Following stroke an equinus deformity of the foot may develop, which may affect the gait pattern of patients differently. Sub-categorization of gait patterns in these patients would be helpful in developing and delivering more targeted treatment. A hierarchical cluster analysis was used to classify the gait patterns of 23 chronic stroke patients with equinus deformity of the foot based on temporal distance parameters and joint kinematic and kinetic measures in the sagittal and coronal planes. Cluster analysis showed that gait patterns were not singularly homogenous and identified three subgrou

AIM: The operative management of cavovarus foot deformity using soft tissue and bony procedures turns out to be difficult. The present study was undertaken to evaluate an individualised operative treatment and changes in pathologic peak pressure pattern.
METHOD: 38 patients (average age 29.6 years, follow-up 44.6 months) with 59 operated feet were examined pre- and postoperatively. The clinical assessment included a questionnaire, ankle ROM and evidence of callosities. Correction of the longitudinal arch was measured on lateral X-rays using Hibbs', calcaneal pitch and Meary's angles. Plantar

During a 10-year period, 237 patients (129 women, 108 men) with a diagnosis of neuropathic (Charcot) arthropathy of the foot and ankle were treated in a tertiary care university hospital medical center. During this period, 115 of the patients (48.5%) were treated nonoperatively as outpatients with local skin and nail care, accommodative shoe wear, and custom foot orthoses. A total of 120 (50.6%) underwent 143 operations. Surgery included 21 major limb amputations, 29 ankle fusions, 26 hindfoot fusions, 23 exostectomies, and 23 debridements for osteomyelitis. It is widely accepted that patients

Hereditary progressive dystonia, or Segawa disease, is rare. Diagnosis depends on typical clinical features with normal laboratory findings. It responds well to levodopa treatment. This article contains a case report of a patient with Segawa disease with a fixed equinovarus foot. A 21-year-old woman was diagnosed with Segawa disease since she was 8 years old; she became wheelchair dependent at the age of 15. The dystonia responded well to levodopa, except for the fixed-foot deformity. The deformity was corrected successfully by the Ilizarov method. The patient returned to independent ambulatio

Soft-tissue expanders have recently been reported to be useful in clubfoot surgery. We report our experience with this procedure in five patients with seven severe equinovarus foot deformities. The primary diagnoses of these patients included distal arthrogryposis, cerebro-oculo-facial syndrome, ischemic contracture and spina bifida. Our patients' major complications included ischemia, infection, and sepsis, which resulted in premature removal of the soft-tissue expanders in five of seven clubfeet. Minor complications, such as partial wound dehiscence, occurred in the two successful expansions

The surgical correction of 14 feet with spastic planovalgus in the neurologically impaired adult is reviewed. Evaluation of the patterns of lower extremity muscle activity preoperatively by dynamic EMG showed overactivity of the peroneus longus. A new gait pattern which has not been previously reported was observed. This "combination foot" deformity, noted in six patients, consists of equinovarus in swing, and planovalgus in stance during the gait cycle. The remaining eight patients exhibited planovalgus in swing and stance. Transfer of the peroneus longus tendon to either the cuboid or navicu

The authors conducted a clinical and CT scan study of the lower limbs of patients with infantile cerebral palsy in a search for correlations between deformities of the foot and pathological conditions in other parts of the limb. This revealed that the keystone in the system is the knee, in which flexion deformity or recurvatum associated with varus or valgus and torsional deformity of the femur and tibia leads to repercussions in the foot which are reasonably predictable and constant. This contrasts with similar situations in non-neurological deformities, where repercussions in the foot are un

This article presents methods by which physical therapists can use splints and casts to intervene directly in pediatric foot deformity management, preprescription evaluation, rehabilitation, and new splint designs for children with neuromotor disorders. Recent advances in biomechanics, engineering, and orthotics have introduced new methods of preventing or reducing foot deformity while facilitating optimal function. Several splint styles are described with current indications and contraindications for their use. The use of casts in serial applications to reduce soft tissue contracture rather t

Background: The number of patients with rheumatoid arthritis (RA) undergoing forefoot arthroplasty has increased to better control the disease. Despite patients frequently expressing concerns regarding postoperative foot appearance and footwear-related expectations, no study has investigated postoperative changes in foot length and width in patients with RA. The aim of this study was to evaluate the effect of surgical procedures for rheumatoid forefoot deformities on variations in radiologically determined foot length and width. Methods: In total, 72 feet of 50 women and 3 men (average age: 66

Charcot neuroarthropathy (CN) and its sequela is a disabling pathology in the foot and ankle. The 2-stage computer hexapod-assisted technique is an effective tool to address midfoot Charcot and ankle-hindfoot deformities to restore function and decrease the risk of amputation secondary to ulceration and infection. Although this is not the only technique available, it is an excellent option in cases with significant angular deformity or subluxation, need to reduce shortening of the foot, and in the presence of soft tissue defects, with or without concurrent soft tissue or bone infection.

Twenty-three foot deformities in 22 patients had been treated without any real surgical incision, 'the bloodless technique', after the application of the Ilizarov external fixator. The ages of the patients ranged from 2 years and 6 months to 49 years. After an average follow-up of 18.3 months (range 8-40 months), all patients had a plantigrade foot. There was no need for soft tissue release or osteotomy. Pin tract infection occurred mostly in the metatarsal wire. However it did not affect final results. The bloodless technique is an effective way of treating foot deformities in children and ad

Background and Objectives: In degenerative spine disease, compensatory mechanisms are activated to maintain upright posture, extending beyond the spine to involve the pelvis, lower limbs, and feet. These adaptations may be accompanied by differences in foot alignment, which could be associated with sagittal balance. The aim of this study is to investigate the relationship between foot alignment and spinal posture in patients with degenerative spine disease and evaluate whether foot deformities are associated with sagittal imbalance in degenerative spine disease. Materials and Methods: We retro

OBJECTIVE: To investigate the safety and effectiveness of using the Taylor spatial frame (TSF) based on the Ilizarov tension-stress principle for treatment of post-burn foot and ankle deformities in adults.
METHODS: A clinical data of 6 patients with post-burn foot and ankle deformities treated between April 2019 and November 2023 was retrospectively analyzed. There was 1 male and 5 females with an average age of 28.7 years (range, 20-49 years). There were 3 cases of simple ankle equinus, 2 cases of ankle equinus, midfoot rocker-bottom foot, and forefoot pronation, and 1 case of calcaneus foo

OBJECTIVE: To review the research progress on correction of severe foot and ankle deformities with digital hexapod external fixators.
METHODS: The relevant research literature on digital hexapod external fixators at home and abroad in recent years was reviewed and analyzed. Taking Taylor spatial frame (TSF) as a representative, this article elaborates on the research progress of this technology in the treatment of severe foot and ankle deformities from aspects such as device principle, technical characteristics, clinical application, complication management, and controversial perspectives, ai

Arthrodesis of the hindfoot is typically used for the correction of severe and arthritic progressive collapsing foot deformity. Concomitant bony or soft tissue procedures may be helpful in patients with congenital abnormalities including the ball-and-socket ankle or congenital vertical talus. Dysplasia of the hindfoot bones may be more common than previously recognized, and corrective procedures or alterations in technique may need to be performed during hindfoot arthrodesis to account for bony deformity.
