Comparison of priming versus slow injection for reducing etomidate-induced myoclonus: a randomized controlled study.

BACKGROUND: Etomidate injection is often associated with myoclonus. Etomidate injection technique influences the incidence of myoclonus. This study was designed to clarify which of the two injection techniques-slow injection or priming with etomidate-is more effective in reducing myoclonus. METHODS: This prospective randomized controlled study was conducted on 189 surgical patients allocated to three study groups. Control group (Group C, n = 63) received 0.3 mg/kg etomidate (induction dose) over 20 s. Priming group (Group P, n = 63) received pretreatment with 0.03 mg/kg etomidate, followed af

The pituitary adenylate cyclase-activating polypeptide (PACAP) protects adrenal function in septic rats administered etomidate.

BACKGROUND: Both hyperinflammation during sepsis and etomidate can suppress adrenal function. In this study, we explored whether treatment with pituitary adenylate cyclase-activating polypeptide (PACAP) relieves adrenal suppression in cecal ligation and puncture (CLP)-induced septic rats. MATERIALS AND METHODS: Female Sprague-Dawley rats were randomly divided into five groups (n=7 per group), including the sham group, sepsis group (CLP group), sepsis and etomidate group (CLP+ETO group), PACAP group, and etomidate alone group (ETO group). Rats were sacrificed on the third day of sepsis, and bl

Beyond adrenal suppression: a comprehensive post-marketing safety profile of etomidate from real world data.

BACKGROUND: Etomidate is a widely used intravenous anesthetic, yet concerns persist regarding its potential for adverse drug reactions (ADRs) beyond well-known adrenal suppression. Pre-marketing trials may not fully capture its long-term or rare safety profile, necessitating continuous post-marketing surveillance. METHODS: A retrospective pharmacovigilance study was performed based on the U.S. FDA Adverse Event Reporting System (FAERS) database (Q1 2004-Q2 2025). Disproportionality analysis using ROR, PRR, BCPNN and EBGM algorithms was conducted and the corresponding 95% CIs were calculated.

Etomidate versus Ketamine as Prehospital Induction Agent in Patients with Suspected Severe Traumatic Brain Injury.

BACKGROUND: Severe traumatic brain injury is a leading cause of morbidity and mortality among young people around the world. Prehospital care focuses on the prevention and treatment of secondary brain injury and commonly includes tracheal intubation after induction of general anesthesia. The choice of induction agent in this setting is controversial. This study therefore investigated the association between the chosen induction medication etomidate versus S(+)-ketamine and the 30-day mortality in patients with severe traumatic brain injury who received prehospital airway management in the Neth

Cushing's Storm: A Case of Paraneoplastic Cushing's Syndrome-Induced Seizures Treated With Continuous Etomidate Infusion.

A 67-year-old female presented with abdominal pain, nausea, vomiting, and unintentional weight loss. Further work-up revealed elevated serum cortisol, hypokalemia, and metabolic alkalosis in the setting of paraneoplastic Cushing's syndrome secondary to small lung cancer. The patient then developed refractory convulsive epileptic seizure despite being on multiple anti-epileptic medications. Here, we present a unique case where continuous etomidate infusion was used to lower serum cortisol, as adrenal insufficiency is associated with etomidate use. This case emphasizes how drug side effects can

Etomidate Attenuates the Ferroptosis in Myocardial Ischemia/Reperfusion Rat Model via Nrf2/HO-1 Pathway.

BACKGROUND: Ferroptosis has been found to play an important role in myocardial ischemia reperfusion (MIR) injury (MIRI). This study aimed to explore whether the improvement effect of Etomidate (Eto) on MIRI was related to ferroptosis. METHODS: The MIRI rats were constructed using left anterior descending artery occlusion for 30 min followed by reperfusion for 3 h. The Eto post-conditioning was performed by Eto administration at the beginning of the reperfusion. For rescue experiments, MIRI rats were pretreated with ferroptosis inducer erastin or Nrf2 inhibitor ML385 intraperitoneally 1 h prio

Etomidate affects the anti-oxidant pathway to protect retinal ganglion cells after optic nerve transection.

Our previous studies revealed that etomidate, a non-barbiturate intravenous anesthetic agent, has protective effects on retinal ganglion cells within 7 days after optic nerve transection. Whether this process is related to anti-oxidative stress is not clear. To reveal its mechanism, we established the optic nerve transection injury model by transecting 1 mm behind the left eyeball of adult male Sprague-Dawley rats. The rats received an intraperitoneal injection of etomidate (4 mg/kg) once per day for 7 days. The results showed that etomidate significantly enhanced the number of retinal ganglio

Etomidate plus propofol versus propofol alone for sedation during gastroscopy: a randomized prospective clinical trial.

BACKGROUND AND AIMS: Sedation with propofol alone during gastroscopy has many side effects. Etomidate has advantages in terms of circulation and respiration compared to propofol. We hypothesized that etomidate plus propofol during gastroscopy would be more safe and effective than propofol alone. METHODS: Four hundred (n = 400) patients were randomly divided into a propofol group (P group) and a etomidate plus propofol group (EP group). The P group was given the first dose of 1 % propofol 1 mg/kg before gastroscopy, and the EP group was given 1 % propofol 0.5 mg/kg plus etomidate 0.1 mg/kg. Re

Butorphanol pre-treatment prevents myoclonus induced by etomidate: a randomised, double-blind, controlled clinical trial.

QUESTIONS UNDER STUDY: Myoclonic movements are common problems during induction of anaesthesia with etomidate. The myoclonus occurring after etomidate administration may represent a form of seizure. Agonistic modulation of the κ opiate receptor may reduce seizures, and butorphanol acts in such a manner. The aim of this randomised, double-blind, placebo-controlled clinical trial was to test our hypothesis that pre-treatment with butorphanol might reduce the incidence and severity of myoclonus induced by etomidate. METHODS: Patients (108) with American Society of Anaesthesiologists physical sta

[L-nitroargininemethylester (L-NAME), a nitric oxide synthase inhibitor, increases the anesthetic potency of etomidate].

OBJECTIVE: Comparable to other intravenous anaesthetics, etomidate is thought to mediate its anaesthetic effect through an action on gamma-amino-butyric-acid (GABA) receptors. Recently, there is evidence that general anaesthetics act on second messenger systems such as the nitric oxide (NO) metabolism too. This study was designed to evaluate the effects of the NO-synthase inhibitor nitro-L-arginine methyl ester (L-NAME) on the anaesthetic potency of the intravenous anaesthetic etomidate. METHODS: With approval of the local animal care committee, the effect of L-NAME on the anaesthetic potency

Acquired 11β-hydroxylase Deficiency by Inhaled Etomidate and its Analogues: A Mimic of Congenital Adrenal Hyperplasia.

The increase in popularity of electronic cigarettes arouses great health concern. We describe a case of a 35-year-old female with chronic use of electronic cigarettes. She presented with secondary amenorrhea and prominent features of hyperandrogenism, including acne vulgaris, male-pattern alopecia, deepening of voice, and hirsutism. The initial biochemical workup revealed a profile indicating 11β-hydroxylase deficiency. However, genetic analysis did not show any evidence of CYP11B1 mutation. This patient manifested the florid signs of disrupted steroidogenesis brought by chronic use of electro

A Comparison of Hemodynamic Changes Between the Use of Etomidate and Propofol as Induction Agents for Anesthesia in Daycare Surgeries.

Background and objective The development of modern anesthetic agents has made it possible to conduct pain-free surgical procedures. The role of the anesthetist in choosing a suitable anesthetic agent to provide a good anesthetic and sedative effect is very important in any surgical procedure. There is always a degree of risk involved as the hemodynamic parameters may be altered. This study aimed to compare the hemodynamic changes and respiratory effects between the use of etomidate and propofol for the induction of general anesthesia (GA) as well as to compare the side effects of both drugs in
