Extragonadal germ cells tumors can arise primarily in the retroperitoneum. It has been suggested that these neoplasms might be metastasis from an occult testicular lesion which would have regressed later. The cause of testicular cancer remains controversial and thought to be multifactorial. However, more recent data show that carcinogenic potential of altered testicular environment might induce cancer and that early surgical correction might decrease the chance of cancer development. Two cases of germ cell tumors (GCTs) of the basal ganglia are presented and 40 previously reported cases are reviewed. The incidence of GCTs of the basal ganglia and thalamus was estimated as less than 14% of all intracranial GCTs. All patients except for two (95%) were male, aged 7-19 years.
