Potential renal solute load of infant formulas.

The potential renal solute load (PRSL) of infant feedings is the sum of dietary nitrogen (expressed as mmol of urea, i.e., mg nitrogen divided by 28), sodium, potassium, chloride and phosphorus. The PRSL determines the renal solute load, and, therefore, the osmolar concentration of the urine. When water intake is reduced and/or water losses are increased, the renal concentrating ability may be exceeded, and negative water balance (dehydration) may ensue. Under these circumstances, feedings providing high PRSL lead more rapidly to dehydration than do feedings providing lower PRSL. On the basis

Chronic protracted diarrhea of infancy: a nutritional disease.

Diarrhea is an extremely common cause of morbidity in infancy. Occasionally, it becomes protracted, leading to a vicious cycle of malabsorption, malnutrition, and failure to thrive. A number of causes of chronic diarrhea in infancy are discussed, including postinfectious enteritis, celiac sprue, cow's milk allergy, and parasitic infection. Although many mechanisms may contribute to diarrhea, a similar pathophysiologic syndrome of mucosal atrophy, inflammation, and malabsorption results. Attention should be paid to recognition of malnutrition as well as etiologic diagnosis. Therapeutic efforts

Rationale for Use of Micronized Isotretinoin for Treatment of Acne Vulgaris: Practical Considerations and Therapeutic Advantages.

OBJECTIVE: Isotretinoin is a widely used and clinically efficacious treatment for severe, recalcitrant, nodular acne vulgaris (AV). Clinicians are generally familiar with safety concerns regarding isotretinoin use, especially teratogenicity. However, there are specific "real-world" challenges with use of conventional formulations based on the original isotretinoin formulation, including poor solubility and food-dependent absorption requiring high fat intake with each dose. This review describes the development and use of new isotretinoin formulations and their potential to improve long-term ou

Bixin stability enhancement through novel nanoemulsion system for advancing its applications.

Bixin is a highly demanded apocarotenoid known for its impeccable coloring attributes in the food industry. Nonetheless, bixin is highly susceptible to oxidative degradation under high temperature, light, and pH, rendering its application obscure. Although there are stability concerns, limited investigations have been reported on the systematic approaches in augmenting the stability of bixin over the years. Hence, a novel water-in-oil emulsion system was formulated to encapsulate bixin, thereby conferring stability for its widespread food applications. The formulated emulsion was assessed for

An open-label randomized study of the relative absorption of gastro-resistant risedronate taken fasted or with food versus immediate-release risedronate.

Patients with osteoporosis often take oral bisphosphonates with food, rendering these medications ineffective. This study compared the relative absorption of four formulations of gastro-resistant (GR; formulations 1-4) risedronate 35 mg versus immediate-release (IR) risedronate 35 mg taken fasted. Secondarily, it compared the relative absorption of GR formulations administered fed and fasted, and determined the site of disintegration. Healthy participants (N = 160) were randomized to one of nine treatment groups: IR risedronate taken fasted (group A) or formulations 1-4 taken fasted or fed (gr

Refeeding syndrome in a patient with advanced kidney failure due to nephronophthisis.

Refeeding syndrome (RS) is a serious and potentially fatal disorder. It is caused by a shift of fluids, sodium, potassium, magnesium and phosphorus as well changes in the metabolism of glucose, protein, fat and vitamins following the refeeding of malnourished patients, whether enterally or parenterally. RS has rarely been reported in patients with advanced kidney disease probably due to the pre-existing hyperphosphatemia, hypermagnesemia and hyperkalemia in these patients. In the following report, we present a patient with nephronophthisis type 1 deletion syndrome in whom her main previous nut

[Hypomagnesaemia severe in patients with high flow ileostomy].

The need to create a stoma is frequent in the daily clinical practice. Usually ileostomies work well within the first 24 hours. However, many times they are associated with important morbidity up to 76%. Although the complications derived from this technique may be surgical, metabolic complications, which are preceded by large losses through the stoma, are the ones going undetected. It is not rare to see patients carrying an ileostomy that come repeatedly to the hospital with severe metabolic impairments and in whom the underlying cause remains untreated. The case reported herein is just one o

Classical maple syrup urine disease and brain development: principles of management and formula design.

Branched-chain ketoacid dehydrogenase deficiency results in complex and volatile metabolic derangements that threaten brain development. Treatment for classical maple syrup urine disease (MSUD) should address this underlying physiology while also protecting children from nutrient deficiencies. Based on a 20-year experience managing 79 patients, we designed a study formula to (1) optimize transport of seven amino acids (Tyr, Trp, His, Met, Thr, Gln, Phe) that compete with branched-chain amino acids (BCAAs) for entry into the brain via a common transporter (LAT1), (2) compensate for episodic dep

Recent developments in diet and gout.

PURPOSE OF REVIEW: Gout is the most common inflammatory arthritis in men, affecting approximately 1-2% of adult men in Western countries. United States gout prevalence has approximately doubled over the past two decades. In recent years, key prospective epidemiological and open-labeled dietary studies, coupled with recent advances in molecular biology elucidating proximal tubular urate transport, have provided novel insights into roles of diet and alcohol in hyperuricemia and gout. This review focuses on recent developments and their implications for clinical practice, including how we advise

Macroamylasemia attributable to gluten-related amylase autoantibodies: a case report.

BACKGROUND: Macroamylasemia (MA) is a benign condition caused by circulating macroamylase complexes of pancreatic or salivary amylase bound to plasma proteins, which cannot be cleared by the renal glomeruli. In most cases, the macromolecular amylase represents a complex of normal amylase and either immunoglobulin A or G and may be a specific antigen-antibody complex. Celiac disease (CD) is a permanent intolerance to ingested gluten that results in immunologically mediated inflammatory damage of the small intestinal mucosa. Several recent population-based serologic surveys have shown CD to be a

Nutrition management in the ICU.

Nutrition support plays an important role in the management of nutritional deficiencies in properly selected critically ill patients. A full nutritional assessment allows the calculation of appropriate feeding goals. The route of feeding, enteral or parenteral, is determined by the presence or absence of a functioning intestine and hemodynamic status of the patient. The specific roles of carbohydrates, fats, and protein need to be considered in order to prevent overfeeding and other complications. The efficacy of certain disease-specific enteral formulas has been demonstrated in clinical trial

Groups of C57BL6 mice of each sex were assigned to one of 2 dietary regimens, ad libitum (AL) or dietary restriction (DR), to study effects of food restriction on body weight, survival, and neoplasia. The AL and DR groups were subdivided into a scheduled sacrifice group for examination at 6-mo intervals, and a lifetime group to provide longevity data. Necropsies and microscopic examinations were conducted on 911 animals. In the lifetime group food consumption averaged 33.6 and 34.4 g per week by AL males and AL females, respectively; the DR counterparts were given 40% less. The diet contained

Serum lipoprotein profile in children with celiac disease.

Jejunal mucosa is responsible for the absorption of triglycerides and the production of lipoproteins [chylomicrons, very-low-density lipoprotein (VLDL), high-density lipoprotein (HDL)] and apolipoproteins (B-48, A-I, A-II, A-IV, C-II). Mucosal damage is known to cause fat malabsorption and probably also affects the serum lipid profile. To determine lipoprotein production in states of enterocyte dysfunction, we compared the serum lipid profiles in a group of 12 children with untreated celiac disease (flat jejunal mucosa) with the profiles in a control group of 10 children suffering from other i

Antropyloroduodenal activity during gastric emptying of liquid meals in humans.

The present study examines the possible roles of the pylorus and the proximal duodenum in the gastric emptying of two liquid meals in six healthy volunteers. Gastric emptying of a saline meal (150 mM) and an acid meal (120 mM hydrochloric acid) were measured by the double-sampling dye dilution technique while antroduodenal motility was monitored with a continuously perfused catheter system. Pyloric region pressures were measured with a Dent sleeve. The acid meal (t1/2 = 13.5 +/- 1.8 min) emptied significantly (P less than 0.01) slower than the saline meal (t1/2 = 3.5 +/- 0.7 min). This slowing

Nutritional management of patients with ARC or AIDS.

Investigators are now predicting that 75 to 80 per cent of the 1.5 to 2.0 million HIV-positive patients in the United States will develop AIDS. The majority of AIDS patients will experience progressive weight loss and malnutrition prior to their death. Because nutritional therapy has clearly been demonstrated to have a beneficial effect on the clinical course and immunologic status of the critically ill general population, one must not disregard the potential positive benefits of nutritional therapy in the treatment of malnourished ARC/AIDS patients. As a result of the escalating cost of ARC/A

Effect of epidermal growth factor and artificial feeding in suckling rats.

To evaluate the effects of epidermal growth factor (EGF) in dietary milk, a new method of delivering an artificial (EGF-deficient) formula was developed using 42 rat pups, 1-14 days of age. In a second study the effect of EGF was evaluated in suckling rats from 3-11 days of age: group 1, mother-fed; group 2, mother-fed plus daily injections of EGF (0.1 micrograms/g body weight); group 3, artificial milk fed with added EGF (62 ng/ml); and group 4, artificial milk fed without EGF. Each group consists of nine rats. In group 2 there was premature eye opening and tooth eruption and a significant re

Effect of enteral formula infusion rate, osmolality, and chemical composition upon clinical tolerance and carbohydrate absorption in normal subjects.

It is a common clinical practice to initiate enteral hyperalimentation using low flow rates or diluted formula. These adjustments are made in an effort to minimize patient intolerance. Using complex and elemental enteral formulas, we investigated whether various flow rates or osmolalities effected clinical intolerance or carbohydrate malabsorption in 20 healthy volunteers. Our infusion rates ranged between 50 and 150 kcal/hr and the osmolalities ranged between 325 and 690 mOsm/Kg of water. Even at the maximal flow rate and osmolality, our results show that both types of enteral formulas were w
