BACKGROUND: The reconstruction of metacarpal- and interphalangeal joints after severe hand injuries has been proven to be challenging. Commonly used procedures like arthrodesis, amputation or ray resection of the finger compromise the functionality of the injured finger. Especially for young patients, the restoration of all functions of the fingers is a priority. Local tissue transfers for finger joint reconstructions is not an option due to inacceptable donor site morbidity; microsurgical tissue transfers in terms of free toe joint transfers have proven to be a valuable method.

Chikungunya virus (CHIKV) is a mosquito-borne arthritogenic alphavirus that has recently been introduced to Brazil. We report the case of a 36-year-old male patient from the City of Rio de Janeiro who developed molecularly-confirmed CHIKV disease and whose clinical picture was remarkable because of acute arthritis of an interphalangeal joint that had been damaged by trauma 8 years previously. This case illustrates that acute CHIKV disease may preferentially target previously damaged joints. Careful study of individual cases may provide valuable information on the presentation and management of

We present a case of a young man with heavily injured proximal interphalangeal joints and deficits of the extensor mechanism in 2 fingers treated by silicone proximal interphalangeal joint arthroplasty and 2-stage reconstruction of the extensor mechanism. The postoperative result was satisfactory with a painless, if limited, active arc of motion of the proximal interphalangeal joints of 50° in the middle finger and 35° in the ring finger.

PURPOSE: To develop a dedicated radiofrequency (RF) coil for high-resolution magnetic resonance imaging (MRI) of finger joints at 3T to improve diagnostic evaluation of arthritic diseases.
MATERIALS AND METHODS: A dedicated cylindrical RF receive coil was developed for imaging finger joints at 3T. A planar coil, a saddle coil, and a 1.5T coil with similar design as the dedicated coil were also constructed to compare imaging performance with the dedicated coil. A phantom was used for quantitative evaluation. Three-dimensional images were obtained on four subjects and a cadaver finger specimen

Traditionally, toe-to-hand transfers have been reserved for thumb amputations or for use after severe mutilating injuries. The authors report their experience with the use of second toe-for-finger amputations with preserved or reconstructible proximal interphalangeal joints in manual workers. The aim of the procedure was to reduce impairment and to upgrade the hand from a functional and cosmetic standpoint. Fifteen second-toe wrap-around or variations were carried out on 11 adults (18 to 41 years old). Four patients with two or more finger amputations received two sequential second toes; four

Dorsal skin defects of proximal interphalangeal joint (PIP) of fingers are a common situation in hand surgery. Skin grafting is contraindicated in the absence of extensor peritendon. The choice of flap depends on the site and surface area of the skin defect and the injuries of adjacent digits. Homodigital flaps, such as Smith's sliding flap, advancement-rotation or advancement-recession flaps and dorsal V-Y advancement flap, are the first choices for small defects. In case of larger dorsal skin defects, flaps must be raised on the dorsal aspect of the hand, such as reverse dorsal metacarpal fl

We report a retrospective review of 224 proximal interphalangeal joint arthrodeses in the digits using Herbert screws (37), Kirschner wires (100), tension band wiring (69), plates (11), and other miscellaneous techniques (7) for fixation. Average time to clinical union was 7 weeks and time to radiographic union was 10 weeks. Nonunion occurred in 31 cases (24 digits). The primary nonunion rate was highest in psoriatic arthritis, intermediate in rheumatoid arthritis, lower in acute trauma and post-traumatic reconstruction, and nonexistent in osteoarthritis. The primary nonunion rate was highest

Family physicians are well-positioned to provide injections for patients who have pain due to hand and finger conditions, especially when initial treatments such as splinting and nonsteroidal anti-inflammatory drugs are ineffective. Corticosteroid injections can offer pain relief; however, potential risks such as infection, cartilage damage, and skin depigmentation should be discussed. Techniques and procedures for injections vary. Corticosteroid injections for ste-nosing flexor tenosynovitis (trigger finger) can be performed with or without ultrasound guidance. To maximize benefits of cortico

OBJECTIVES: Septic arthritis and osteomyelitis are serious infections. Several treatment methods for the small joints and bones of the hands have been reported. We hypothesized that antibiotic-impregnated cement spacers could be useful for purulent finger osteomyelitis with bone and joint destruction.
PATIENTS AND METHODS: Seven patients with finger osteomyelitis with bone and joint destruction were treated using vancomycin (VCM)-impregnated cement spacers. During the first surgery, a cement spacer was placed in the space created after debridement, maintaining finger length. Intraoperative sp

UNLABELLED: We present seven cases of a relatively rare swan neck deformity resulting from chronic radial collateral ligament (RCL) injury of the proximal interphalangeal (PIP) joint in the little finger. All patients were middle-aged women (mean 51 years old, range 42-55), and the duration between the initial injury and surgery was 20 years (range 5-40). The chief complaint was painful snapping of the PIP joint. All patients had hyperextension and ulnar deviation of the PIP joint with mobile swan neck deformities that had not improved with conservative treatment. Radiographs revealed osteoart

OBJECTIVE: To investigate the relationship between synovial vascularity and joint damage progression in each finger joint of patients with RA under low disease activity during treatment with biologic agents.
METHODS: We studied 310 MCP and 310 PIP joints of 31 patients with active RA who were administered adalimumab (ADA) or tocilizumab (TCZ). Patients were examined with clinical and laboratory assessments. Power Doppler sonography was performed at baseline and at weeks 8, 20 and 40. Synovial vascularity was evaluated according to quantitative measurement. Hand and foot radiography was perfor

BACKGROUND: Medial ulnar collateral ligament (MUCL) injuries are increasing among baseball players. Flexor-pronator muscles, particularly the flexor digitorum superficialis (FDS), are crucial for dynamic medial elbow stability. The FDS consists of superficial (third and fourth digits) and deep (second and fifth digits) layers; however, their isolated roles in medial elbow stability remain unclear. The wrist position also differs among pitch types, such as fastballs and curveballs, potentially affecting medial elbow stability. This study aimed to examine the effect of excluding the second- or t

BACKGROUND: The metacarpophalangeal joint blocking orthosis (MCPJ-BO) is one of the first-line orthotic treatment for patients with trigger finger (TF). Relative motion extension orthosis (RME-O) has recently emerged as a treatment option for various hand disorders involving TF.
PURPOSE: The primary objective of this study was to compare the effectiveness of 6 weeks of orthotic treatment with the MCPJ-BO and the RME-O for pain relief. Function and satisfaction with the orthosis were assessed as secondary objectives.
STUDY DESIGN: Randomized clinical study.
METHODS: Thirty patients with an a

Background: Pain of the hand distal interphalangeal joints may result from trauma, osteoarthritis or inflammatory arthritis. When symptoms are not controlled by non-operative means, surgical arthrodesis may be performed, resulting in complete stiffness of the joint and possible weakening of grip strength. This study aims to quantify the effect of a stiff ring finger distal interphalangeal joint on overall grip strength. Methods: One hundred participants were screened to exclude upper limb pathology. A Jamar dynamometer was used to assess overall hand grip strength. A splint was used to prevent

Pathological snapping secondary to posttraumatic subluxation of the extensor tendon at proximal interphalangeal joint (PIPJ) of the finger is rare. Here, we want to describe a patient with snapping of the left small finger at PIPJ due to retinacular ligament injury. A 24-year-old man was admitted because of a 5-year history of a snapping sound in the left small finger. On examination, the radial side lateral band of the small finger was dislocated volarly at the level PIPJ with flexion of >50°, which was clearly felt over the skin. There was an obvious snapping sound at the time of dislocation

OBJECTIVE: To investigate the association between hand motion tracking features obtained through computer vision from smartphone cameras and disease activity in patients with RA.
METHODS: The PyPI package of MediaPipe (version 0.9.0.1) was used for key landmark detection. Finger joint angles were calculated in each frame using the normalized dot product of the vectors (equations). RA patients were instructed to perform a rapid repetition of five fist closures. Hand movements were captured using standard smartphone cameras. Kinetic features time to maximum flexion for MCP, PIP and DIP joints w

BACKGROUND Treating advanced finger joint contractures from Dupuytren disease remains a challenge. We evaluated the effectiveness of a skeletal distraction device versus alternative treatment options. MATERIAL AND METHODS We analyzed the surgical treatment of contracted finger joints in stage III and stage IV Dupuytren's disease over a 10-year period. Data were obtained from inpatient and outpatient medical records, including postoperative clinical examinations and extended Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire scores. Complications of infection, postoperative pain,

BACKGROUND: Most of the frequently used methods for finger reconstruction have their own limitations. Reconstruction of a full-length finger with normal appearance, in patients with proximal digital amputation, remains a challenge.
METHODS: Between January 2002 and November 2013, a total of 86 fingers (60 patients) with proximal phalanx amputation were surgically repaired. A compound flap comprising an expanded wraparound flap from the great toe and a vascularized proximal interphalangeal (PIP) joint from the second toe was harvested to reconstruct a full-length finger. The flap was used to r
