Papillary adenocarcinoma occurring in a gastric hyperplastic polyp observed by magnifying endoscopy and treated with endoscopic mucosal resection.

The patient was a 75-year-old man. He had been diagnosed with gastric hyperplastic polyp 4 years previously. The color of the apex of this polyp was whitish. Magnifying endoscopy findings revealed disappearance of the mucosal microstructure with irregular branched capillaries at the top of the polyp. Endoscopic mucosal resection (EMR) was performed. Histological examination revealed that a part of the polyp surface was replaced with papillary adenocarcinoma. Diagnosis of papillary adenocarcinoma in a hyperplastic polyp with mucosal invasion was made. Magnifying endoscopy was useful for the det

Risk Stratification Model for Surveillance Interval Extension After Hot Endoscopic Mucosal Resection of Large Non-Pedunculated Colon Polyps.

BACKGROUND: Current guidelines recommend uniform 6-month surveillance after endoscopic mucosal resection (EMR) of large non-pedunculated colon polyps (LNPCPs). We aimed to develop and validate a risk stratification model to identify lesions at lower recurrence risk. METHODS: Retrospective multicenter study utilizing prospectively maintained databases across 12 centers (2017-2024) evaluated LNPCPs ≥20 mm removed by hot EMR. Twenty variables underwent multivariate Cox regression with stepwise selection to identify recurrence predictors. Validation was performed in a separate single-center cohor

Prophylactic clipping versus no clipping after endoscopic mucosal resection of large nonpedunculated colon polyps: a cost-effectiveness analysis.

BACKGROUND AND AIMS: Delayed postpolypectomy bleeding (DPPB) is an established adverse event after EMR of large nonpedunculated colon polyps (LNPCPs). Although clipping is commonly used in clinical settings, particularly for LNPCPs and for patients on antithrombotic agents, the cost-effectiveness of prophylactic clipping is not well studied. METHODS: We conducted an incremental cost-effectiveness analysis comparing prophylactic clipping (PC) versus no clipping (NC) for DPPB over a 1-year time horizon using a Markov decision tree model based on pooled randomized controlled trial data. Costs fo

Endoscopic anti-reflux mucosal resection for patients with gastroesophageal reflux disease: Clinical efficacy and impact on gut microbiota.

BACKGROUND: In recent years, endoscopic anti-reflux mucosal resection (ARMS) has demonstrated benefits, including good efficacy, ease of operation, low cost, and fewer complications; however, it is still in the exploratory stage. AIM: To evaluate the clinical efficacy of ARMS in patients with gastroesophageal reflux disease (GERD) and its effects on the gut microbiota. METHODS: This single-center, retrospective, self-controlled study included 80 patients with GERD. All patients underwent endoscopic ARMS and were followed for at least 3 months after surgery. The primary outcome measures were

Novel closure method for a large mucosal defect after endoscopic resection: String clip suturing method with an anchor.

Duodenal endoscopic submucosal dissection (ESD) is a high-risk technique; however, prophylactic closure of mucosal defects reduces the risk. Unfortunately, we have encountered cases where closure is difficult, especially in large lesions. Therefore, we developed a novel closure technique, a string clip suturing method with an anchor (SCSM-A). This study aimed to elucidate the feasibility of this method. Five patients underwent this method for the closure of mucosal defects after duodenal ESD. The initial string clip was deployed at the anal end of the mucosal defects and the second clip was de

Deep mural injury and perforation after colonic endoscopic mucosal resection: a new classification and analysis of risk factors.

OBJECTIVES: Perforation is the most serious complication associated with endoscopic mucosal resection (EMR). We propose a new classification for the appearance and integrity of the muscularis propria (MP) after EMR including various extents of deep mural injury (DMI). Risk factors for these injuries were analysed. DESIGN: Endoscopic images and histological specimens of consecutive patients undergoing EMR of colonic laterally spreading lesions ≥20 mm at a large Australian tertiary referral endoscopy unit were retrospectively analysed using our new DMI classification system. DMI was graded acco

Lesion isolation by circumferential submucosal incision prior to endoscopic mucosal resection (CSI-EMR) substantially improves en bloc resection rates for 40-mm colonic lesions.

BACKGROUND AND AIMS: En bloc resection is preferred for colonic laterally spreading tumors, but is limited to 20 mm with endoscopic mucosal resection (EMR) using normal saline submucosal injection. Our aims were to compare the efficacy and safety of circumferential submucosal incision prior to EMR (CSI-EMR) versus conventional EMR for en bloc resection of artificial lesions 40 x 40 mm in size using submucosal injection of succinylated gelatin in a porcine colon model. SUBJECTS AND METHODS: Two areas of normal rectosigmoid mucosa measuring 40 x 40 mm were marked with soft coagulation for en bl

Modified endoscopic mucosal resection versus endoscopic submucosal dissection for rectal neuroendocrine tumors ≤10 mm: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Endoscopic resection is the standard treatment for rectal neuroendocrine tumors (r-NETs) ≤10 mm, yet the optimal technique remains debated. Modified endoscopic mucosal resection (m-EMR) has been proposed as an alternative to endoscopic submucosal dissection (ESD), but prior evidence is mainly retrospective and recent randomized controlled trials (RCTs) are inconclusive. METHODS: We systematically searched CENTRAL, PubMed, Embase, and WanFang from January 1, 1970 to December 23, 2025, for RCTs comparing m-EMR and ESD in r-NETs ≤10 mm. Evidence certainty was evaluated using GRADE, a

Effect of prophylactic endoscopic clip placement on clinically significant post-endoscopic mucosal resection bleeding in the right colon: a single-centre, randomised controlled trial.

BACKGROUND: Endoscopic mucosal resection (EMR) is a cornerstone in the management of large (≥20 mm) non-pedunculated colorectal polyps. Clinically significant post-EMR bleeding occurs in 7% of cases and is most frequently encountered in the right colon. We aimed to assess the use of prophylactic clip closure in preventing clinically significant post-EMR bleeding within the right colon. METHODS: We conducted a randomised controlled trial at a tertiary centre in Australia. Patients referred for the EMR of large non-pedunculated colorectal polyps in the right colon were eligible. Patients were r

Efficacy and safety of underwater endoscopic mucosal resection (UEMR) for non-pedunculated colorectal polyps: a prospective study IN Vietnam.

INTRODUCTION: Colorectal cancer is a leading cause of global malignancy and mortality. While conventional endoscopic mucosal resection (CEMR) is the standard for removing premalignant lesions, it requires submucosal injection. Underwater EMR (UEMR) eliminates the need for injection by utilizing water immersion to achieve mucosal separation and lesion buoyancy. Prospective data from resource-limited healthcare settings like Vietnam remain scarce. METHODS: This prospective observational study was conducted at 115 People's Hospital, Ho Chi Minh City, from May to July 2025. We consecutively enrol

Construction and validation of machine learning-based predictive model for colorectal polyp recurrence one year after endoscopic mucosal resection.

BACKGROUND: Colorectal polyps are precancerous diseases of colorectal cancer. Early detection and resection of colorectal polyps can effectively reduce the mortality of colorectal cancer. Endoscopic mucosal resection (EMR) is a common polypectomy procedure in clinical practice, but it has a high postoperative recurrence rate. Currently, there is no predictive model for the recurrence of colorectal polyps after EMR. AIM: To construct and validate a machine learning (ML) model for predicting the risk of colorectal polyp recurrence one year after EMR. METHODS: This study retrospectively collect

Safety and efficacy of physician-administered balanced-sedation for the endoscopic mucosal resection of large non-pedunculated colorectal polyps.

Background and study aims Because of concerns about peri-procedural adverse events (AEs), guidelines recommend anesthetist-managed sedation (AMS) for long and complex endoscopic procedures. The safety and efficacy of physician-administered balanced sedation (PA-BS) for endoscopic mucosal resection (EMR) of large non-pedunculated colorectal polyps (LNPCPs) ≥20 mm is unknown. Patients and methods We compared PA-BS with AMS in a retrospective study of prospectively collected data from consecutive patients referred for management of LNPCPs (NCT01368289; NCT02000141). Intra-abdominal bleeding as a rare complication after colonic endoscopic mucosal resection in a patient taking direct oral anticoagulants.

We describe the case of an 84-year old female who was taking rivaroxaban 30 mg/day and had a medical history of atrial fibrillation. She underwent endoscopic mucosal resection of a 5-mm adenoma located in the hepatic flexure of the transverse colon. Following the procedure, she developed gradually intensifying abdominal pain, with the appearance of small amount of blood in the feces. For that reason, she visited our facility on the 5th day post-endoscopic mucosal resection. At the time of the visit, contrast-enhanced abdominal computed tomography revealed no extravasation or free air; however,

Anti-reflux mucosectomy using a cap-assisted endoscopic mucosal resection method for refractory gastroesophageal disease: a prospective feasibility study.

BACKGROUND AND AIMS: Endoscopic therapy of gastroesophageal reflux disease (GERD) overcomes the "treatment gap" for patients with refractory GERD, who are not willing to go into surgery. We propose an easy and efficient technique that is referred to as anti-reflux mucosectomy (ARMS) using cap-assisted endoscopic mucosal resection (EMR-C) which could be called ARMS-C. This study aimed to investigate the short-term outcomes of ARMS-C in GERD patients. METHODS: From December 2016 to February 2018, we performed ARMS-C in 33 patients with pathologic reflux disease and esophageal hypersensitivity.
