Protocol for an evaluation of the initiation of an integrated longitudinal outpatient care model for severe chronic non-communicable diseases (PEN-Plus) at secondary care facilities (district hospitals) in 10 lower-income countries.

INTRODUCTION: The Package of Essential Noncommunicable Disease Interventions-Plus (PEN-Plus) is a strategy decentralising care for severe non-communicable diseases (NCDs) including type 1 diabetes, rheumatic heart disease and sickle cell disease, to increase access to care. In the PEN-Plus model, mid-level clinicians in intermediary facilities in low and lower middle income countries are trained to provide integrated care for conditions where services traditionally were only available at tertiary referral facilities. For the upcoming phase of activities, 18 first-level hospitals in 9 countries

Use of Moulage to Evaluate Student Assessment of Skin in an Objective Structured Clinical Examination.

PURPOSE: This study investigated the physician assistant (PA) student evaluation of skin and the use of moulage in an objective structured clinical examination (OSCE). METHODS: Clinical-year PA students participated in an OSCE, taking a history, completing a physical examination, and writing a focused clinical note. The standardized patient's history simulated a primary diagnosis of right-sided carpal tunnel syndrome and an incidental skin lesion consistent with melanoma, only if the student made an inquiry in this replication study. Faculty members' assessments of students' physical examinat

Mainstreaming nutrition into maternal and child health programmes: scaling up of exclusive breastfeeding.

Interventions to promote exclusive breastfeeding have been estimated to have the potential to prevent 13% of all under-5 deaths in developing countries and are the single most important preventive intervention against child mortality. According to World Health Organization and United Nations Children Funds (UNICEF), only 39% infants are exclusively breastfed for less than 4 months. This review examines programme efforts to scale up exclusive breastfeeding in different countries and draws lesson for successful scale-up. Opportunities and challenges in scaling up of exclusive breastfeeding into

AI feedback-seeking and learning engagement in art education: psychological antecedents and reflective mechanisms.

INTRODUCTION: Generative artificial intelligence provides students with a feedback source that differs from traditional teacher and peer interaction, yet limited research has explained why students actively seek feedback from AI or how such feedback-seeking is associated with subsequent learning engagement. Drawing on transactional stress and coping theory and social cognitive theory, this study examined the psychological antecedents and reflective mechanisms of AI feedback-seeking in art education. METHODS: Two complementary studies were conducted. Study 1 utilized survey data from 315 highe

Strategies and synergies underlying replacement of vestibular function with prosthetic feedback.

This study investigated changes in movement strategies and muscle synergies when bilateral peripheral vestibular loss (BVL) subjects are provided prosthetic feedback of their pelvis sway during stance. Six BVL subjects performed 3, for them, difficult stance tasks: standing eyes closed, on a firm surface, on a foam surface, and standing eyes open on foam. Movement strategies were recorded as roll and pitch ratios of upper and lower body velocities with body-worn gyroscopes. Surface EMG recordings were taken from two pairs of antagonistic, lower leg and trunk muscles in order to note synergy ch

Mechanisms accounting for Allee effects during HIV-1 infection.

The infection of target cells by viruses involves several positive feedback loops. Many viruses increase the likelihood of infecting a target cell by increasing the local multiplicity of infection (MOI), and viruses like HIV-1 increase the availability of target cells by activating quiescent CD4+ cells. Positive feedbacks during viral infection can lead to bistability phenomena, meaning that a stable uninfected steady state co-exists with a stable infected steady state. This could have the consequence that too small viral inocula are unable to infect a host, which is a so-called Allee effect,

How to modify expectations of social rejection? An experimental study using a false-feedback paradigm.

BACKGROUND AND OBJECTIVES: Negative expectations (NEs) are fundamental to various mental disorders. Finding ways to modulate NEs would help to improve clinical treatment. The present study investigated how previously formed expectations of social rejection are revised in the context of novel positive social experiences, and whether their revision can be modulated by differentially shifting participants' attentional focus. METHODS: Our sample of 124 healthy participants was randomly assigned to four experimental conditions and received manipulated social feedback in multiple alleged webcam con

Dopamine alters motor learning performance in the presence and absence of feedback.

We often choose to learn motor skills not only because of some external reward, but also because motor learning, in and of itself, is satisfying. While dopamine is thought to drive reward-based motor learning, it remains unclear whether dopamine is implicated in motor learning under conditions ostensibly driven by intrinsic rewards/motivation (i.e., in the absence of extrinsic feedback or reward). Here, by pharmacologically manipulating dopamine using the dopamine precursor Levodopa, we investigated the role of dopamine in an explicit motor learning task guided by internally determined signals

The effect of feedback and recollection rejection instructions on the development of memory monitoring and accuracy.

Recollection rejection (a form of memory monitoring) involves rejecting false details on the basis of remembering true details (recall to reject), thereby increasing memory accuracy. This study examined how recollection rejection instructions and feedback affect memory accuracy and false recognition in 5-year-olds, 6- and 7-year-olds, 8- and 9-year-olds, and adults. Participants (N = 336) completed three study-test phases. Instructions and item-level feedback were manipulated during the first two phases, with the third phase including a test containing no instructions or feedback to evaluate l

Exploring the Impact of Machine-Learned Predictions on Feedback from an Artificial Limb.

Learning to get by without an arm or hand can be very challenging, and existing prostheses do not yet fill the needs of individuals with amputations. One promising solution is to improve the feedback from the device to the user. Towards this end, we present a simple machine learning interface to supplement the control of a robotic limb with feedback to the user about what the limb will be experiencing in the near future. A real-time prediction learner was implemented to predict impact-related electrical load experienced by a robot limb; the learning system's predictions were then communicated

The two faces of Janus: Processes can be both exogenous forcings and endogenous feedbacks with wind as a case study.

Janus is the Roman god of transitions. In many environments, state transitions are an important part of our understanding of ecological change. These transitions are controlled by the interactions between exogenous forcing factors and stabilizing endogenous feedbacks. Forcing factors and feedbacks are typically considered to consist of different processes. We argue that during extreme events, a process that usually forms part of a stabilizing feedback can behave as a forcing factor. And thus, like Janus, a single process can have two faces. The case explored here pertains to state change in dr

Assessment burden by design: exploring the variability in competence by design assessment forms.

BACKGROUND: The Royal College of Physicians and Surgeons of Canada's (RCPSC) Competence by Design (CBD) framework has been criticized for increasing assessment burden due to the high number of required Entrustable Professional Activity (EPA) assessments. Another contributing factor may be the inefficient design of assessment forms. We explored variability in form design to identify differences that could impact learners' and assessors' experience with CBD. METHODS: Annotated assessment forms for the 'Core 1' (C1) EPA were collected in March 2023 from all (n = 14, 100%) RCPSC emergency medicin

Utility and User Experience with 3-Dimensional Vibrotactile Kinesthetic Feedback for Improving Reach-to-Grasp Accuracy and Efficiency After Stroke: A Case Series.

Recent studies have shown that supplemental sensory feedback systems have potential to mitigate functional impairment after neuromotor injury through mechanisms of sensory augmentation or replacement. In this proof-of-concept case series, we evaluated multi-session 3-dimensional kinesthetic vibrotactile feedback training as a means to enhance the accuracy and efficiency of goal-directed reaching in the absence of visual feedback in survivors of stroke. A motion capture system converted real-time position of the contralesional hand within a Cartesian frame of reference into spatiotemporal patte

Test-Retest Reliability of Large Language Models and Their Concordance With Human Experts in Rubric-Based Scoring of Clinical Summary Statements: Experimental Comparative Study.

BACKGROUND: Clinical summary statements convert unstructured patient information into a concise format. They are widely used in virtual patients (VPs) to train medical students in diagnostic reasoning. Most VP systems provide little individualized feedback on the quality of learners' summaries, and earlier automated approaches have required complex natural language processing, limiting their scalability. Large language models (LLMs) may offer a more flexible, rubric-guided way to score summary statements and support feedback. Still, their test-retest reliability and concordance with expert rat

Neural correlates of altered feedback learning in women recovered from anorexia nervosa.

Anorexia nervosa (AN) is associated with exaggerated self-control and altered reward-based decision making, but the underlying neural mechanisms are poorly understood. Consistent with the notion of excessive cognitive control, we recently found increased dorsal anterior cingulate cortex (dACC) activation in acutely ill patients (acAN) on lose-shift trials in a probabilistic reversal learning (PRL) task. However, undernutrition may modulate brain function. In attempt to disentangle trait from state factors, the current fMRI study investigated cognitive control in recovered patients (recAN). Thi

Inside the Black Box of Audit and Feedback: a Laboratory Study to Explore Determinants of Improvement Target Selection by Healthcare Professionals in Cardiac Rehabilitation.

Audit and feedback (A&F) is widely used to aid healthcare professionals in improving clinical performance, but there is little understanding of the underlying mechanism that determines its effectiveness. The aim of this paper is to investigate the process by which healthcare professionals select indicators as improvement targets based on A&F. We performed a laboratory study among 41 healthcare professionals in the context of a web-based A&F intervention designed to improve the quality of cardiac rehabilitation care in the Netherlands. Feedback was provided on eighteen quality indic

Solving complex perceptual discrimination problems: techniques for the development of problem-solving strategies.

Techniques for developing problem-solving strategies in handicapped children when they are faced with complex perceptual discriminations were investigated. Specifically, the effectiveness of feedback, modeling, and self-instruction were compared with each other and a control condition on a specially designed matching-to-sample task. The task was designed so that the distinctive features of the stimuli could be identified for instructional purposes. The results indicated that only the self-instruction technique facilitated performance on the posttest. In addition, these skills generalized to a

RESECT: A Randomised Controlled Trial of Audit and Feedback in Non-muscle-invasive Bladder Cancer Surgery.

BACKGROUND AND OBJECTIVE: We aimed to determine whether audit, feedback, and education improves surgical performance after transurethral resection of bladder tumour surgery for non-muscle-invasive bladder cancer and as a secondary aim if it reduced recurrence rates. METHODS: This cluster randomised controlled trial compared audit and feedback plus peer comparison and education, with audit alone for four coprimary outcomes: (1) Single-instillation chemotherapy, (2) detrusor muscle sampling, (3) documentation of tumour features, and (4) resection completeness. Experience-dependent plasticity of conceptual representations in human sensory-motor areas.

Concepts are composed of features related to different sensory and motor modalities such as vision, sound, and action. It is a matter of controversy whether conceptual features are represented in sensory-motor areas reflecting the specific learning experience during acquisition. In order to address this issue, we assessed the plasticity of conceptual representations by training human participants with novel objects under different training conditions. These objects were assigned to categories such that for one class of categories, the overall shape was diagnostic for category membership, where
