Can a volunteer-based service create an effective, locally appropriate and scalable model of prehospital care for traffic injury victims on the national highways of Bangladesh? Operational outcomes after 10 years of TraumaLink.

INTRODUCTION: Road traffic injuries continue to be a leading killer worldwide, but many developing countries lack formal prehospital services. This leaves victims without access to care when it can be most effective in preventing death and disability and contributes to the disproportionate burden of deaths and injuries on the roads in low-income and middle-income countries. METHODS: To address the lack of prehospital care in Bangladesh, we developed TraumaLink. The service uses an emergency hotline number and 24-hour call centre with local first responders trained in basic trauma first aid, p

Clinical features of 1487 COVID-19 patients with outpatient management in the Greater Paris: the COVID-call study.

Clinical features of COVID-19 have been mostly described in hospitalized patients with and without ICU admission. Yet, up to 80% of patients are managed in an outpatient setting. This population is poorly documented. In France, health authorities recommend outpatient management of patients presenting mild-to-moderate COVID-19 symptoms. The aim of this study was to describe their clinical characteristics. The study took place in an emergency medical dispatching center located in the Greater Paris region. Patients included in this survey met confirmed COVID-19 infection criteria according to the

Suspicion and treatment of severe sepsis. An overview of the prehospital chain of care.

BACKGROUND: Sepsis is a life-threatening condition where the risk of death has been reported to be even higher than that associated with the major complications of atherosclerosis, i.e. myocardial infarction and stroke. In all three conditions, early treatment could limit organ dysfunction and thereby improve the prognosis. AIM: To describe what has been published in the literature a/ with regard to the association between delay until start of treatment and outcome in sepsis with the emphasis on the pre-hospital phase and b/ to present published data and the opportunity to improve various lin

Interobserver variability in preclinical assessment of collision variables following traffic accidents.

PURPOSE: Traffic accidents persist as a leading cause of death. European law mandates the integration of automatic emergency call systems (eCall). Our project focuses on an automated injury prediction device for car accidents, correlating technical and epidemiological input data, such as age, gender, seating position, impact on the passenger compartment, seatbelt usage, impact direction, EES, vehicle class, and airbag deployment. This study aims to explore interobserver variability in data collection quality in real accident scenarios. The assessment will evaluate the impact of user training a

Rural and Ethnic Disparities in Out-of-hospital Care and Transport Pathways After Road Traffic Trauma in New Zealand.

INTRODUCTION: The out-of-hospital emergency medical service (EMS) care responses and the transport pathways to hospital play a vital role in patient survival following injury and are the first component of a well-functioning, optimised system of trauma care. Despite longstanding challenges in delivering equitable healthcare services in the health system of Aotearoa-New Zealand (NZ), little is known about inequities in EMS-delivered care and transport pathways to hospital-level care. METHODS: This population-level cohort study on out-of-hospital care, based on national EMS data, included traum

Treatment of Diseases in the Civil Society of the Han Empire.

The separation of medicine and shamanism, which began during the Spring and Autumn Period, led to the emergence of "excellent physicians" who were distinct from shamans, a trend that continued from the Warring States Period through the early Han Dynasty. However, while there were some instances of these highly skilled physicians treating commoners, their medical practice was primarily concentrated among the ruling elite-including rulers, high-ranking officials, and those associated with them. Furthermore, physicians with outstanding treatment records in the general populace were sought after b

"They made me feel like I mattered": a qualitative study of how mobile crisis teams can support people experiencing homelessness.

BACKGROUND: Mobile crisis teams (MCTs) can be important alternatives to emergency medical services or law enforcement for low-acuity 911 calls. MCTs address crises by de-escalating non-violent situations related to mental health or substance use disorders and concurrent social needs, which are common among people experiencing homelessness (PEH). We sought to explore how an MCT in one city served the needs and supported the long- and short-term goals of PEH who had recently received MCT services. METHODS: We conducted 20 semi-structured interviews with service recipients of the Street Crisis R

A reassessment and review of the Bam earthquake five years onward: what was done wrong?

INTRODUCTION: An earthquake measuring 6.6 on the Richter scale on 23 December 2003 devastated the city of Bam in southeastern Iran. During the response and recovery phases, considerable shortcomings were discovered. The dire situation in the affected area, a variety of urgently required interventions, and the large number of aid organizations involved brought about difficulties in management, coordination, and communication among authorities and aid organizations. This article highlights flaws in management in the various aspects of this disaster in order to assess what was done, and what shou

Patient experience of severe acute dyspnoea and relief during treatment in ambulances: a prospective observational study.

BACKGROUND: Acute dyspnoea is common among ambulance patients, but little is known of the patients' experience of symptom. We aimed to investigate ambulance patients initial perceived intensity of acute dyspnoea, and whether they experienced relief during prehospital treatment. Furthermore, to investigate the validity and feasibility of using a subjective dyspnoea score in the ambulance, and its association with objectively measured vital signs. METHODS: We performed a prospective observational study in the North Denmark Region from 1. July 2017 to 30. March 2019. We studied patients over the

Out-of-hospital cardiac arrest surveillance --- Cardiac Arrest Registry to Enhance Survival (CARES), United States, October 1, 2005--December 31, 2010.

PROBLEM/CONDITION: Each year, approximately 300,000 persons in the United States experience an out-of-hospital cardiac arrest (OHCA); approximately 92% of persons who experience an OHCA event die. An OHCA is defined as cessation of cardiac mechanical activity that occurs outside of the hospital setting and is confirmed by the absence of signs of circulation. Whereas an OHCA can occur from noncardiac causes (i.e., trauma, drowning, overdose, asphyxia, electrocution, primary respiratory arrests, and other noncardiac etiologies), the majority (70%--85%) of such events have a cardiac cause. The ma

Management of prehospital thrombolytic therapy in ST-segment elevation acute coronary syndrome (<12 hours).

UNLABELLED: Acute myocardial infarction (AMI) is the prototype of a real emergency, and both efficacy and speed are necessary for effective management. The advent of thrombolysis therapy has transformed the care of these patients. In fact, the most frequent complication of AMI is sudden death which still occurs within the first hour after symptom onset. Thrombolytic therapy has been shown to reduce early and long term mortality about 20%. The mortality gain is dependent on the delay time of early reperfusion. A large number of studies have shown that this relationship is best described as expo

Critical older driver errors in a national sample of serious U.S. crashes.

OBJECTIVE: Older drivers are at increased risk of crash involvement per mile traveled. The purpose of this study was to examine older driver errors in serious crashes to determine which errors are most prevalent. METHODS: The National Highway Traffic Safety Administration's National Motor Vehicle Crash Causation Survey collected in-depth, on-scene data for a nationally representative sample of 5470 U.S. police-reported passenger vehicle crashes during 2005-2007 for which emergency medical services were dispatched. There were 620 crashes involving 647 drivers aged 70 and older, representing 25

Impact of the emergency medical services crew size (2 vs. ≥3 personnel) on prehospital performance and survival in severe trauma: a nationwide cohort study in Korea.

PURPOSE: Timely and coordinated prehospital interventions are critical to improving outcomes in patients with severe trauma. In Korea, national discussions are underway to expand emergency medical services (EMS) crews from the standard two-person configuration to three, yet nationwide evidence supporting this policy remains limited. This study aimed to evaluate whether EMS crew size (2 vs. ≥3 personnel) is associated with differences in prehospital care performance and survival outcomes among patients with severe trauma, accounting for potential confounding factors, including the COVID-19 peri

En Route Resuscitation - Utilization of CCATT to Transport and Stabilize Critically Injured and Unstable Casualties.

INTRODUCTION: The U.S. Air Force utilizes specialized Critical Care Air Transport Teams (CCATT) for transporting "stabilized" patients. Given the drawdown of military forces from various areas of operation, recent CCATT operations have increasingly involved the evacuation of unstable and incompletely resuscitated patients from far forward, austere locations. This brief report describes unique cases representative of the evolving CCATT mission and provides future direction for changes in doctrine and educational requirements in preparation for en route combat casualty care. Centiles for the shock index among injured children in the prehospital setting.

OBJECTIVE: The shock index (SI), the ratio of heart rate to systolic blood pressure, is a clinical tool for assessing injury severity. Age-adjusted SI models may improve predictive value for injured children in the out-of-hospital setting. We sought to characterize the proportion of children in the prehospital setting with an abnormal SI using established criteria, describe the age-based distribution of SI among injured children, and determine prehospital interventions by SI. METHODS: We performed a multi-agency retrospective cross-sectional study of children (<18 years) in the prehospital se

Selected Impacts of Urban Heat Islands on Emergency Medical Services Utilization in Rhode Island.

INTRODUCTION: Excessive environmental heat exposure is clearly associated with an increased likelihood that individual patients will suffer adverse health outcomes. Such heat exposure also strains healthcare systems via increased utilization, a burden which can challenge systems' capacities. Health impacts vary geographically with urban heat islands potentially contributing to higher temperatures and greater health risks. However, those most vulnerable to this exposure are not well identified. Our objective in this novel study was to compare and quantify differences in emergency medical servic

Beyond the Algorithm: The ERC Guidelines 2025 and the Maturity of Resuscitation Medicine.

The European Resuscitation Council Guidelines 2025, officially launched as the Guidelines on Cardiopulmonary Resuscitation 2025 on 22 October 2025 in Rotterdam and published in Resuscitation (1) as a thematic collection of guideline papers, represent far more than a scheduled update of resuscitation algorithms. That reading would be too narrow. The ERC Guidelines 2025 are better understood as a portrait of a discipline that has entered a new phase in its understanding of the continuum of resuscitation care. In these guidelines, resuscitation medicine is not reduced to the dramatic moment of ca
