Blood products and liver transplantation: A strategy to balance optimal preparation with effective blood stewardship.

BACKGROUND: Unanticipated transfusion requirements during liver transplantation can delay lifesaving intraoperative resuscitation and strain blood bank resources. Risk-stratified preoperative blood preparation can mitigate these deleterious outcomes. STUDY DESIGN AND METHODS: A two-tiered blood preparation protocol for liver transplantation was retrospectively evaluated. Eleven binary variables served as criteria for high-risk (HR) allocation. Primary outcomes included red blood cell (RBC), plasma (FFP), and platelet (Plt) utilization. Secondary outcomes included product under- and overprepar

A review of in vitro methods to predict the clinical significance of red blood cell alloantibodies.

This review was derived from a presentation made on September 2, 2016, for the first Academy Day presented by the Working Party on Immunohematology at the International Society of Blood Transfusion (ISBT) Congress in Dubai. The focus of this review is on the clinical significance of alloimmunization in transfusion-specifically, the parameters that contribute to a clinically significant alloantibody. The areas of focus were as follows: Introduction, Technical Aspects, and Indications and Limitations. Each section contains a brief review of selected literature and experiential knowledge. Case re

Adverse effects of small-volume red blood cell transfusions in the neonatal population.

BACKGROUND: Adverse transfusion reactions in the neonatal population are poorly understood and defined. The incidence and pattern of adverse effects due to red blood cell (RBC) transfusion are not well known, and there has been no systematic review of published adverse events. RBC transfusions continue to be linked to the development of morbidities unique to neonates, including chronic lung disease, retinopathy of prematurity, intraventricular haemorrhage and necrotising enterocolitis. Uncertainties about the exact nature of risks alongside benefits of RBC transfusion may contribute to evidenc

Whole blood transfusion and paroxysmal nocturnal haemoglobinuria meet again: Minor incompatibility, major trouble.

BACKGROUND AND OBJECTIVES: The field of transfusion medicine started out with whole blood transfusion to treat severe anaemia and other deficiencies, and then transitioned to component therapy, largely leaving the practice, and experiences, of whole blood transfusions behind. Currently, the field is circling back and whole blood is gaining ground as an alternative to massive transfusion protocols. MATERIALS AND METHODS: Herein we describe a severely anaemic paroxysmal nocturnal haemoglobinuria (PNH) patient initially suspected of suffering from renal haemorrhage, receiving a standard low-titr

Red Blood Cell Storage and Post-transfusion Recovery.

Red blood cell (RBC) transfusion is essential for treating severe anemia, acute blood loss, and RBC production disorders by restoring oxygen-carrying capacity. Ensuring the safety and efficacy of transfused RBCs involves collection, processing, and storage. Despite optimal conditions, stored RBCs experience the "storage lesion," involving biochemical and morphological changes that affect their quality. Post-transfusion recovery (PTR), indicating the survival and functionality of transfused RBCs, is influenced by the storage lesion and varies among donors due to genetic and environmental factor

Development and Evaluation of a PCV-Dependent Visual Hemolysis Color Scale for Packed Red Blood Cell Products.

OBJECTIVE: To develop a PCV-dependent hemolysis color scale and evaluate its accuracy in predicting supernatant hemoglobin concentration in packed red blood cell (pRBC) products, helping to determine transfusion safety. DESIGN: Prospective experimental study. SETTING: University veterinary teaching hospital. INTERVENTIONS: Serial dilutions of a pRBC unit were performed to create a range of hemolyzed supernatant samples. A commercial graphics program was used to allocate computerized color to each hemolyzed sample, constructing a color scale. Study participants were then asked to use the col

The impact of pressure and temperature on the quality of suspended red blood cells: An ex vivo simulation study.

OBJECTIVE: This study aims to explore the impact of pressurisation and simultaneous warming at a combination of 50 kPa and 46°C on the quality of suspended red blood cells in an ex vivo environment. BACKGROUND: During massive rapid blood transfusion, pressure and temperature-controlled blood warming devices are often used to prevent hypothermia caused by the infusion of large amounts of cold blood. If the pressure and temperature are not properly applied during this process, it can endanger the patient's life safety. METHODS/MATERIALS: 400 mL of human suspended red blood cells stored at 2-6°

Split red blood cell units contain defined extracellular K+ levels, which are improved by a washing procedure.

BACKGROUND AND OBJECTIVES: We should control free K+ during massive transfusion (>80 mL/kg) of red blood cells (RBCs) in small children. To manage this, several national and international guidelines recommend using RBCs stored only up to 7 days. We tested a washing step for RBCs in saline-adenine-glucose-mannitol (SAGM) with or without irradiation to reduce supernatant K+ levels, improve quality and potentially extend the shelf life of stored RBCs. MATERIALS AND METHODS: RBCs of 240-330 mL were prepared from whole blood donations, then split into halves and stored in SAGM solution at 4 ± 2°C

Fresh frozen plasma-to-red blood cell ratio is an independent predictor of blood loss in patients with neuromuscular scoliosis undergoing posterior spinal fusion.

PURPOSE: In major trauma with massive blood loss, higher fresh frozen plasma (FFP)-to-red blood cell (RBC) ratios have been associated with improved morbidity and mortality. Our population of patients with neuromuscular scoliosis undergoing posterior spinal fusion (PSF) often lose volumes of blood considered massive, that is, half a blood volume in 3 hours. In this retrospective cohort study, we examined the association of FFP ratio with blood loss in this elective surgical population. METHODS: Patients with neuromuscular scoliosis undergoing PSF with unit rod fixation were identified from ou

Red blood cell components: time to revisit the sources of variability.

Quality and safety of red blood cell (RBC) components is managed by screening of donors and strict regulatory controls of blood collection, processing and storage procedures. Despite these efforts, variations in RBC component quality exist as exemplified by the wide range in storage-induced haemolysis. This article provides a brief overview of the variables that contribute or potentially contribute to the quality of stored RBC components, including blood collection, processing, and donor-related variables. Particular focus is made on donor health and lifestyle factors that are not specifically

Phase I/II safety study of transfusion of prion-filtered red cell concentrates in transfusion-dependent patients.

BACKGROUND AND OBJECTIVES: Variant Creutzfeldt-Jakob (vCJD) is a fatal transfusion transmissible prion infection. No test for vCJD in the donor population is currently available. Therefore, prion removal by filtration of red cell concentrate (RCC) is an attractive option for prevention. MATERIALS AND METHODS: Twenty patients were recruited with ethical permission, to receive clinically necessary transfusion containing one unit of pfRCC. Follow-up at 24 hours, 6 weeks and 6 months was undertaken. A second pfRCC was administered to 6 patients with similar follow up. pfRCC were prepared using th

Therapy in adenosine deaminase and purine nucleoside phosphorylase deficient patients.

The discovery of the causal association of adenosine deaminase (ADA) and purine nucleoside phosphorylase (PNP) deficiency with some forms of primary immunodeficiency disease had led to new approaches to therapy, such as enzyme replacement. In ADA deficiency, bone marrow transplantation remains the primary method of choice. If no suitable bone marrow donor is available, enzyme replacement with irradiated erythrocyte transfusions should be considered. The latter therapy may be sustained by treatment with thymic factors. In ADA deficiency, bone marrow transplantation and, in about 50% of the case

Human heterophil antibodies against rat erythrocytes. II. Cross reactivity with human A and B substances.

Naturally occurring titres of heterophil haemagglutinins against rat erythrocytes are related to ABO blood groups, being higher in people of blood groups O and A and lower in people of blood groups B and AB. Comparable titres of haemagglutinin activity can be demonstrated against human AB erythrocytes as well as rat erythrocytes. Activity against rat erythrocytes can be absorbed with human AB erythrocytes and vice versa. Furthermore, after absorption of heterophil antibody with human A or B erythrocytes, heat elution allows partial recovery of the antibody which reacts with rat erythrocytes. A

Clinical Case of a Transfusion-Associated Canine Mycoplasma haemocanis Infection in the Republic of Korea: A Case Report.

This report describes the first clinical case of a transfusion-associated Mycoplasma haemocanis infection in a dog in Korea. A 6-year-old male Maltese underwent a red blood cell transfusion for idiopathic immune-mediated hemolytic anemia. Eighteen days after the blood transfusion, the recipient's packed cell volume decreased and basophilic organisms were found on erythrocytes. A polymerase chain reaction and sequential analysis showed that both the donor dog and recipient dog had M. haemocanis. Six weeks after doxycycline administration, no organisms were detected and the recipient's anemia ha

Red Blood Cell Alloimmunization in Transfused Patients With Sickle Cell Disease in Sub-Saharan Africa; a Systematic Review and Meta-Analysis.

Sickle cell disease (SCD) is the most common monogenic disorder in sub-Saharan Africa (SSA). Blood transfusion to increase the oxygen carrying capacity of blood is vital in the management of many patients with SCD. However, red blood cell (RBC) alloimmunization is a major challenge to transfusions in these patients. Commonly in SSA, pretransfusion tests only involve ABO D grouping and compatibility without RBC antibody testing. Data on the frequency of RBC alloimmunization in patients with SCD in SSA are limited. We performed a systematic review and meta-analysis on available data on alloimmun

In a canine pneumonia model of exchange transfusion, altering the age but not the volume of older red blood cells markedly alters outcome.

BACKGROUND: Massive exchange transfusion of 42-day-old red blood cells (RBCs) in a canine model of Staphylococcus aureus pneumonia resulted in in vivo hemolysis with increases in cell-free hemoglobin (CFH), transferrin-bound iron (TBI), non-transferrin-bound iron (NTBI), and mortality. We have previously shown that washing 42-day-old RBCs before transfusion significantly decreased NTBI levels and mortality, but washing 7-day-old RBCs increased mortality and CFH levels. We now report the results of altering volume, washing, and age of RBCs. STUDY DESIGN AND METHODS: Two-year-old purpose-bred i

[Transfusion management of patients with alloanti-Gerbich antibodies: case report].

INTRODUCTION: Transfusion management of patients who are alloimmunized against high-prevalence erythrocyte antigens is often problematic. Strategy management depends, not only on the specific clinical circumstances of the patient, but also on the acceptable time frame. In patients without clinically significant antibody incompatible transfusion it may be less harmful than delaying medical intervention. CASE OUTLINE: We report a 57-year-old female from Libya, blood group O, RhD-positive, who was treated at the Institute of Cardiovascular Diseases of Vojvodina. At the Blood Transfusion Institut
