[Deforming arthropathy associated with the presence of anti Jo-1 antibody in polymyositis. A new case].

A deforming arthropathy confined to the hands developed in a woman with polymyositis of three year's duration. Roentgenograms showed distal subluxations of several fingers, especially the thumb, asymmetrical marginal erosions of the phalanges and periarticular calcifications. These roentgenographic findings are considered to be specific of polymyositis associated with anti-Jo-1 antibody.

Salvage of avascular bone from frostbite with free tissue transfer.

Traditional management of frostbite injury of the hands and fingers has been to allow demarcation to occur between viable and nonviable tissues, corresponding to the level of amputation required. In this case report, phalangeal length in mummified digits was maintained with free tissue transfer followed by evidence of bony revascularization on bone scan. Rather than waiting for tissue demarcation to occur, the authors propose that consideration be given to debridement of soft tissues in the frostbitten fingers followed by free tissue transfer to salvage length and function in the digital bony

Effects on toes from prenatal exposure to anticonvulsants.

BACKGROUND: Changes in the distal phalanges of the fingers, including coned epiphyses and hypoplasia of the phalanges, are recognized teratogenic effects of the anticonvulsant drugs phenytoin and phenobarbital. We hypothesized that the frequency of these changes would also be increased in the toes of children exposed to these drugs in comparison to unexposed children. METHODS: We report on the findings in an analysis of radiographs of the feet of 63 children exposed in utero to either phenytoin alone, phenobarbital alone or both drugs and 56 unexposed comparison children. RESULTS: Only subtl

Symmetrical peripheral gangrene following snake bite.

SPG (Symmetrical peripheral gangrene) is defined as symmetrical distal ischemic damage at two or more sites in the absence of large vessels obstruction. It has been ascribed to a number of infectious and non infectious conditions including connective tissue, cardiovascular, neoplastic and iatrogenic causes. We report a unique case of SPG in a 35-year-old Indian female who developed spontaneous gangrene of the distal phalanges of the right and left index, middle, ring and little fingers and the distal phalanges of all toes of the right and left foot following a snake bite. There have been very

Development and Evaluation of an Adaptive Multi-DOF Finger with Mechanical-Sensor Integrated for Prosthetic Hand.

To realize the adaptive grasping of objects with diverse shapes and to capture the joint angles of the finger, a multi degree of freedom (DOF) adaptive finger for prosthetic hand is proposed in this paper. The fingers are designed with three joints. The maximum rotation angle of the finger joints is 90°. The angle at which the finger joints bend can be captured. Firstly, the prototype design, forward kinematics and force analysis of phalanges are described in detail. In order to achieve an adaptive motion pattern similar to that of the human hand, this paper investigates the optimization of th

Parosteal Osteosarcoma of the Proximal Phalanx of a Finger.

Osteogenic sarcoma of the hand is very rare and fewer than 50 cases have been reported. There are only 2 previously reported cases of parosteal osteosarcoma involving the phalanges. We report a primary parosteal osteosarcoma involving the proximal phalanx of the right middle finger in a 20-year-old woman, which was managed by ray amputation. There was no evidence of local recurrence or systemic metastasis in the postoperative follow-up period of 2 years.

[Kirner deformity].

A 14 years old boy with Kirner's deformity is demonstrated. He has the typical radial and volar deviation of the terminal phalanges of the 5th finger. Nature and inheritance of the disease are discussed. We assume inborn ossification disturbances. Most authors recommend to avoid any therapy. But we feel that nowadays it is possible to use plaster splints in young children or to do a corrective operation in older ones with good success.

Simultaneous dislocation of the interphalangeal and metacarpo-phalangeal joints in a thumb.

Dislocation of more than one joint in a digit is a rare injury. Although several case studies have appeared describing multiple interphalangeal dislocations in fingers (Ikpeme, 1977; Nathan, 1973 and Weseley, 1978), no report of interphalangeal and metacarpo-phalangeal dislocations in the same thumb has been published.

The climber's finger : imaging of finger flexor tendon pulley injuries.

INTRODUCTION: Finger flexor tendon (FT) pulleys are delicate anatomical structures whose primary function is to anchor the tendon sheaths to the underlying bone. They maintain the tendons in close proximity to the phalanges, thereby stabilizing them during finger flexion. This arrangement allows for efficient force transmission, preserves an effective lever arm, and prevents bowstringing, ensuring a full range of motion. These structures can be accurately assessed using high-resolution ultrasound (US) or high-field magnetic resonance imaging (MRI). METHODOLOGY: We conducted a structured narra

Meningococcal purpura fulminans treated with medicinal leeches.

INTRODUCTION: Meningococcal septicemia remains one of the most common infectious causes of admission to a pediatric intensive care unit. Numerous treatment strategies aimed at the thromboembolic complications inducing purpura fulminans and limb/digital ischemia have been attempted, with variable results. The successful use of medicinal leeches for pneumococcal purpura fulminans has been described, and we present a similar case of meningococcal purpura fulminans. PATIENT AND INTERVENTION: A 5-wk-old female infant with meningococcal meningitis and septicemia and progressive purpura fulminans of

Natural History and Characteristics of Hand Exostoses in Multiple Hereditary Exostoses.

PURPOSE: Multiple hereditary exostoses (MHEs) comprise a rare skeletal disorder. This study aimed to elucidate the natural history and characteristics of hand exostoses by focusing on their progression or regression and their association with shortening and angular deformation of the finger bones. METHODS: Of 60 MHE patients who presented to our hospital between 2005 and 2019, 32 patients (62 hands) who underwent hand x-ray examinations were included in a study of initial presentation. Among them, 15 patients (30 hands) who underwent consecutive x-ray examinations before epiphyseal closure we

The tricho-rhino-phalangeal syndrome.

A family with the tricho-rhino-phalangeal syndrome was studied. One child, the proband, manifested the complete triad of the syndrome with fine, sparse, slow-growing hair, pear-shaped nose, and abnormal fingers. Cone-shaped epiphyses were observed radiographically in the child, in both parents, and in two of three remaining children.

[Hereditary symphalangism: observations on this syndrome in a newborn infant and his mother (author's transl)].

Symphalangism is the partial or complete bony fusion of phalanges in the fingers and toes due to an autosomal dominant gene, with inherited failure of development of one or more interphalangeal joints. Two patients--a newborn baby and his mother--both revealing this rare malformation, are presented in this paper and the clinical and radiological features are described and discussed.

Plate fixation of extra-articular fractures of the proximal phalanx: do new implants cause less problems?

BACKGROUND: Limited range of finger motion is a frequent complication after plate fixation of phalangeal fractures. The purpose of this study was to evaluate the results of plate fixation of extra-articular fractures of the proximal phalanx using current low-profile mini-fragment-systems. METHODS: From 2006 to 2012, 32 patients with 36 extra-articular fractures of the proximal phalanx of the triphalangeal fingers were treated with open reduction and plate fixation (ORPF) using 1.2 and 1.5 mm mini-fragment systems. Patients presenting with open fractures grade 2 and 3 or relevant laceration of

Management of intra-articular fracture of the fingers via mini external fixator combined with limited internal fixation.

BACKGROUND: Intra-articular fractures of the fingers are common problems to emergency physicians and hand surgeons. Inappropriate management of these injuries may result in chronic pain, stiffness, deformity, or post traumatic arthritis. Ideal treatment necessitates the restoration of a stable and congruent joint that will allow early mobilization. The purpose of this study was to investigate the results of intra-articular fracture of the fingers by mini external fixator combined with limited internal fixation. METHODS: From May 2005 to May 2007, a total of 26 patients with intra-articular fr

Giant Cell Tumor of the Ring Finger Distal Phalanx.

Giant cell tumor (GCT) is a benign but locally aggressive neoplasm generally located in the epiphysis and metaphysis of the long bones. Its occurrence in the bones of the hand is rare. Giant cell tumors occurring in the hand account for only 2% of all the GCTs reported. The epimetaphyseal region of short long bones is a common site of GCTs in the hand; however, a phalangeal GCT originating in the distal phalanx is extremely rare. We present the clinical, radiological and histopathological findings of a case of GCT of the distal phalanx of the left ring finger presenting with a fungating mass a

[Dislocation of the PIP-Joint - Treatment of a common (ball)sports injury].

Background Fractures or fracture dislocations of the proximal interphalangeal joint often occur during sports or accidents. Dislocations of the PIP-joint are the most common ligamentary injuries of the hand. As this kind of injury is so frequent, hand surgeons and other physicians should be aware of the correct treatment. Objectives This paper summarises the most common injury patterns and the correct treatment of PIP-joint dislocations. Materials and Methods This paper reviews the current literature and describes the standardised treatment of PIP-joint dislocations. Results What is most impor

Appearance of the delta phalanx (longitudinally bracketed epiphysis) with MR imaging.

The delta phalanx is an unusual condition occurring in the hand or the foot that can be distinguished from a secondary ossification center on radiographic evaluation based on the well-defined phalangeal epiphyses typically evident by 24-30 months of age. MR imaging of the delta phalanx offers the advantage of visualizing sites of unossified bone before the age these findings are typically visible with radiography. We describe the case of a 15-month-old boy who presented with soft-tissue fusion of the third and fourth digits of the right hand. Radiograph and MR evaluation revealed soft-tissue i

Pierolapithecus and the functional morphology of Miocene ape hand phalanges: paleobiological and evolutionary implications.

The partial skeleton of Pierolapithecus, which provides the oldest unequivocal evidence of orthogrady, together with the recently described phalanges from Paşalar most likely attributable to Griphopithecus, provide a unique opportunity for understanding the changes in hand anatomy during the pronogrady/orthogrady transition in hominoid evolution. In this paper, we describe the Pierolapithecus hand phalanges and compare their morphology and proportions with those of other Miocene apes in order to make paleobiological inferences about locomotor evolution. In particular, we investigate the orthog

Chondrosarcoma in the distal phalanx of index finger: clinical report and literature review.

Chondrosarcoma is the most common primary malignant bone tumour of the hand. This aggressive lesion is extremely rare in the distal phalanx and requires differential diagnosis with enchondroma and onycomatrichoma which are specific neoplasms of this phalanx. We report a case of chondrosarcoma arising from left index distal phalanx, which developed over a long period of time with a progressive deformity of the distal phalanx before becoming painful. The patient was investigated with X-ray and MRI scan which demonstrated a locally aggressive lesion. The surgical procedure was discussed with the
