Sedation for comfort at end of life.

Treatment for patients who are dying from cancer and are suffering with physiologic and existential symptoms is an important and valuable skill for health care providers. However, the treatment for suffering at the end of life and the use of sedation for comfort often are misunderstood. The following is a discussion of the clinical skills and ethical considerations that health care providers should have when treating terminal patients with cancer.

Aspirational autonomy and psychedelic-assisted therapy in advanced cancer: ethical lessons from clinical cases.

Psychedelic-assisted therapy (PAT) is increasingly discussed as a potential intervention for anxiety, depression, and existential distress among patients with advanced cancer. Yet the ethical challenges of PAT for patients approaching the end of life remain underdeveloped, particularly for oncologists who may be asked to counsel patients or consider referral to clinical trials. This article uses 3 deidentified clinical vignettes from a clinical trial of psilocybin-assisted therapy in a palliative oncology context to examine what it means for PAT to "work." We argue that PAT should not be evalu

Searching for Meaning in Chaos: Viktor Frankl's Story.

The existential psychiatrist Viktor Frankl (1905-1997) lived an extraordinary life. He witnessed and experienced acts of anti-Semitism, persecution, brutality, physical abuse, malnutrition, and emotional humiliation. Ironically, through these experiences, the loss of dignity and the loss of the lives of his wife, parents and brother, his philosophy of human nature, namely, that the search for meaning is the drive behind human behaviour, was moulded. Frankl formulated the basis of his existential approach to psychological practice before World War II (WWII). However, his experiences in the conc

Do patients with different mental disorders show specific aspects of shame?

Shame is related to several mental disorders. We assume that facets of shame, namely bodily, cognitive and existential shame, may occur in typical patterns in mental and personality disorders. An excessive level of shame may lead to psychopathological symptoms. However, a lack of shame may also lead to distress, for instance as it may facilitate violation of social norms and thus may promote interpersonal problems. In this study we investigated facets of shame in females suffering from various mental disorders and personality disorders presumably associated with specific aspects of shame. Wome

Psychological variables potentially implicated in opioid-related mortality as observed in clinical practice.

Opioid-related deaths in the United States have become a public health problem, with accidental and unintended overdoses being especially troubling. Screening for psychological risk factors is an important first step in safeguarding against nonadherence practices and identifying patients who may be vulnerable to the risks associated with opioid therapy. Validated screening instruments can aid in this attempt as a complementary tool to clinicians' assessments. A structured screening is imperative as part of an assessment, as clinician judgment is not the most reliable method of identifying nona

Meaning-Centered Psychotherapy for Cancer Caregivers Study protocol of a randomized controlled trial.

BACKGROUND: Caregivers of patients with advanced cancer shoulder immense responsibilities as they care for patients, including symptom and medication management, providing emotional support, and navigating healthcare treatment and decision-making. Due to the heavy toll of these responsibilities, caregivers are at high risk for profound mental health challenges, including anxiety, depression, and posttraumatic stress disorder. A key driver of this heightened risk for psychopathology in caregivers is existential distress, manifesting as a loss of meaning and purpose, decreased spiritual well-bei

Exploring Meaning-Making and Identity Transformation Among Adults Living with Type 2 Diabetes in Saudi Arabia: A Hermeneutic Phenomenological Study.

BACKGROUND: Diabetes mellitus is highly prevalent in Saudi Arabia and shapes individuals' lived experiences within culturally and religiously embedded contexts. Although existing research has extensively examined biomedical, behavioral, and psychosocial dimensions of diabetes using quantitative and conventional qualitative approaches, there remains limited understanding of how individuals interpret, construct meaning, and reconstruct identity in relation to the illness over time. In particular, phenomenological inquiry is needed to access the lived, embodied, and existential dimensions of type

Goethe's anxieties, depressive episodes and (self-)therapeutic strategies: a contribution to method integration in psychotherapy.

BACKGROUND: In psychiatry and psychotherapy, abstract scientific principles need to be exemplified by narrative case reports to gain practical precision. Goethe was one of the most creative writers, productive scientists, and effective statesmen that ever lived. His descriptions of feelings, emotions, and mental states related to anxieties, depressive episodes, dysthymia, and creativity are unique in their phenomenological precision and richness. His life and work can thus serve as an excellent example enhancing our understanding of the relationship between anxiety, depression and creativity.

Impaired boundaries, sense of self, and personal control: the lifelong trajectory of co-occurring obesity and adverse childhood experiences for adult women.

PURPOSE: To deepen the understanding of the phenomenon of co-occurring obesity and ACEs for adult women. METHODS: A qualitative, phenomenological, Reflective Lifeworld Research (RLR) approach was used. Fourteen women living with obesity (BMI ≥ 30) who were between the ages of 18 and 64, and who had at least one ACE participated. RESULTS: The results revealed three essential meanings of the phenomenon: a boundaryless childhood existence, being in and out of control, and a vague or distorted sense of self. Six intertwined constituent meanings further elucidate the phenomenon: obesity as an exi

Epidemic imaginaries in literature and film: a comparative study of narrative responses to plague and pandemic.

Epidemics have historically been both biomedical disasters and cultural stories that societies interpret and recount. This study explores the epidemic imaginary, examining how literature and cinema symbolically depict contagion by analysing narratives of contagion produced during the Plague era in literature and modern pandemic representations in film. The research considers some plague-era literature, such as Albert Camus' The Plague (1947) and Thomas Mann's Death in Venice (1912), alongside more recent pandemic literature like Dean Koontz's The Eyes of Darkness (1981), as well as two films:

Philosophical aspects of migraine: Headache treatment is a human-to-human service.

Migraine is a complex neurobiological disorder that extends beyond physical symptoms to encompass profound mental, emotional and existential dimensions. Despite its prevalence, the philosophical aspects of migraine remain underexplored in scientific research. This review examines migraine through a philosophical lens, addressing questions of pain, suffering, consciousness and communication. It highlights the different perspectives of neuroscientists, clinicians and patients: the former seeks to understand migraine as a neurological dysfunction, while the latter experiences it as an intensely p

"My bus is here": a phenomenological exploration of "living-with-dying".

OBJECTIVES: This article has 2 aims. The first is to demonstrate how the application of an innovative qualitative methodology generated novel insights into the experience of living with advanced cancer. The article's second aim is to challenge the idea that the identification of shared themes provides the researcher with access to the meaning and significance of the experience of "living-with-dying." METHOD: The research used object elicitation together with existentially informed hermeneutic phenomenological analysis. The analysis is based on 10 semistructured interviews with people who are

Self-neglect of spinal injured patients.

Self-neglect is a recognised, but poorly emphasised, complication of spinal cord injury, with a prevalence in this study of 9% of a consecutive sample of 400 patients. Self-neglect may be the presenting symptom of problems related to adjustment to disability, a major depressive disorder (DSM III criteria), or a clear and rational wish to die (existential self-neglect). Management is discussed, emphasising the importance of respecting spinal injured patients' autonomy.

Assisted suicide in Austria: implications for media reporting.

Assisted suicide is an emerging phenomenon in many western countries including Austria, with important implications for public mental health. From a suicide prevention and public health standpoint, it is essential that assisted suicide should neither be stigmatized nor romanticized or presented as the only option to address existential suffering. Balanced reporting is key to educate the public about available options at the end of life including palliative care.

Clinical holistic medicine: induction of spontaneous remission of cancer by recovery of the human character and the purpose of life (the life mission).

The recovery of the human character and purpose of life with consciousness-based medicine seems to be able to induce spontaneous remissions in several diseases. On two different occasions, we observed breast tumors reduced to less than half their original diameters (clinically judged) during a holistic session, when working with the patients in accordance with the holistic process theory of healing, the life mission theory, and the theory of human character. One tumor was histologically diagnosed as malign breast cancer prior to the session, while the other was under examination. As both patie

Intolerance of Uncertainty and Life Satisfaction: The Mediating Roles of Meaning in Life and Death Anxiety.

Uncertainty is an inevitable part of human life, yet its association with life satisfaction remains insufficiently understood. While previous research has linked intolerance of uncertainty (IoU) primarily to anxiety and distress, less attention has been given to the existential processes through which it may be associated with overall well-being. The present study examined whether meaning in life (MiL) and death anxiety mediate the association between IoU and life satisfaction. Participants were 485 adults (68.3% female) aged 18-63 years (M = 32.02, SD = 11.98). Using structural equation model

Lived experiences of correctional officers in treating drug offenders: a hermeneutic phenomenological approach.

PURPOSE: This study explored the lived experiences of correctional officers working with individuals convicted of drug offenses. METHODS: Data were collected through individual in-depth interviews and observations with 10 correctional officers who had experience working with individuals convicted of drug offenses. Data were collected from September 15 to November 2, 2025, and analyzed using van Manen's hermeneutic phenomenological approach to identify essential themes. RESULTS: The themes were interpreted within four existential grounds: body, space, other, and time. The lived body was exper
