Evaluation of the Length of Anal High-Pressure Zone as a Barrier for Fecal Incontinence.

PURPOSE: Fecal incontinence (FI) is a multifactorial problem. The length of the anal high-pressure zone (HPZ) may serve as an independent barrier for anal continence, but its role in the pathogenesis of FI has not been examined. We evaluated the association between the length of anal HPZ and anal sphincter pressures in FI patients with or without anal sphincter defects and compared this with healthy controls. METHODS: Patients with FI (≥2 episodes per month) underwent high-resolution anorectal manometry and anal ultrasound tests to assess the anal HPZ, anal sphincter pressures and anal morpho

Effect of home biofeedback treatment in patients with fecal incontinence: a pilot study.

BACKGROUND: Fecal incontinence is a common condition that markedly impairs quality of life. Anal biofeedback therapy is recommended as first-line treatment; however, its widespread use is limited by the need for repeated sessions, trained staff, high cost, and patient embarrassment, often resulting in poor adherence. This study aimed to evaluate the effectiveness of self-administered home-based biofeedback therapy and to determine factors associated with treatment success. METHODS: This prospective, single-center study included adult patients presenting with fecal incontinence. Disease severi

The Digital Rectal Examination: Appropriate Techniques for the Evaluation of Constipation and Fecal Incontinence.

Disorders of defecation, including constipation and fecal incontinence, are very common. The digital rectal examination (DRE) is a key component in the early evaluation of patients with these complaints. Confident performance of a DRE requires dedicated training for the clinician and hands-on experience with the technique. DRE can yield a diagnostic accuracy comparable to specialized physiologic tests, including anorectal manometry. This review will describe in detail the steps required to perform a thorough DRE evaluation, as well as the proper interpretation of observations. Thereafter, the

Prevalence and Risk Factors of Anal and Fecal Incontinence in Japanese Medical Personnel.

OBJECTIVES: This study aimed to elucidate the actual state of anal incontinence (AI), fecal incontinence (FI), and the associated factors in Japanese medical personnel. METHODS: A questionnaire was completed by Japanese medical personnel after listening to lectures on AI. AI was defined as involuntary loss of feces or flatus. RESULTS: A total of 463 persons (mean age, 35.6 years; range, 20-91; male/female/no answer, 132/324/7) participated in the questionnaire. AI occurred in 34.4% of 450 participants (flatus/liquid stool/solid stool: 30.4%/3.6%/0.4%). AI was significantly more prevalent in

Non-dynamic graciloplasty is an effective treatment for patients with passive fecal incontinence.

BACKGROUND: Fecal incontinence is a multifactorial problem and its etiology is complex. Various therapies are available and different success rates have been described. The aim of this study was to assess the effectiveness and safety of non-dynamic graciloplasty in patients with passive fecal incontinence. METHODS: We retrospectively studied charts of patients with fecal incontinence treated with graciloplasty at our institution from November 2015 until June 2018. Patients were included according to the following criteria: (1) presence of predominantly passive fecal incontinence and (2) prese

Impact of Transanal Irrigation Device in the Management of Children With Fecal Incontinence and Constipation.

OBJECTIVES: Children with fecal incontinence and constipation can be classified into 3 groups: neurogenic bowel dysfunction (NBD) related to spinal cord defects (NBD), refractory constipation (RC), or anorectal malformations (ARMs). The transanal irrigation (TAI) device (Peristeen) was approved in 2012 by the Food and Drug Administration. This system uses a pump rather than gravity to instill water as a colonic irrigant and uses balloon occlusion of the rectum. Our aim was to evaluate the effectiveness of TAI (Peristeen) in children who failed to respond to conservative measures for stool inco

Clinical and physiological risk factors for fecal incontinence in chronically constipated women.

BACKGROUND: Fecal incontinence (FI) and chronic constipation (CC) are disabling symptoms that cause a significant public health problem. The pathophysiology of combined constipation and FI is not fully understood. Our aim was to delineate the clinical, physiological and anatomical factors that may contribute to the association of FI and CC. METHODS: A retrospective study was performed in a pelvic floor unit in a tertiary medical center. Consecutive female patients diagnosed with CC were included, and further divided into two groups according to the co-occurrence of FI. Demographic characteris

Patient-Controlled Biofeedback Device for the Treatment of Fecal Incontinence: A Pilot Study.

Although biofeedback has been used as a first-line therapy for fecal incontinence, it is known to be time consuming and demands attendance to a hospital during the whole period of treatment. In this study, we describe a new biofeedback device specifically developed for home treatment of fecal incontinence, which consists of a microprocessor controlled unit able to register and store the anal pressure waves corresponding to exercises performed by patients at home. In order to test the new device, a pilot study including ten patients with fecal incontinence was conducted. Evaluation of patients

Is electromyography a predictive test of patient response to biofeedback in the treatment of fecal incontinence?

AIM: Biofeedback is effective in more than 70% of patients with fecal incontinence. However, reliable predictors of successful treatment have not been identified. The aim was to identify clinical variables and diagnostic tests, particularly electromyography, that could predict a successful outcome. METHODS: We included 135 consecutive women with fecal incontinence treated with biofeedback. Clinical evaluation, manometry, ultrasonography, electromyography, and pudendal nerve terminal motor latency were performed before therapy. Treatment outcome was assessed using a symptoms diary, Wexner inco

Use of hollow microneedles for targeted delivery of phenylephrine to treat fecal incontinence.

A hollow microneedle (HM) was prepared to deliver a phenylephrine (PE) solution into the anal sphincter muscle as a method for treating fecal incontinence. The goal of this study was the local targeted delivery of PE into the sphincter muscle through the perianal skin with minimal pain using hollow microneedles, resulting in the increase of resting anal sphincter pressure. PE was administered on the left and the right sides of the anus of a rat through the perianal skin using 1.5mm long HM. An in vivo imaging system study was conducted after injection of Rhodamine B, and a histological study w

Developing a new electromyography-based algorithm to diagnose the etiology of fecal incontinence.

PURPOSE: For surface electromyography (sEMG) to become widely used in fecal incontinence (FI) etiology assessment, one would have to create a simple, step-by-step, computer-aided, electromyography-based algorithm that would become the basis for a computer-aided diagnosis (CAD) system. Thus, the aim of this work was to develop such an algorithm. METHODS: Each patient included in the study underwent a structured medical interview, a general physical examination, and a proctological examination. Patients that scored more than 10 points on the fecal incontinence severity index (FISI) underwent fu

Rectal motility in patients with idiopathic fecal incontinence: a study with impedance planimetry.

INTRODUCTION: Most patients with fecal incontinence have poor anal sphincter function. In patients with idiopathic fecal incontinence no structural abnormality can be identified. The aim of the present study was to compare rectal motility patterns in patients with idiopathic fecal incontinence and in healthy controls. METHODS: Rectal impedance planimetry provides simultaneous measurement of rectal pressure, anal pressure, and rectal cross-sectional area at 5 levels. This allows highly detailed description of rectoanal motility. In 12 female patients with idiopathic fecal incontinence (mean ag
