medical center. PARTICIPANTS: Forty-two patients (N=42) who underwent EVT for anterior-circulation large-vessel occlusion and were stable within 48 hours after reperfusion were randomly assigned to early out-of-bed mobilization (EM; n=21) or usual care (UC; n=21) groups.

Development and Internal Validation of a Prognostic Model for Delayed Ambulation Recovery After Major Abdominal Surgery in Northwest Ethiopia. A Prospective Multicenter Study.

BACKGROUND: Delayed ambulation recovery following major abdominal surgery presents a significant prognostic challenge, contributes to increased morbidity, mortality, and prolonged hospital stay. Despite recommendations advocating early mobilization, many patients, especially in low-resource settings, experience delayed recovery, and practical risk prediction models are lacking to identify high-risk patients to implement targeted interventions and personalized care. OBJECTIVE: To develop and internally validate a prognostic model predicting delayed ambulation recovery on postoperative day one a

Ultra-Early Mobilization After Split-Thickness Skin Grafting in Burn Patients is not Associated with Graft Loss.

Timing of mobilization following split-thickness skin grafting (STSG) in burn patients varies amongst burn centers, due to concerns for graft loss with earlier postoperative mobilization. Existing literature supports the feasibility of earlier mobilization after STSG but is limited in scope. This study aimed to explore the association between time to mobilization after STSG and graft loss, hypothesizing that ultra-early mobilization within 24 h of STSG does not increase the risk of graft loss. Adult patients who underwent STSG for treatment of thermal burn injuries at an American Burn Associat

What Do Patients Fear on Their First Out-of-Bed Mobilisation Following Open-Heart Surgery? A Phenomenological Exploration.

BACKGROUND: Patients' early experiences with mobilisation are crucial for preventing muscle weakness, enhancing respiratory function, supporting the cardiovascular system and accelerating the recovery process by avoiding complications associated with immobility. However, nurses sometimes encounter difficulties due to patients' reluctance to mobilise. AIM: To explore patients' experiences during their first out-of-bed mobilisation after open-heart surgery and to comprehend the underlying emotions associated with this experience. STUDY DESIGN: Participants were recruited using purposeful sampl

Feasibility of repeated early mobilization in orthostatic intolerance after total hip arthroplasty.

INTRODUCTION: Orthostatic intolerance (OI) is a barrier for early mobilization and same-day discharge after total hip arthroplasty (THA), with an estimated 30 % of patients affected within the first 6 h after surgery. Since repeated mobilization is known to be valuable in non-surgical OI conditions, we aimed to evaluate the feasibility of an early standardized repeated mobilization procedure as treatment of postoperative OI after THA. METHODS: This study was a single-arm, two-center hypothesis-generating feasibility study in patients undergoing THA. At first mobilization 4 h after surgery pat

"I will get out of this" - The patients' experiences of early mobilisation in intensive care. A hermeneutic study.

BACKGROUND: The significance of early mobilisation in intensive care has become increasingly apparent along with a growing understanding of patient experiences within this critical setting. However, there is still a need for more knowledge regarding the complex experiences of the patients. Therefore, this study aimed to gain an in-depth understanding of the significance and deeper meaning of early mobilisation in patients recently treated in intensive care. METHODS: A qualitative study with a hermeneutic, interpretive approach. Semi-structured interviews were conducted with 30 participants re

The Evolution of Geriatric Ankle Fracture Management to Promote Mobility.

Geriatric ankle fracture management requires a comprehensive approach that balances appropriate patient optimization, fracture reduction, stability, and early mobilization. Surgical and nonsurgical options should be cautiously considered based on patient and fracture characteristics. Early weight-bearing treatment options should be pursued when possible. There are a myriad of treatment options including fibular intramedullary devices or minimally invasive joint prep tibiotalocalcaneal nailing that minimize soft tissue disruption and can provide robust stability sufficient to allow early weight

Early mobilization in coronavirus-19 patients treated with extracorporeal membrane oxygenation.

BACKGROUND: Coronavirus disease 2019 (COVID-19) pneumonia can be associated with refractory respiratory failure requiring extracorporeal membrane oxygenation(ECMO). Although ECMO has helped many COVID patients, optimal management strategies for these patients remain unknown. METHODS: We conducted a retrospective review of all COVID patients requiring ECMO at our hospital. Six months into the pandemic, we changed our management strategy to focus on early mobilization. The early mobilization effort included tracheostomy within 48 h of cannulation, decreasing sedation, and an aggressive physical

Nail plate combination in fractures of the distal femur in the elderly: A new paradigm for optimum fixation and early mobilization?

Distal femoral fractures in elderly or osteoporotic patients constitute a challenging injury, especially in the settings of fracture comminution or periprosthetic fractures. A recent trend in the treatment of these difficult injuries is the double fixation with a nail and a plate, a strategy that comes with advantages including faster weight-bearing and lower risk for non-union. Although biomechanical studies have demonstrated the superiority of nail-plate constructs, there is a paucity in the literature regarding the indications and results of treatment. A review of the literature to date was

Ensuring Early Mobilization Within an Enhanced Recovery Program for Colorectal Surgery: A Randomized Controlled Trial.

OBJECTIVE: To estimate the extent to which the addition of staff-directed facilitation of early mobilization to an Enhanced Recovery Program (ERP) impacts recovery after colorectal surgery, compared with usual care. SUMMARY BACKGROUND DATA: Early mobilization is considered an important component of ERPs but, despite guidelines recommendations, adherence remains quite low. The value of dedicating specific resources (eg, staff time) to increase early mobilization is unknown. METHODS: This randomized trial involved 99 colorectal surgery patients in an established ERP (median age 63, 57% male, 8

Interventions for treating chronic ankle instability.

BACKGROUND: Chronic lateral ankle instability occurs in 10% to 20% of people after an acute ankle sprain. Initial treatment is conservative but if this fails and ligament laxity is present, surgical intervention is considered. OBJECTIVES: To compare different treatments, conservative or surgical, for chronic lateral ankle instability. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, CINAHL and reference lists of articles, all to February 2010. [Growth modulation in operative treatment of juvenile scoliosis by USS paediatric].

OBJECTIVE: Growth modulation in operative treatment of juvenile scoliosis can be done by USS paediatric instrumentation to control spinal growth in patients of small stature with juvenile scoliosis. The double-rod system has to be distracted every 4-6 months. The system is used in young patients too tall for VEPTR (vertical expandable prosthetic titanium rib) instrumentation. The system with a very low profile allows reduced soft-tissue pressure saving soft tissue from atrophy or the development of pseudocysts above the screws. With this procedure controlled growth with growth modulation of th

Guidelines for prevention of venous thromboembolism in immobile patients secondary to neurological impairment.

BACKGROUND: Venous thromboembolism (VTE) is a major cause of morbidity and mortality in hospitalized patients and 7% of these cases are due to immobility secondary to a neurological impairment. Many guidelines are available to guide clinicians dealing with medical or surgical patients. However, and with the exception of spinal injuries, no guidelines are available to deal with other neurologically impaired patients at risk of VTE. AIM: Our study aimed at gathering evidence from the literature to enable us to deal with the main controversial issues of VTE prevention. Guidelines will be formula

[Rehabilitation after osteoporosis-induced fracture. Getting your patient quickly back on her feet].

Appropriate rehabilitation is essential for patients with osteoporotic fracture, since it not only significantly improves their quality of life, but, in many cases, may even prevent a patient becoming a candidate for the nursing home, thus considerably reducing the costs to the social system. Rehabilitation of patients with osteoporosis includes adequate pain treatment, early mobilization, specific training of muscles and coordination, instruction on how to avoid falls, on nutrition, ergotherapy and sports therapy including informing the patient how to remain permanently active without outside

Bilateral posterior hip dislocations with femoral head fractures.

An unusual case of bilateral posterior fracture-dislocation of the hip (Pipkin Type IV) occurred in a 63-year-old man with Paget's disease of the pelvis. Other injuries included a displaced humeral shaft fracture and patellar ligament disruption. Bilateral cemented total hip arthroplasty was performed to avoid the need for prolonged immobilization. Postoperative low-dose irradiation was used because of the risk of heterotopic ossification.
