[Current trends of the treatment of the femoral neck fractures in seniors].

INTRODUCTION: Proximal femur fracture of the seniors is often "omen" for their life. These fractures are the most frequent cause of acute hospitalization of the aged and on the contrary to previous years the procedures are considered as emergent - life saving. OBJECTIVE: The aim of the study was to find out which of the contemporary used surgical procedures for the treatment of unstable femoral neck fractures at the Department of Trauma Surgery in Kosice yield in the lowest number of failures and postoperative complications. Subsequently we have analyzed changes in surgical treatment of these

Pathologic fracture of the femoral neck in a female soccer player.

We report a case of a healthy, young, female soccer player who developed progressive pain in her right hip. A bone cystic lesion was found in the right femoral neck and proximal femur. The lesion was considered a benign bone cyst and the patient was treated with injections of autologous bone marrow and grafting into the femoral neck. However, the cystic lesion did not heal. Subsequently, the patient fell and developed a fracture of the femoral neck that required internal fixation. The bone biopsy showed characteristic histologic features of fibrous dysplasia. A bone scan showed other areas of

Insufficiency fracture of the femoral neck after intramedullary nailing.

We report a patient with insufficiency fracture that occurred after intramedullary nailing for a subtrochanteric fracture. Intramedullary nailing is speculated to have increased the stress in the already osteoporotic subcapital region. It therefore should be recognized as a causative factor in insufficiency fracture of the femoral neck. Careful follow-up is needed for patients with this condition.

The outcome of Austin-Moore hemiarthroplasty for fracture of the femoral neck.

One hundred and fifty-four consecutive patients who sustained a fracture of the femoral neck and were treated with an Austin-Moore hemiarthroplasty were followed up longitudinally for 10 years, or until revision of the prosthesis, or death of the patient. Clinical assessments were performed at 1, 3, 5, and 10 years after surgery. At 3 years, 46% of the patients were community ambulators; 10%, household ambulators; 6%, nonfunctional ambulators; 29%, nonambulators; and 9% unknown. At 10 years, 31% were community ambulators; 35%, household ambulators; 4%, nonfunctional ambulators; and 30%, nonamb

[Painless femoral neck fracture in a patient with multiple sclerosis].

We describe a patient with multiple sclerosis who suffered a painless hip fracture. She was admitted to the hospital three weeks after falling on her left hip. She had been unable to walk after the trauma, but had had no pain. The condition was initially misinterpreted as multiple sclerosis schub. X-ray examination showed fracture of the left femoral neck, and by the time she was admitted to hospital the patient was paretic. She has improved substantially after an operation involving cementless total hip arthroplasty. Painless fractures in patients with multiple sclerosis may be confused with

Bilateral nonunited femoral neck fracture in a child with osteopetrosis.

We report a case of osteopetrosis with bilateral nonunited femoral neck and coxa vara in a 7-year-old boy. There was a history of traumatic transcervical left femoral neck fracture unsuccessfully treated by internal fixation. We performed a bilateral subtrochanteric valgus osteotomy. K-Wire fixation failed on the right side due to hardness of the bone. Finally, internal fixation with an angular plate after predrilling of the femoral neck offered stability. Bone union was achieved on both sides, resulting in full recovery of normal physical activity.

[An unusual cause of fracture of the femoral neck: bone sarcoidosis].

A case of pathological fracture of the femoral neck in a 84-year old female is presented. The histological examination of the femoral head showed sarcoidosis. This diagnosis was not known before the trauma. Other localisations of bone sarcoidosis were found in the tarsus and the fingers after a systematic X-ray check-up.

The architecture of cancellous and cortical bone in femoral neck fracture.

The architecture of cancellous bone as well as quantity and quality is considered important in maintaining mechanical integrity. To determine whether abnormalities of architecture occur in femoral neck fracture we measured trabecular width and number in iliac crest bone biopsies of 68 women with femoral fracture and compared them with data from a postmortem series of age-matched women without known bone disease. Cortical thickness was measured in 27 of the fracture patients and 17 controls. After exclusion of ten biopsies with increased osteoid surface no significant difference was seen in mea

Acute femoral neck fracture in children--preliminary report.

Nineteen children, aged 3-15 years, with acute femoral neck fracture were treated. The follow-up period ranged from 2 years to 6 years, 9 months (avg. 54.8 +/- 14.5 months). The clinical and radiological assessments were carried out according to the assessment system of Ratliff. Undisplaced fractures and all intertrochanteric fractures were treated conservatively and displaced ones by reduction and multiple pinning. The outcomes reveal 14 (79%) with good results, 2 (10.5%), fair and 3 (15.8%), poor. Four hips (21%) developed avascular necrosis with 2 requiring secondary surgical procedures. Bo

Femoral neck fracture in a child with autosomal dominant osteopetrosis.

A child with autosomal dominant osteopetrosis had successful treatment of a femoral neck fracture by internal fixation. Drilling the pins into the femoral neck was extremely difficult. The histopathology in osteopetrotic bone provides a probable explanation for the technical difficulty seen in this case.

Association of Surgical Approach with Intraoperative Femoral Fracture Patterns During Cementless Hemiarthroplasty for Femoral Neck Fracture.

Background/Objective: Intraoperative femoral fractures during cementless hemiarthroplasty may present with distinct anatomical patterns. We examined whether surgical approach was associated with greater trochanteric versus stem-stable periprosthetic fracture patterns in older patients with osteoporotic-type femoral morphology; stem geometry was evaluated as an additional implant-related factor. Methods: We retrospectively reviewed 292 patients aged ≥75 years who underwent cementless bipolar hemiarthroplasty for low-energy displaced femoral neck fractures at a tertiary referral trauma center be

Subtrochanteric Femoral Fracture in a Patient with Osteopetrosis: Treated with Internal Fixation and Complicated by Intraoperative Femoral Neck Fracture.

OBJECTIVE: Osteopetrosis (OP) is a rare, heritable skeletal disorder that is caused by osteoclast dysfunction, leading to failure of bone resorption and increased bone density. The fragility of such dense bone may result in an increased incidence of fractures. Furthermore, surgery in patients with OP is associated with increased technical difficulty and a higher risk of complications. CASE REPORT: We report a case of a 20-year-old woman with autosomal dominant OP who developed a subtrochanteric femoral fracture. The fracture was treated by open reduction and internal fixation using a dynamic

Subchondral insufficiency fracture of the femoral head after internal fixation for femoral neck fracture: histopathological investigation.

Late segmental collapse after internal fixation for femoral neck fracture is the phenomenon observed in post-traumatic osteonecrosis of the femoral head (ON), which has generally been reported to occur over a year or more after internal fixation. Subchondral insufficiency fracture of the femoral head (SIF) has also been recognized to cause femoral head collapse, however, only two cases of SIF after internal fixation for femoral neck fracture have been reported. We report a case with femoral head collapse observed 5 months after internal fixation for femoral neck fracture, which was histopathol

A rare complication after intertrochanteric fracture treated with proximal femoral nail: femoral neck insufficiency fracture.

We present a case, 82-year-old woman, sustaining intertrochanteric fracture treated with proximal femoral nail. Up to 14 months postoperatively, everything went on seamlessly. Then, there was a rapid onset of worsening of the functional capacity. She came to outpatient clinic by using wheelchair. Plain X-rays revealed a femoral neck fracture on the operative side with no history of trauma. Laboratory studies showed no significant abnormalities but vitamin D [25 (OH) D] level was 14.82 ng/ml. X-ray survey of skeletal system was not indicative of osteomalacia. We performed total hip replacement

Risk factors for intraoperative greater trochanteric fractures in hemiarthroplasty for intracapsular femoral neck fractures.

PURPOSE: Hemiarthroplasty is widely accepted as the treatment of choice in elderly patients with a displaced intracapsular femoral neck fracture. Intraoperative greater trochanteric fractures thwart this successful procedure, resulting in prolonged recovery, inferior outcome, and increased risk of revision surgery. Hence, this study analyzed factors potentially associated with an increased risk for intraoperative greater trochanteric fracture. METHODS: This retrospective study included 512 hemiarthroplasties in 496 patients with a geriatric intracapsular femoral neck fracture from July 2010 t
