Neutral lipid storage disease with subclinical myopathy due to a retrotransposal insertion in the PNPLA2 gene.

An 18-year-old girl referred to a rheumatologist with malar flush and Gottran papules was found to have a markedly elevated serum CK. She was a good student and an avid ballet dancer. A muscle biopsy showed massive triglyceride storage, which was also found in peripheral blood granulocytes (Jordan anomaly) and cultured skin fibroblasts. Assessment using computerized dynamometry and cycle ergometry showed normal strength and muscle energetics, but proton spectroscopy revealed severe triglyceride accumulation in both skeletal and cardiac muscle. Sequencing of PNPLA2, the gene responsible for neu

[Transient ischemic ECG changes in a patient with acute cholecystitis without a history of ischemic heart disease].

That abdominal distention, nausea, and chest pain may be accompanied by ischemic ECG changes is well known and has been described in the literature. However, very few cases have been reported with acute cholecystitis and ECG changes not due to cardiac ischemia. We present a previously healthy 20-year-old woman admitted with acute cholecystitis. Prior to surgery routine ECG showed ST-depression and T-wave inversion. The day following cholecystectomy the ECG returned to normal. 1 month later, ergometry and echocardiography were both negative. Based on the literature and our personal experience,

A randomized, controlled trial of creatine monohydrate in patients with mitochondrial cytopathies.

Fatigue in patients with mitochondrial cytopathies is associated with decreased basal and postactivity muscle phosphocreatine (PCr). Creatine monohydrate supplementation has been shown to increase muscle PCr and high-intensity power output in healthy subjects. We studied the effects of creatine monohydrate administration (5 g PO b.i.d. x 14 days --> 2 g PO b.i.d. x 7 days) in 7 mitochondrial cytopathy patients using a randomized, crossover design. Measurements included: activities of daily living (visual analog scale); ischemic isometric handgrip strength (1 min); basal and postischemic exerci

Cerebellar neuromodulation synchronized to ankle-assist cycling yields prolonged post-assist dorsiflexion.

Rhythmic lower-limb movements require precise coordination across multiple joints, a process in which the cerebellum is thought to play a critical role. This study investigated whether cerebellar neuromodulation synchronized with mechanical ankle assistance modulates use-dependent motor adaptation during cyclic movement in healthy young adults. A portable ankle-assist ergometer (AAE) delivered phase-specific dorsiflexion torque during cycling, and participants completed three within-subject conditions: Sham stimulation, synchronized transcranial alternating current stimulation (tACS), and unsy

Force-velocity and power-velocity relationships in on-water rowing.

Force-velocity (FV) relationships are used to evaluate athletes and individualise training in several tasks. In rowing, only one previous study has determined FV relationships using ergometers. This study aimed to develop a method to obtain FV relationships during on-water rowing, to assess their quality and inter-day reliability and to compare them with ergometer profiles. Fourteen participants performed 8 all-out starts in instrumented double scull boats with 3 scull lengths under two conditions: two active rowers or one active and one passive rower. The FV relationships, reconstructed with

Evaluation of the strength of elbow flexors in patients with neuromuscular diseases.

In planning the optimum treatment for patients with neuromuscular diseases (NMD), it is essential to know as much as possible about their functional state. Assessment of the strength of certain muscles is the most direct measure of motor deficiency. In the development of normative data needed for patients with NMD, the use of torque measurements is required. Forty-nine patients (31 men and 18 women),f rom 18 to 54 years (mean age 33 +/- 8.9 years), were included in the study. Five groups of patients, each having one of five different NMDs, were formed. We tested unilaterally the biceps brachii

Effects of heart-rate feedback on estimated cardiovascular fitness in patients with panic disorder.

Psychological parameters that are believed to affect estimations of cardiovascular fitness were examined in patients with panic disorder and nonclinical controls. Fifty-four participants [panic disorder patients (n = 27) and age- and sex-matched nonclinical controls (n = 27)] completed a cycle ergometer test and were compared on the basis of estimated VO2 max. Participants were randomly assigned to experimental conditions in which they received heart-rate feedback or no feedback during the test. Patients with panic disorder exhibited lower VO2 max and decreased exercise tolerance (i.e., were m

[Ergospirometry in the diagnosis of disorders of the oxygen transport system in patients with chronic bronchitis].

Ergospirometry was used to carry out a study with the increasing exercise in 56 patients with chronic bronchitis and 46 healthy subjects. The limit physical potentialities, qualitative and quantitative characteristics of ventilation, hemodynamics, gas exchange at the height of the exercise were compared. It has been established that in the early disease stages, there was a tendency to hyperventilation during exercise in response to the deterioration of bronchial patency, which was accompanied by significant deterioration of the efficacy of ventilation with respect to the level of gas exchange

[Acute pulmonary embolism: beware of the wolf in sheep's clothing].

Two male patients aged 57 and 73 were referred to the cardiologist because of progressive dyspnoea. In one patient, the general practitioner had previously adopted an expectative policy because of a clean chest X-ray. At presentation after 4 weeks, the patient was diagnosed with and treated for acute coronary syndrome because of minor ECG abnormalities. Additional CT scanning showed a large saddle embolus. Despite adequate treatment, the patient suffered an electrical asystole and died. The other patient underwent ECG, bicycle ergometry, MRI adenosine, echocardiography and lung function tests

The ventilatory response to moderate hypocapnic exercise in human beings.

We examined the exercise hyperpnoea during hypocapnia which, by necessity, was brought about by voluntary hyperventilation (VHV). At cessation of VHV, ventilation (VE) decays gradually exhibiting a phenomenon known as the afterdischarge (AD; Eldridge, Med. Sci. Sports Exerc. 26:319--327, 1994). A new technique was employed to overcome the potentially contaminating influence of the AD upon the exercise hyperpnoea. Ventilatory parameters were measured in eight subjects during hyperoxic cycle ergometry at 3 intensities. The pure VE response to 'hypocapnic' exercise was obtained by subtracting the

[Choice of treatment methods in initial forms of ischemic heart disease].

The authors examined 125 patients with initial forms of ischemic heart disease (duration of no more than 2 months). The examination comprised coronarography, ventriculography, 24-hour ECG monitoring, and bicycle ergometry. The patients were divided into two groups: group 1 consisted of 81 patients who received nonoperative treatment; group 2 was formed of 44 patients who underwent operation for aortocoronary shunting. The results of treatment were appraised according to three parameters: a state of angina pectoris, prevention of myocardial infarction, and survival. They were studied in the hos

The importance of different atrioventricular intervals for exercise capacity.

In order to determine whether different atrioventricular intervals influence the maximal exercise capacity, 15 patients with second degree or complete atrioventricular block and no signs of left ventricular failure were studied. They all had atrioventricular synchronous pacemakers. Maximal exercise tests (sitting bicycle ergometry) were performed at four different atrioventricular intervals (50, 100, 150, and 200 ms) and during rate-matched asynchronous ventricular pacing. The patients thereby served as their own controls. Ventricular rate, maximal oxygen uptake and minute ventilation were mea

Value of exercise testing to evaluate the indication for surgery in asymptomatic patients with valvular aortic stenosis.

BACKGROUND AND AIMS OF THE STUDY: The study aim was to assess the value of exercise stress testing in identifying asymptomatic patients with moderate or severe valvular aortic stenosis (AS). These patients generally develop symptoms during follow up, and require valvular replacement surgery (VRS) at one to three years after single symptom-limited exercise stress testing. Limited data are available on predictors of outcome in asymptomatic patients with valvular AS. A single symptom-limited exercise stress test might offer more precise risk stratification of patients referred for cardiological e

The use of nifedipine in patients with Eisenmenger's syndrome complicating patency of the arterial duct.

Nifedipine has a vasodilatory effect on both the systemic and the pulmonary circulation. Its preferential effect on reducing the pulmonary vascular resistance has been studied in patients with primary pulmonary hypertension. In 4 patients with the Eisenmenger's syndrome complicating patency of the arterial duct (ductus arteriosus), we have studied the possibility of this selective pulmonary vasodilatory effect of nifedipine in reducing the right-to-left shunting. The degree of differential cyanosis was taken to reflect the right-to-left shunting. This was assessed continuously by 2 pulse oxime

The stroke rate influences performance, technique and core stability during rowing ergometer.

The aim of this study is to determine the effect of stroke rate on performance, technique and core stability during rowing ergometer. Twenty-four high-level rowers performed maximal intensity one-minute bouts at 20, 28 and 34 spm on a RowPerfect3 ergometer. Power at the handle, legs, trunk and arms levels were determined, and core kinematics and neuromuscular activations were measured. The power at the handle was enhanced with a higher stroke rate in the first half of the drive phase due to higher segment's powers. This resulted in technical changes, as for instance greater mean to peak power

Energetics of paraplegic cycling: a new theoretical framework and efficiency characterisation for untrained subjects.

Complete lower-limb paralysis resulting from spinal cord injury precludes volitional leg exercise, leading to muscle atrophy and physiological de-conditioning. Cycling can be achieved using phased stimulation of the leg muscles. With training there are positive physiological adaptations and health improvement. Prior to training, however, power output may not be sufficient to overcome losses involved in rotating the legs and little is known about the energetics of untrained paralysed muscles. Here we propose efficiency measures appropriate to subjects with severe physical impairment performing
