Maintaining essential health services during COVID-19: cross-country lessons of health system resilience from Asia, Sub-Saharan Africa and Latin America.

The COVID-19 pandemic severely disrupted the delivery of essential health services (EHS) worldwide, contributing to excess morbidity and mortality from preventable conditions. Some countries employed innovative strategies that may have enabled their health systems to be more resilient than others in responding to COVID-19. This cross-country analysis aimed to identify beneficial practices and policies employed by six low- and middle-income countries (LMICs) in Asia, Sub-Saharan Africa and Latin America to maintain access to EHS while responding to COVID-19. Cross-country research partners (CCR

Expanding choice through defined contributions: overcoming a non-participatory health care economy.

The fate of the Patient Protection and Affordable Care Act of 2010 is uncertain. Much of the opposition is grounded in popular hostility to expansive federal control over individuals' health care decision-making. But the new law reinforces existing third-party payment, primarily through employers and government programs. This financing already restricts personal choice of health plans and coverage options. Private employers, managed care executives, and public officials make the key spending decisions in health care. Unlike consumers in other sectors of the economy, individuals are mostly pass

Artificial Intelligence and Climate Risk Shocks.

Artificial intelligence (AI) has been widely applied across various fields and has demonstrated effectiveness to some extent. However, some scholars have raised concerns about its ethical implications and potential rebound effects, particularly in the context of climate issues. To address these debates, we obtained cross-national panel data for 51 countries from 1996 to 2023 through the ISETS Energy Finance Network, WIPO, and World Bank databases. A two-way fixed effects model was used to examine the relationship between AI and physical climate risks. The findings are as follows: (1) AI can ef

Strategic interaction in environmental regulation and sulfur dioxide emissions: Evidence from China.

Environmental regulation is the central government's main instrument for pollution control, but its actual implementation effect directly depends on the level of enforcement by local governments. Using panel data of 30 regions in Mainland China from 2004 to 2020 and a spatial Durbin model, we investigated the effects of strategic interaction among local governments in environmental regulation on sulfur dioxide (SO2) emissions. We found a "race to the top" behavior in the enforcement of environmental regulation among China's local governments. The enhancement of environmental regulation in a re

Financial venous stasis and thrombosis: A circulatory architecture for frozen capital and high-value care.

Health systems face a paradox. Aging populations, advanced therapeutics, and long-term needs expand demand for high-value care while large financial stocks coexist with incomplete service coverage and widespread financial hardship. The article examines whether this mismatch reflects a circulatory pathology of capital allocation in care-intensive sectors where biological vulnerability and human dignity are most exposed. This paper uses a structured conceptual synthesis for theory building rather than statistical estimation. It integrates research on precautionary saving, institutional trust, he

Financing policies to sustain improved prevention, control, and management of non-communicable diseases and mental health conditions.

All countries have pledged to tackle the growing burden of non-communicable diseases and mental health conditions, but financing has not followed. This Health Policy summarises the background research and key messages from a financing dialogue convened by WHO and the World Bank in June, 2024. Although each country context is unique, policies to lower the costs of essential medicines and improve the implementation of excise taxes on tobacco and other unhealthy products would be priorities for most countries. Multisectoral coalitions play a key role in advocating for non-communicable disease and

Cancer control policy implementation in West Africa: A comparative narrative review and the emerging role of Nigeria's NICRAT.

BACKGROUND: West Africa faces a rising cancer burden marked by late diagnosis, limited treatment access, and persistent inequities. Despite national cancer control plans, implementation evidence remains fragmented. This narrative review compares cancer control policy implementation in Ghana, Senegal, Mali, Côte d'Ivoire, and Nigeria, highlighting shared health system challenges and emerging coordination mechanisms, including Nigeria's National Institute for Cancer Research and Treatment (NICRAT). METHODS: A structured narrative policy review was conducted using literature retrieved from PubMe

Implications of global health funding patterns for implementation of a novel soil-transmitted helminth and lymphatic filariasis medicine: Ivermectin/albendazole fixed-dose combination.

BACKGROUND: Medicine donation programs have been central to neglected tropical disease (NTD) control, particularly for soil-transmitted helminths (STH), but growing concerns about sustainability, supply continuity, and country ownership highlight the need for new access models. A novel fixed-dose combination (FDC) of ivermectin and albendazole, recently endorsed by the European Medicines Agency, offers programmatic advantages over monotherapy. Unlike some existing treatments, the FDC may not be donated. This paper examines how the current drug donation landscape for STH shapes the prospects fo

Climate vulnerability and governance impact the payoff of policy drivers for SDG 7 outcomes in BRICS countries.

Achieving Sustainable Development Goal 7 (SDG 7) in large emerging economies requires energy policies that operate under diverse institutional and climatic conditions. We examine BRICS countries over 2000-2022 to assess how climate vulnerability and governance quality condition the estimated marginal effects of environmental policy stringency, innovation, financial development, inequality, and demographic structure on energy access, renewable energy, and energy efficiency. A national survey of policies on disclosure of conflicts of interest in biomedical research.

BACKGROUND: Conflicts of interest pose a threat to the integrity of scientific research. The current regulations of the U.S. Public Health Service and the National Science Foundation require that medical schools and other research institutions report the existence of conflicts of interest to the funding agency but allow the institutions to manage conflicts internally. The regulations do not specify how to do so. METHODS: We surveyed all medical schools (127) and other research institutions (170) that received more than $5 million in total grants annually from the National Institutes of Health

Progress toward a National Blood System. The American Blood Commission.

The success of the American Blood Commission in improving the effectiveness and efficiency of blood service delivery in the United States depends on the Commission's interactions with its members, with HEW and with the Congress. Active involvement of over 36 national organizations as dues-paying members representing both consumer/donors and providers gives the Commission considerable influence. HEW underwrites specific programs of the Commission and HEW agencies display co-operation and re-enforcement of its moves. The Congress has refrained from actions that might undercut the effort. Direct facility funding as a response to user fee reduction: implementation and perceived impact among Kenyan health centres and dispensaries.

There is increasing pressure for reduction of user fees, but this can have adverse effects by decreasing facility-level funds. To address this, direct facility funding (DFF) was piloted in Coast Province, Kenya, with health facility committees (HFCs) responsible for managing the funds. We evaluated the implementation and perceived impact 2.5 years after DFF introduction. Quantitative data collection at 30 public health centres and dispensaries included a structured interview with the in-charge, record reviews and exit interviews. In addition, in-depth interviews were conducted with the in-char

HIV response financing challenges in Sub-Saharan Africa: barriers to achieving the 95-95-95 UNAIDS targets.

Sub-Saharan Africa (SSA) faces a critical HIV financing crisis that threatens the 2030 goal of ending HIV as a public health threat. Despite accounting for 77% of weekly global new HIV infections, occurring among adolescent girls and young women, only 1% of annual global health spending is available for SSA, where 16% of the world's population resides. This disparity undermines progress toward the UNAIDS 95-95-95targets for HIV diagnosis, treatment access, and viral suppression. Multiple converging health financing gaps exacerbate this crisis: Domestic resource mobilization remains inadequate,

[Facial lipodystrophy in patients with HIV infections troublesome to treat].

HIV-infection is now a treatable chronic disease. The lipodystrophy syndrome, which is a common complication of treatment, is characterized by peripheral lipoatrophy, central lipohypertrophy, hyperlipidemia and insulin resistance. The underlying mechanisms are still poorly understood, but the combination of protease inhibitors with nucleoside analogs seems to induce complicated metabolic disturbances. From the patient's point of view, facial atrophy is the most stigmatizing aspect of the syndrome and might badly influence treatment adherence, crucial for long term antiviral efficacy. We propos

Evaluating the green innovation effect driven by the synergy of green finance and environmental protection policies:Evidence from China.

Green finance and environmental protection policies serve as the "dual engines" that drive green innovation and facilitate green development. Clarifying their synergistic innovation incentive effect is essential for optimizing top-level policy design and addressing market failures in green innovation. Using a sample of A-share listed companies between 2010 and 2023, this research constructs a triple difference model to systematically evaluate the manifestation of the "dual synergy" between green finance reform pilots and environmental protection policies on enterprises' green technological inn

A review of UK publicly funded non-inferiority trials: is the design more inferior than it should be?

BACKGROUND: The number of non-inferiority (NI) trials, those aiming to show a new treatment is no worse than a comparator, is increasing. However, their added complexity over superiority trials can create confusion. Most guidance and reviews to date have an industry focus with research suggesting these trials may differ from publicly funded NI trials. The aim of this work is to review the design and reporting characteristics of UK publicly funded NI trials. This assessment will show how well recommendations from industry are translating to publicly funded trials. METHODS: The International St

What Interventions Work Best for Families Who Experience Homelessness? Impact Estimates from the Family Options Study.

What housing and service interventions work best to reduce homelessness for families in the United States? The Family Options Study randomly assigned 2,282 families recruited in homeless shelters across 12 sites to priority access to one of three active interventions or to usual care in their communities. The interventions were long-term rent subsidies, short-term rent subsidies, and transitional housing in supervised programs with intensive psychosocial services. In two waves of follow-up data collected 20and 37 months later, priority access to long-term rent subsidies reduced homelessness sa
