Necrotizing fasciitis of the scalp in neonates.

Two premature infants with acute necrotizing fasciitis of the scalp are reported. In both cases the infection most likely arose from infiltrations of intravenous fluids into the subcutaneous region of the scalp. Aggressive surgical debridement, as well as antibiotic therapy, resulted in healing in both infants. A review of neonatal fasciitis is included.

Periocular necrotizing fasciitis in an infant.

Periocular necrotizing fasciitis developed in a 12-month-old boy with swelling of both eyes and redness and a discharge from the left eye approximately 36 hours after blunt trauma. Computed tomography revealed preseptal and soft-tissue edema on the left side, but no signs of orbital involvement, orbital fractures, or drainable abscess in the anterior left lower eyelid. The inflammatory signs worsened over the next day, and there was purulent discharge from the left lower eyelid and an abscess and necrosis of the lower eyelid skin. He did well following surgical debridement and treatment with i

Necrotizing fasciitis of the lower limbs.

We report an uncommon severe soft-tissue infection of the thighs in a male child with acute lymphoblastic leukemia. Early and aggressive medical treatment and the conservative surgical approach were successful. Necrotizing fasciitis should be suspected in any soft-tissue infection until it can be definitely ruled out, since prompt deliver of medical and surgical intervention is essential.

Diagnosis and management of necrotizing fasciitis.

Necrotizing fasciitis is a rapidly progressive, life-threatening soft tissue infection which is rapidly fatal unless diagnosed promptly and treated with immediate debridement of necrotic tissue. As early clinical suspicion is paramount to improved survival, this review aims to increase awareness of the condition.

Necrotizing fasciitis of the head and neck.

BACKGROUND: Necrotizing fasciitis of the head and neck is a rapidly progressing and life-threatening condition. The purpose of this study was to describe the patients with a focus on clinical presentation, microbiology, treatment, and prognosis. PATIENTS AND RESULTS: Seventeen patients (10 men; median age, 54 years) were included. Nine patients underwent minor head and neck surgery immediately prior to necrotizing fasciitis. The typical course was a quickly spreading erythema, pronounced tenderness, and severe pain. Imaging demonstrated diffuse swelling of the soft tissue, poorly differentiat

Necrotizing fasciitis of the chest wall.

Necrotizing fasciitis (NF) is an uncommon infection caused by microorganisms called 'flesh eating bacteria'. It remains a life-threatening condition associated with high mortality rate. Its location to the chest wall is exceptional. Herein, we report the case of a 39-year-old female, without comorbidity, presenting a NF of the chest wall complicating an empyema. We describe the surgical management with a three-step procedure: antibiotherapy-debridement, vacuum-assisted closure and delayed surgical reconstruction.

Approach to debridement in necrotizing fasciitis.

Aggressive debridement is a cornerstone intervention in necrotizing fasciitis. Our approach consists of 4 steps: (1) confirming the diagnosis and isolate the causative organism; (2) defining the extent of fasciitis; (3) surgical excision; and (4) post-excision wound care. The extent of the infection is defined by probing the wound bluntly. Systematic excision follows. Fascial excision must be complete and uncompromising with the full extent of the involved wound laid open. We classify the infected skin into zones 1, 2, and 3. Zone 1 is necrotic tissue. Zone 2 is infected but potentially salvag

Mortality in patients with necrotizing fasciitis.

BACKGROUND: The prognostic factors that determine outcome in patients with necrotizing fasciitis remain poorly understood. The aim of this study was to analyze the variables that affect the mortality and morbidity of patients with necrotizing fasciitis and to create a simple method for estimating the probability of mortality. METHODS: The authors undertook a retrospective review of all patients with necrotizing fasciitis treated in three tertiary care hospitals in Ontario, Canada, between January of 1994 and June of 2001. Demographic, comorbid illness, and disease-specific data were collated

Necrotizing fasciitis of the scalp in a newborn.

BACKGROUND: Fetal scalp electrode monitoring is usually without complications, but on rare occasions it can serve as a portal of entry for organisms colonizing the maternal genital tract. CASE: We present a case of neonatal necrotizing fasciitis of the scalp that was associated with intrapartum fetal scalp electrode monitoring. Skin cultures grew Group A Streptococcus M11 T nontypeable serotype, an unusual cause of neonatal necrotizing fasciitis. The neonate's mother had a concurrent perineal infection and the same Group A streptococcal serotype was cultured from maternal blood and vaginal sw

[Diagnosis and therapy of necrotizing fasciitis].

Necrotizing fasciitis is a soft tissue infection with a lethality ranging up to 80%. Infection causes the activation of interleukin, tumor necrosis factor alpha, and gamma-interferon through a triggering mechanism. This results in a capillary thrombosis with necrosis of the fascia, cutis, and subcutis. The patient's history often reveals a triggering event in the form of a recent minimal trauma or operative procedure. In a fulminant necrotizing fasciitis, the development of sepsis with consecutive multiple-organ failure mainly determines the outcome of the disease. Diagnosis is made initially

Necrotizing fasciitis of the scalp in a neonate.

We report an 11-day-old baby who presented with necrotizing fasciitis of the scalp from which Escherichia coli was cultured. Treatment consisted of administration of parenteral broad-spectrum antibiotics and debridement. Skin grafting of the resulting scalp defect was not permitted by the parents. The wound healed with scar tissue over 3 months.

[Management of necrotizing cellulitis and fasciitis].

A literature review did not reveal any controlled study on the management of necrotizing fasciitis. Treatment protocol includes: - an immediate or early surgical management with debridement of all necrotic tissue and extensive fasciotomy followed by a surgical reexamination of the infected area in the following days; - an initial antibiotic therapy targeting aerobic Gram-positive and Gram-negative organisms and anaerobes (e.g: amoxicilline-clavulanic acid or vancomycin-metronidazole); - an adequate nutritional support, infusion, and resuscitation; - hyperbaric oxygen therapy may be considered

[The therapeutic approach to necrotizing fasciitis].

Necrotizing fasciitis are characterized by the necrosis of fascias, and their severe consequences in terms of morbidity and mortality. An early diagnosis, based on sometimes subtle cutaneous lesions (associated to a sepsis syndrome) allows to start resuscitation and decide on a probable surgery. 3 major forms can be distinguished: streptococcal fasciitis, due to beta-hemolytic streptococci, often following minor trauma, and increasingly associated to a streptococcal toxic shock syndrome (STTS); clostridial gangrene (often polymicrobial when developed on a open wound or after surgery); and syne

Diagnosis of necrotizing fasciitis in children.

Necrotizing fasciitis is a rare but progressive soft tissue infection. This condition is difficult to recognize in the early phase, when it is often confused with cellulitis. We report the cases of four children with necrotizing fasciitis. The initial presentation in these cases was cellulitis. Fever and soft tissue swelling occurred within 24 h and spreading erythema within 4 to 12 h. Radiologic studies of the lesions showed soft tissue thickening. Ultrasonography of the lesions demonstrated distorted, thickened fascia with fluid accumulation. Well-defined, loculated abscesses were demonstrat
