The healing of tibial fracture and response of the local lymphatic system.

BACKGROUND: Damage of tissues by mechanical injury and inflammation is followed by reaction of the regional lymphoid tissue, lymphatics, and lymph nodes. In our previous lymphoscintigraphic studies, we showed that closed fractures of a lower limb cause reaction of the local lymphoid tissue. There was dilation of lymphatics draining the site of the fracture and enlargement of inguinal lymph nodes. These changes persisted even after clinical healing of the fracture. In the long-lasting nonhealing fractures, the lymphoscintigraphic pictures were different. The draining lymphatics became obliterat

Assessment of the lateral ligaments of the ankle after healed fractures.

BACKGROUND: Lateral ligament complex injuries are difficult to diagnose immediately after ankle fracture, and treatment is focused on the fracture. This study examines the prevalence of ligamentous injuries after severe ankle fractures. METHODS: Lateral ligament instability can be revealed by inversion and anterior stress views after fracture healing. The results of 54 inversion and anterior stress examinations of the ankle after fracture healing, using a Telos stress device at 15 kPa force, were compared with uninjured ankles. RESULTS: No patient had 5 degrees or more of talar tilt or 6 mm

Fractures of mandible: is spontaneous healing possible? Why? When?

Closed and open reduction of fractures of the mandible constitutes an endless debate, and it is interesting that modern times have not brought a conclusion to this controversy. From 1995 to 1997 in the Plastic Surgery Department of Argerich Hospital in Buenos Aires, Argentina, 23 patients were evaluated with different fractures of the mandible in which neither closed nor open reduction was performed. Seventeen patients (74%) presented associated fractures in different areas of the mandible and 6 (26%) only had one fracture; 2 of them (9%) were edentulous. Three (13%) of the 23 patients had mor

Fracture fixation, then and now: When implants learn to heal.

BACKGROUND: Conventional fracture fixation has long provided reliable mechanical stability, yet it remains largely passive in the biological process of healing. The persistence of nonunion, delayed union, and implant-related complications reveals the limitations of a fixation-centric paradigm. Emerging advances in materials science, biology, digital technologies, and implant design have progressively challenged this model, opening the door to a broader reframing of fracture management. PURPOSE: To propose a conceptual and strategic reframing of fracture management-shifting emphasis from mecha

Effect of Systemic Oxytocin Administration on the Healing of Mandibular Fractures.

BACKGROUND: It has been determined that oxytocin (OT), a hormone primarily involved in childbirth and lactation, has anabolic effects on bone metabolism. PURPOSE: The purpose of the study was to measure and compare radiographic and clinical fracture healing between the OT-treated and control groups. STUDY DESIGN, SETTING, AND SAMPLE: This is an in vivo animal study using a mandibular fracture model in 42, 6-month-old male Wistar rats. The fracture line was created and fixed using mini plates and screws. PREDICTOR VARIABLES: The predictor variable was OT exposure. The animals were randomly a

The Effects of Phenyramidol and Diclofenac Treatment on Fracture Healing in Rats.

BACKGROUND: Fracture healing or nonunion refers to a process in which many factors interact. In this study, we aimed to evaluate the radiological, histological, and biomechanical effects of phenyramidol and diclofenac, which are frequently used to treat post-fracture ture pain worldwide, on fracture healing and nonunion in a rat femur fracture model. METHODS: In this study, 72 male Wistar-Albino rats aged 2-3 months and weighing 250 ± 30 g were divided into 4 main groups. The rats were divided into 12 subgroups according to the early, middle, and late periods. A fracture model was created in

Radiographic timelines for pediatric healing fractures: a systematic review.

Skeletal fractures, a common injury in physically abused children, often go undetected and untreated for significant lengths of time and are sometimes incidentally discovered radiographically. Our objective was to review current literature for scientific studies of pediatric fracture healing with associated timelines. We conducted a search of Embase, EBSCOhost, MEDLINE (PubMed), and Web of Science for literature published from the earliest available up to August 2018. We evaluated the included articles for quality, with consideration for use in clinical and forensic settings. Of a total of 313

Complications of implant removal after healed hip fractures.

INTRODUCTION: Osteosynthesis of pertrochanteric fractures is one of the most frequently performed procedures in orthopaedic trauma care. Auto-dynamization during fracture healing can lead to lateralization of the screw or blade. This can cause debilitating complaints, which sometimes necessitate the removal of the screw or blade. After removal of the implant, we observed spontaneous femoral neck fractures in five patients. This article presents an evaluation of these cases with reference to literature. MATERIALS AND METHODS: Five cases of spontaneous femoral neck fracture (SFNF) after cephalo

Delayed healing of lower limb fractures with bisphosphonate therapy.

INTRODUCTION: Bisphosphonate therapy (BT) is used commonly in the management of osteoporosis. A systematic review was conducted investigating delayed union of lower limb, long bone fractures in patients on BT. We specifically assessed whether BT increases the risk of delayed union or non-union in lower limb, long bone fractures. METHODS: A literature search was conducted in the PubMed and Embase™ on 4 November 2014. Articles that investigated lower limb fractures, history of BT and fracture union were included in the review. RESULTS: A total of 9,809 papers were retrieved and 14 were deemed

The effect of smoking on fracture healing and on various orthopaedic procedures.

Ten percent of all fractures lead to problems with healing. Smoking is said to be a cause. There are 13.5 million smokers in the U.K. Healing of tibial fractures, for instance, requires two more months in smokers. Nicotine, carbon monoxide and hydrogen cyanide are most often seen as the offenders, among the 4000 chemicals found in cigarettes. Many studies plead for the negative effect of smoking in general, yet there is uncertainty as to the precise role of nicotine. The authors recommend that patients should attempt smoking cessation therapy before elective orthopaedic treatment.

Prognostic significance of BsALP in healing of long bone fractures.

The aim of this work was to assess the relationship of both total alkaline phosphatase (ALP) and bone-specific alkaline phosphatase (BsALP) with the course and outcome of operatively treated long-bone fractures. The activity of total ALP and BsALP was measured in 41 patients with a long bone fracture, comprising 26 men and 15 women. All patients were treated operatively. Total ALP and BsALP levels were measured in sera on day 1, 7, 14 and 21 after sustaining injury. Patient monitoring included X-rays. According to the outcome, patients were divided into two groups: the fast healing group and t

Spontaneously healed root fractures: two case reports.

We report on two undiagnosed horizontal root fractures of two maxillary right central incisors in two different patients caused by dental trauma which occurred several years ago. These fractures were discovered during a routine full-mouth radiographic examination. While, in case one, the fracture was located in the middle-third of the root, it was in the cervical-third of the root in case two. The fractured teeth were observed to have healed spontaneously without any sign or symptom, and tested as vital to electric pulp test.

Molecular aspects of healing in stabilized and non-stabilized fractures.

Bone formation is a continuous process that is initiated during fetal development and persists in adults in the form of bone regeneration and remodeling. These latter two aspects of bone formation are clearly influenced by the mechanical environment. In this study we tested the hypothesis that alterations in the mechanical environment regulate the program of mesenchymal cell differentiation, and thus the formation of a cartilage or bony callus, at the site of injury. As a first step in testing this hypothesis we produced stabilized and non-stabilized tibial fractures in a mouse model, then use

[A biomechanical evaluation of the effects of XW630 on the healing of experimental fracture].

The aim of this study was to evaluate whether tetracycline-estrone (XW630) plays a role in the healing of fracture. 120 rabbits were divided into 4 groups and subjected to osteotomies at the middle of both radials, and they were given XW630, estrone (E2), tetracycline (T) and 0.9% sodium chloride (C) respectively after operation by means of intramuscular injection (M) or local injection(L) in the fracture region. The biomechanical properties, including bending and compression, and the biochemical changes were tested at 30 days, 60 days and 90 days after osteotomy. The results showed that the b

The healed Segond fracture: a characteristic residual bone excrescence.

This report describes the natural history of the Segond fracture and documents the radiographic appearance of the healed Segond fracture. The clinical and radiographic records of 129 patients with acute anterior cruciate ligament (ACL) injuries were reviewed. Four (3.1%) of these patients had Segond fractures. On follow-up radiographic examination, seven patients demonstrated a characteristic bone excrescence arising 3-6 mm inferior to the lateral tibial plateau. In four of the five patients for whom acute injury films were available this excrescence arose at the site of the earlier Segond fra

Influence of Age on Fracture Healing in Young and Middle-Aged Mice in a Proximal Femur Fracture Model.

Osteoporotic hip fractures are a considerable cause of pain and disability particularly among the elderly. Osteoporosis causes loss of bone stability, which in turn leads to an increased risk of fractures especially in metaphyseal bone. Moreover, the body's capacity for healing is diminished, resulting in prolonged recovery times following these fractures. The underlying mechanisms are not fully elucidated yet. Therefore, the aim of the current study was to determine the influence of age on a murine proximal femur fracture model in young and middle-aged mice and whether the model could be suit

Do peripheral melatonin agonists improve bone fracture healing? The effects of agomelatine and ramelteon on experimental bone fracture.

Melatonin improves fracture healing, but the long-term use of melatonin seems impracticable in the treatment of fracture due to side effects caused by hormonal stress on chronological rhythm. Ramelteon (RAMEL) and agomelatine (AGO) are non-selective peripheral melatonin receptor (MT) agonists. This study investigated the effects on bone fracture healing of these MT agonists, which do not affect the central nervous system. The rats were divided into 6 groups, including Group 1 (SHAM): sham operated group; Group 2 (FRACTURE): femoral fracture control; Group 3 (FR + AGO30): femoral fracture + ago
