Varying conceptions of competence: an analysis of how health sciences educators define competence.

CONTEXT: Current debate in medical education focuses on the nature of 'competency-based medical education' (CBME) and whether or not it should be adopted. Many medical schools claim to run 'competency-based' curricula, but the structure of their programmes can differ radically. A review of the existing CBME literature reveals that little attention has been paid to defining the concept of competence. A straightforward examination of what is meant by the term 'competence' is noticeably missing from the literature, despite its impact on medical training. OBJECTIVES: This paper aims to illustrate

[Who will be a good physician? Admission procedures for medical and dental students].

Since 2005, German universities are free to select 60% of their freshmen according to their own admission processes. In 2008, selection of medical students in Germany was mainly based on grades achieved in final school examinations (Abiturnote). Further criteria were used in various combinations: some medical schools conducted interviews or tests, while others rewarded work experience, research awards, or cultural and social dedication. However, solely high school grades and some measures of ability show acceptable validity coefficients with regard to academic and professional success. Evidenc

The High Clinic: a pilot project of a new model for an outpatient, community-based teaching clinic in rheumatology.

The objectives were to develop an outpatient, community-based clinical education experience for second-year rheumatology fellows based on sound education principles, and to pilot the program to test its practicability and assess student perception of its educational efficacy. The outpatient clinical education experience consisted of four steps. In Step 1 second-year rheumatology fellows assessed patients and reviewed their cases with two supervisors. Step 2 entailed each fellow presenting a brief summary of each case to their colleagues and supervisors for discussion and analysis of learning i

Dyslexic doctors: a resource in need of discovery.

Dyslexic students are applying for admission to medical school. In most cases they have worked hard in college, and their credentials are good (with the likely exception of a low score on Skills Analysis--Reading on the Medical College Admissions Test). In the past a number of well known dyslexics have made substantial contributions in various disciplines, including a few in medicine. Without appropriate assistance, many dyslexic students will be unable to compete successfully and to complete medical school. With appropriate assistance, however, they can complete medical school and become comp

Are deaf students' reading challenges really about reading?

Reading achievement among deaf students typically lags significantly behind hearing peers, a situation that has changed little despite decades of research. This lack of progress and recent findings indicating that deaf students face many of the same challenges in comprehending sign language as they do in comprehending text suggest that difficulties frequently observed in their learning from text may involve more than just reading. Two experiments examined college students' learning of material from science texts. Passages were presented to deaf (signing) students in print or American Sign Lang

Comparison of Artificial Intelligence Chatbots and Dental Students on Context of Dental Trauma.

AIM: Daily use of chatbots has become widespread in dental education just as in every other field. Dental students consult them when performing tasks and studying for exams. This study aims to test comprehension abilities and knowledge of 5 different chatbots (Gemini 1.5/1.5Pro, ChatGPT 4.0/4.0o, and Deepseek) and compare them with students' at the topic of dental trauma. Beyond exam accuracy: Tracking a persistent-failure set reveals visual dental reasoning gaps in multimodal LLMs.

OBJECTIVES: To benchmark late-2025 general-purpose multimodal large language models (LLMs) on the Japanese National Dental Examination (JNDE) and to reassess a previously identified persistent-failure set. METHODS: GPT-5.2T, Claude 4.5, and Gemini 3 were tested in January 2026 using a zero-shot protocol (no prompt engineering) on 350 publicly released JNDE-2025 questions (202 text-only; 148 visually-based) and on 33 questions in a persistent-failure set that had been answered incorrectly by all models in a prior JNDE-2024 evaluation. Responses were scored against the official answer key; pair

Three large language models demonstrate competitive performance in Traditional Chinese Medicine national medical licensing examinations over two years.

Large language models (LLMs) are increasingly integrated into medical education and assessment. However, their performance in high-stakes, non-English medical licensing examinations-particularly within culturally distinct medical systems such as Traditional Chinese Medicine (TCM)-remains insufficiently evaluated. This study aimed to systematically compare the performance of Gemini 3 Pro, DeepSeek V3.1, and GPT-5.2 in the Chinese National Traditional Chinese Medicine Licensing Examination (TCMLE) over two consecutive years (2023 and 2024), and to assess their potential implications for TCM educ

Visualizing cognitive processes in medical education: Forward and backward reasoning in a digital family medicine assessment.

BACKGROUND: When evaluating medical students, wouldn't it be helpful to visualize and understand your students' clinical reasoning (CR)? There are different types of CR for students to use in their multiple-choice questions (MCQs). While forward reasoning uses data to generate a hypothesis, backward reasoning looks at possible prompts (answers) to generate a hypothesis. This study implements a new approach to visualizing CR during digital MCQ assessment. Furthermore, it examines the effect of feedback given during the learning process, also known as formative feedback, on the thought process.

The NEST: An upstream approach to student success.

BACKGROUND: Pre- and post-licensure nursing students often face academic challenges in high stakes courses such as pathophysiology and pharmacology. To promote nursing program completion and timely graduation, it is important that students achieve success in these courses. PURPOSE: Recognizing the need to provide comprehensive academic support to students enrolled in pathophysiology and pharmacology, one school of nursing implemented an academic excellence initiative called NEST (Nursing Excellence through Scholarship and Technology). METHOD: The NEST program combines targeted academic inter

Validation of a specific competency and sub-competencies in interculturality for Health Sciences programs.

UNLABELLED: The provisions of the Sustainable Development Goals and the World Health Organization indicate that progress must be made toward equity to improve people's lives. In Chile, the achievement of intercultural competence in professional training in health sciences is still pending. The Tuning project indicates that professional training should be oriented toward a competency-based model to ensure a graduate profile that is relevant to the context. AIM: To validate a specific competency and sub-competencies in interculturality for Health Sciences degree programs. METHODS: Descriptive

Evolution of AI in anatomy education study based on comparison of current large language models against historical ChatGPT performance.

The integration of Large Language Models (LLMs) in medical education has gained significant attention, particularly in their ability to handle complex medical knowledge assessments. However, a comprehensive evaluation of their performance in anatomical education remains limited. To evaluate the performance accuracy of current LLMs compared to previous versions in answering anatomical multiple-choice questions and assessing their reliability across different anatomical topics. We analyzed the performance of four LLMs (GPT-4o, Claude, Copilot, and Gemini) on 325 USMLE-style MCQs covering seven a

Effectiveness of Different Teaching Modalities in Human Gross Anatomy Education.

With increasing cost and time constraints, anatomy education has shifted away from classic dissection in favor of more effective teaching modalities. Here we investigated different teaching modalities on test performance and student satisfaction in the human gross anatomy laboratory at the New York Institute of Technology College of Osteopathic Medicine (NYITCOM). The anatomy laboratory for first-year osteopathic students at NYITCOM was divided into stations including radiology, case-based learning, prosection, and dissection. Content covered at those stations was tested via anatomy laboratory

Cheating 2.0: A reprofiling of the 10 most wanted test cheaters in the digital age.

In the last issue of Medical Education Unleashed, Royal et al (2016) introduced the "Ten Most Wanted" test cheaters. Almost a decade later, significant changes in assessment format and delivery have altered the landscape of academic dishonesty. This paper revisits the original 10 cheaters, updates their statuses and identifies new contenders. Cheating remains a pervasive issue in medical education, exacerbated by the shift to online assessments during the COVID-19 pandemic. In medical education, cheating threatens assessment validity, patient safety and institutional reputation, and may correl
