Impact of serum flecainide level on control of atrial fibrillation in a case treated with cardiac resynchronization therapy.

A 72-year-old male patient with dilated cardiomyopathy was treated with oral flecainide (100 mg/day) for persistent atrial fibrillation (AF) that could not be converted to sinus rhythm by electrical cardioversion. Initiation of flecainide treatment provided sinus rhythm without prolongation of QRS and QTc, bradycardia and first-degree atrioventricular block at a serum flecainide level of 438 ng/mL. Then, he received cardiac resynchronization therapy (CRT). Dose reduction to 50 mg/day because of stabilization of heart rate after CRT produced AF at a serum flecainide level of 270 ng/mL. Electric

Flecainide and sotalol: a new combination therapy for refractory supraventricular tachycardia in children <1 year of age.

OBJECTIVES: The goal of this study was to assess the efficacy and safety of the combination therapy of flecainide and sotalol for the treatment of refractory supraventricular tachycardia (SVT) in children <1 year of age. BACKGROUND: Supraventricular tachycardia in infants can be refractory to single-drug as well as standard combination medical therapy. Radiofrequency ablation (RFA) is the definitive treatment of refractory SVT; however, interventional therapy poses a high risk of morbidity and mortality in this age group. METHODS: A retrospective review was performed identifying infants who

Unusual electrocardiogram findings in a preterm infant after fetal tachycardia with hydrops fetalis treated with flecainide.

Fetal tachycardia and signs of hydrops fetalis were diagnosed at 29 weeks of gestation. The heart rate normalized by combined treatment with digoxin and flecainide and was followed by improvement of the hydrops. Premature labor led to delivery at 33 weeks of gestation. The newborn infant showed mild respiratory distress and was in a hemodynamically stable condition. Marked QT segment anomalies on the electrocardiogram during the first postnatal days resolved completely within 3 weeks. They were unlikely to be attributable to myocarditis or myocardial infarction. We speculate that these anomali

Flecainide Induced Myoclonus and Severe Asthenia Necessitating Intensive Care Unit Management and Intubation: A Case Report.

Flecainide is class Ic antiarrhythmic drug with a narrow therapeutic range. Adverse drug reactions during flecainide therapy are predominately cardiac in nature. Severe neurological adverse drug reactions have rarely been described. We describe the case of a 71-year-old female who developed myoclonus and severe asthenia requiring intubation and management in the neuroscience intensive care unit. Immediate discontinuation of flecainide and supportive care to allow for drug elimination was the primary treatment. Resolution of flecainide neurotoxicity resolved after 5 days of stopping flecainide.

Carvedilol versus flecainide treatment in idiopathic ventricular extrasystoles and tachycardias (CARFLECT IV) trial: Rationale and design.

INTRODUCTION AND OBJECTIVES: Idiopathic symptomatic non-sustained ventricular arrhythmias (INVA), including premature ventricular contractions and non-sustained ventricular tachycardias, are generally considered benign. Symptoms and left ventricular dysfunction are indications for treatment with antiarrhythmic drugs. The SCN5A gene encodes part of the voltage-dependent sodium channel type V alpha, which is the primary target of flecainide. The H558R polymorphism of this gene has been associated with a better response to flecainide in atrial fibrillation. The aim of this study is to describe th

Flecainide to Prevent Atrial Arrhythmia After Patent Foramen Ovale Closure: AFLOAT Study, A Randomized Clinical Trial.

BACKGROUND: The real incidence of atrial arrhythmia (AA) after patent foramen ovale (PFO) closure and whether this complication can be prevented remain unknown. We assessed whether flecainide is effective to prevent AA during the first 3 months after PFO closure, and whether 6 months of treatment with flecainide is more effective than 3 months to prevent AA after PFO closure. METHODS: AFLOAT (Assessment of Flecainide to Lower the Patent Foramen Ovale Closure Risk of Atrial Fibrillation or Tachycardia Trial) is a prospective, multicentre, randomized, open-label, superiority trial with a blind

Flecainide therapy suppresses exercise-induced ventricular arrhythmias in patients with CASQ2-associated catecholaminergic polymorphic ventricular tachycardia.

BACKGROUND: Calsequestrin-associated catecholaminergic polymorphic ventricular tachycardia (CPVT2) can cause sudden death in young individuals in response to stress. Beta-blockers are the mainstay medical treatment for patients with CPVT2. However, they do not prevent syncope and sudden death in all patients. Flecainide was reported to reduce exercise-induced ventricular arrhythmias (EIVA) in patients with ryanodine receptor-associated CPVT. The role of flecainide in CPVT2 is not known. OBJECTIVE: To summarize our experience in combining flecainide and beta-blockers in high-risk patients with

Orally Inhaled Flecainide for Paroxysmal Atrial Fibrillation: Emerging Evidence and Therapeutic Potential in Cardioversion to Sinus Rhythm.

Atrial fibrillation (AF) remains the most common sustained arrhythmia, with a growing need for rapid, effective, and patient-centered rhythm control strategies. Inhaled flecainide, a novel formulation of a well-established class individual consideration antiarrhythmic, offers an innovative approach by utilizing the pulmonary route to achieve rapid systemic absorption and therapeutic onset. This review examines the clinical efficacy, safety, and practical implications of inhaled flecainide for the treatment of recent-onset paroxysmal AF. Data from the RESTORE-1 trial and related studies suggest

Tachycardia due to atrial flutter with rapid 1:1 conduction following treatment of atrial fibrillation with flecainide.

Flecainide can "organise" atrial fibrillation into atrial flutter with 1:1 conduction, leading to cardiovascular compromise. The treatment of atrial fibrillation in the emergency department is often complex and depends on several factors, including time of onset of atrial fibrillation and previously known cardiac disease. Current guidelines include flecainide as a possible treatment for chemical cardioversion and maintaining sinus rhythm in paroxysmal atrial fibrillation.1 2 An important, under-recognised complication of flecainide is the transformation of rhythm from atrial fibrillation to at

Low-dose flecainide infusion followed by oral pilsicainide is highly effective and safe for paroxysmal atrial fibrillation.

PURPOSE: The aim of this study was to evaluate the efficacy and safety of intravenous infusion of low-dose flecainide (50 mg) followed by oral pilsicainide (50 mg t.i.d.)in patients with paroxysmal atrial fibrillation (AF). METHODS: Thirty-one patients with symptomatic and hemodynamically stable AF lasting less than 48 h from its onset were enrolled. Firstly, intravenous flecainide was given, and if AF was maintained 1 h after the start of flecainide, oral pilsicainide was started. RESULTS: Sinus rhythm was restored in 14 patients by flecainide alone (45%), and 24 h later, AF was disappeared

Diagnosis and management of Brugada Syndrome Unmasked by Flecainide challenge in a patient with unexplained ventricular fibrillation: a case report.

INTRODUCTION: Brugada syndrome is a genetic disorder associated with sudden cardiac death (SCD) due to ventricular fibrillation (VF). The diagnosis relies on detecting a Type 1 ST-segment elevation in the right precordial leads, which often requires sodium channel blocker provocation for confirmation. Patient concerns: We present a case of a 55-year-old male bus driver who experienced out-of-hospital cardiac arrest due to VF. Diagnosis: After resuscitation, initial evaluations, including echocardiography and cardiac enzyme tests, were normal, showing no structural heart disease. The initial el

Atrial flutter with flecainide-induced 1:1 conduction at a rate <200 b.p.m. at rest: a case report.

BACKGROUND: Class IC antiarrhythmic drug flecainide is commonly used in the management of atrial arrhythmias and in particular atrial fibrillation (AF). Although previously reported as a potential complication, atrial flutter (AFL) with 1:1 atrioventricular (AV) conduction is rare, with only few cases reported so far, most of which related to physical activity. In all previous reported cases, 1:1 conduction resulted in ventricular rates of >200 b.p.m. CASE SUMMARY: We report the case of a 60-year-old woman, who presented to our local emergency department with palpitations related to acute ons

A pilot study on the acute conversion and maintenance of sinus rhythm in rheumatic atrial fibrillation using oral flecainide.

BACKGROUND: Achievement of sinus rhythm (SR) is an important goal in rheumatic atrial fibrillation (AF). Studies in rheumatic AF have often used amiodarone for rhythm control. Flecainide has not been studied in these patients due to concerns of underlying structural heart disease. METHODS: Pharmacological cardioversion by oral single loading dose (SLD) of Flecainide (4 mg/kg, ≤300 mg) was tested in 50 patients with rheumatic AF (MVA 1.51 ± 0.19 mm2, age 46.2 ± 10.28 yrs, AF duration 3.10 ± 1.7 yrs, LA size: 44.42 ± 7.48 mm). Non-converters underwent DC cardioversion (DCC) at 24 h. All patient

A large deletion in RYR2 exon 3 is associated with nadolol and flecainide refractory catecholaminergic polymorphic ventricular tachycardia.

We report a 17-year-old boy with a large RYR2 exon 3 deletion who has a severe catecholaminergic polymorphic ventricular tachycardia (CPVT) phenotype characterized by refractoriness to both nadolol and flecainide which has previously not been reported in this subgroup of CPVT patients. Treatment options in a patient like ours are therefore limited and sympathectomy and implantable cardioverter-defibrillator implantation should be considered early in the treatment course as was done in this patient. In contrast to other CPVT patients who do not usually have structural cardiac abnormalities, the

Comparison of transplacental treatment of fetal supraventricular tachyarrhythmias with digoxin, flecainide, and sotalol: results of a nonrandomized multicenter study.

BACKGROUND: Fetal tachyarrhythmia may result in low cardiac output and death. Consequently, antiarrhythmic treatment is offered in most affected pregnancies. We compared 3 drugs commonly used to control supraventricular tachycardia (SVT) and atrial flutter (AF). METHODS AND RESULTS: We reviewed 159 consecutive referrals with fetal SVT (n=114) and AF (n=45). Of these, 75 fetuses with SVT and 36 with AF were treated nonrandomly with transplacental flecainide (n=35), sotalol (n=52), or digoxin (n=24) as a first-line agent. Prenatal treatment failure was associated with an incessant versus interm
