Nonsexually Transmitted Genital Ulcer Secondary to Hand, Foot, and Mouth Disease.

BACKGROUND: Genital ulcers that are not the result of sexually transmitted infections (STIs) are uncommon, and case reports predominantly feature pubertal girls. CASE: A 38-year-old monogamous, nulliparous woman acutely developed painful vaginal ulcers 7 days after babysitting a child who was subsequently diagnosed with hand, foot, and mouth disease. The patient developed nonspecific systemic symptoms followed by painful and rapidly expanding genital ulcers. Laboratory test results for herpes, syphilis, human immunodeficiency virus, gonorrhea, chlamydia, and Epstein-Barr virus were all negati

Squamous Cell Carcinoma Arising in a Chronic, Nonhealing Diabetic Foot Ulcer.

UNLABELLED: Diabetic foot ulcers (DFUs) are a severe and costly complication of diabetes and may result in foot amputation. CASE REPORT: A 69-year-old man with a 10-year history of type 2 diabetes, who was undergoing routine care for diabetes in the authors' clinic, developed a DFU of his right foot of 4 years' duration. The wound did not respond to conventional treatments, and imaging studies were normal. His 2 biopsies tested negative for malignancy. The DFU was fully removed surgically. Following surgical removal, squamous cell carcinoma (SCC) was diagnosed in the histologic study of the w

Lower-extremity amputation risk after charcot arthropathy and diabetic foot ulcer.

OBJECTIVE: To compare risks of lower-extremity amputation between patients with Charcot arthropathy and those with diabetic foot ulcers. RESEARCH DESIGN AND METHODS: A retrospective cohort of patients with incident Charcot arthropathy or diabetic foot ulcers in 2003 was followed for 5 years for any major and minor amputations in the lower extremities. RESULTS: After a mean follow-up of 37 +/- 20 and 43 +/- 18 months, the Charcot and ulcer groups had 4.1 and 4.7 amputations per 100 person-years, respectively. Among patients <65 years old at the end of follow-up, amputation risk relative to pa

Neuropathic foot ulcer prevention in diabetic American Indians with hallux limitus.

Infected neuropathic ulcerations are the leading cause of diabetes-related partial foot amputations at the Phoenix Indian Medical Center. Ten hallucal ulcerations in seven American Indian patients with hallux limitus were resolved by local wound care and partial first metatarsophalangeal joint resection. The average length of postsurgical follow-up care was 28.8 months. There have been no recurrences of the plantar hallux ulcerations in any of the patients.

[A case of non-familial ulcer mutilating acropathy of the foot].

A case of non-familial pseudo-syringomyelic acropathy is presented; in view of its rarity, numerous hypotheses had to be considered with regard both to diagnosis and to pathogenesis. It was first taken to be an undoubted case of osteomyelitis, but the rapid advance of the osteolytic form, which resisted all treatment both local and general, whether antibiotic, surgical or orthopaedic, led to consideration of the possibility of sarcoma or some other benign tumoral form. The conclusion was reached that doubtful diagnosis can be settled only by bioptic examination, whereas aetiopathogenetic doubt

Australian Diabetes Foot Network: management of diabetes-related foot ulceration - a clinical update.

Appropriate assessment and management of diabetes-related foot ulcers (DRFUs) is essential to reduce amputation risk. Management requires debridement, wound dressing, pressure off-loading, good glycaemic control and potentially antibiotic therapy and vascular intervention. As a minimum, all DRFUs should be managed by a doctor and a podiatrist and/or wound care nurse. Health professionals unable to provide appropriate care for people with DRFUs should promptly refer individuals to professionals with the requisite knowledge and skills. Indicators for immediate referral to an emergency department

SFDI biomarkers provide a quantitative ulcer risk metric and can be used to predict diabetic foot ulcer onset.

AIMS: Annually, up to 4% of people with diabetes present with a chronic foot ulcer. Quantitative real-time testing to identify patients at risk for ulceration can guide preventative care. Here, we assess whether a non-invasive optical imaging technique, Spatial Frequency Domain Imaging (SFDI), can identify patients at the highest risk for ulceration and predict ulcer onset. METHODS: We imaged 252 subjects with diabetes at Kaiser Permanente, Southern California. SFDI derived tissue biomarkers of microcirculation were compared between subjects with and without a history of ulceration, and subje

Is a Left-to-Right >2.2°C Difference a Valid Measurement to Predict Diabetic Foot Ulceration in People with Diabetes and a History of Diabetic Foot Ulceration?

Monitoring foot skin temperatures at home have been shown to be effective at preventing the occurrence of diabetic foot ulcers. In this study, the construct validity of using >2.2°C difference between contralateral areas on the foot as a warning sign of imminent ulceration is explored. Thirty participants with diabetes at high risk of ulceration (loss of protective sensation and previous ulceration and/or amputation) monitored their foot temperatures at six sites, four times a day for six days using a handheld infrared thermometer. Walking activity, time of day, and environmental temperature w

Comparison of demographic and clinical characteristics influencing health-related quality of life in patients with diabetic foot ulcers and those without foot ulcers.

BACKGROUND: A number of studies have demonstrated that health-related quality of life (HRQoL) is negatively affected by diabetic foot ulcers. The aim of this study was to compare HRQoL in diabetic patients with and without foot ulcers and to determine demographic and clinical factors influencing HRQoL. METHODS: There were no variables affecting HRQoL except for gender in diabetic patients without foot ulcers. Demographic and clinical variables were recorded and HRQoL was evaluated using the Short Form 36 (SF-36) survey for all participants. The summary physical component score (PCS) and menta

Deep foot infections in patients with diabetes and foot ulcer: an entity with different characteristics, treatments, and prognosis.

We report findings in 223 consecutively included people with diabetes, foot ulcer and a deep foot infection treated by a multidisciplinary diabetic foot care team at the University Hospital in Lund, Sweden. The aim of the present study was to evaluate type and characteristics of deep foot infections and their relation to choice of treatment and outcome. Three different groups of deep foot infections were identified; osteomyelitis only (n = 112), deep soft tissue infection only (n = 46) and combined infections (osteomyelitis and deep soft tissue infection, n = 65). The various types of deep foo

Platelet to lymphocyte ratio correlates with diabetic foot risk and foot ulcer in patients with type 2 diabetes.

Diabetic foot ulcer is a major complication in patients with diabetes. Platelet-lymphocyte ratio (PLR) has been reported to have a predictive effect to some diabetic complications in recent years. However, it has not been fully elucidated about the relationship between diabetic foot risk or diabetic foot ulcer and PLR in patients with type 2 diabetes. Therefore, we aimed to evaluate this relationship. In this cross-sectional study, we evaluated the relationships between patient's diabetic foot risk with the criteria of the International Working Group on the Diabetic Foot (IWGDF) and prevalent

Recent Advances in Nanozymes Toward Diabetic Foot Ulcers.

Diabetic foot ulcers (DFUs) are one of the most serious and intractable complications of diabetes, caused by oxidative stress due to hyperglycemia, chronic inflammation, and repeated infections. Conventional treatments such as insulin therapy do not improve the pathological wound microenvironment, resulting in slow healing and a high rate of recurrence. Nanozymes, with superior catalytic stability, tuneable enzyme-like activities, and multifunctional synergy, have emerged as a promising therapeutic strategy. Nanozymes that imitate natural enzymes such as glucose oxidase, peroxidase, catalase,

Folinic Acid Improves Healing of Diabetic Foot Ulcers.

A novel folinic acid (FA) wound treatment (FAWT) significantly (p < 0.05) improved healing (re-epithelialization) of chronic diabetic foot ulcers (DFUs). In a double-blind RCT, 10 (n = 10) chronic DFU subjects received daily topical treatment in either the Control group [(n = 5); PluroGel] or the FAWT group [(n = 5); PluroGel with FA 2.5%]. After 12 weeks, FAWT subjects experienced significantly (p < 0.05) greater %-DFU area reduction (88% [SD: 16]) vs. Control (40% [SD: 39]), respectively. Proteomic analysis of keratinocytes (KCs) pre- and post-FAWT, using Reverse Phase Proteomic Array (RPPA)

Galectins: Role and Therapeutics in Diabetes and Diabetic Foot Ulcers.

Diabetes is a chronic inflammatory disease due to decreased insulin release or insulin resistance. Diabetes complications stem from high blood sugar damaging blood vessels and nerves, leading to issues like heart disease, stroke, kidney failure, nerve damage, vision loss, foot ulcers, gum disease, skin infections, and digestive/bladder issues. Galectins, especially galectin-3, are emerging as key players in diabetes complications, promoting fibrosis, inflammation, and vascular damage. This suggests that galectins may be potential therapeutic targets in diabetes and its complications, and a nee
