Outcomes of Local versus Free Flaps for Lower Extremity Trauma.

BACKGROUND: Lower extremity reconstruction often requires soft tissue transfer for limb salvage. Flaps are allocated based on injury size, location, and shape coupled with surgeon expertise. Ideally, vascularized tissue should have similar outcomes across local and free tissue transfers. By evaluating outcomes from a Level 1 trauma center, we aim to provide recommendations regarding surgical management of leg reconstruction with respect to local versus free flap implementation. METHODS: This retrospective review evaluated patient medical history, demographics, flap characteristics, and outcom

Reconstruction of complex hemipelvectomy defects: A 17-year single-institutional experience with lower extremity free and pedicled fillet flaps.

INTRODUCTION: Hemipelvectomy procedures result in massive soft tissue defects. The standard approach is to reconstruct the defect with anterior or posterior hemipelvectomy flaps. Certain situations preclude the use of local tissue flaps, and an alternative is the use of leg fillet flaps, circumferential pedicled or free flaps harvested from the amputated part. The purpose of this study is to present our institution's experience with using pedicled and free fillet flaps to reconstruct hemipelvectomy soft tissue defects. METHODS: We performed a retrospective chart review of patients who underwe

Current management of glaucoma and the need for complete therapy.

OBJECTIVE: The placement of osteosynthetic materials in the leg may be complicated by hardware exposure. Successful soft tissue reconstruction often provides a critical means for limb salvage in patients with hardware exposure in the leg. Free flaps are currently considered the standard surgical procedure for soft tissue coverage of the wounds with internal hardware exposure. However, to date, no conclusive literature shows the superiority of a specific type of flap. MATERIALS AND METHODS: The current review compares data from the literature concerning outcomes and complications of free and p

Operation Iraqi freedom: A report on a series of soldiers treated with free tissue transfer by a plastic surgery service.

Free flaps in combat wounds are predisposed to failure. Few reports are available on their use in American military combat wounds. We present our experience with free flaps during Operation Iraqi Freedom. This is a retrospective review of soldiers treated by plastic surgeons at Brooke Army Medical Center. Eight free flaps were for soft tissue coverage in which local tissue was not available. Causes of the wounds: 2 from a rocket-propelled grenade, 4 from explosive devices, 1 from a fall, and 1 from a helicopter crash. Indications for the flaps were 2 exposed calvaria, 3 lower-extremity fractur

Lateral Arm Free Flap With Preservation of the Posterior Antebrachial Cutaneous Nerve.

The lateral arm free flap offers many advantages in reconstruction of soft tissue defect and reconstruction of extremities. However, this free flap is associated with sensory loss at the posterior forearm due to injury of the posterior antebrachial cutaneous nerve (PABCN).The PABCN-sparing lateral arm free flaps were performed in 19 patients with various soft tissue defects of the extremity, and the outcomes of free flap reconstructions using this modification are evaluated. All flaps survived without partial necrosis. Three patients experienced transient sensory loss in the posterior area of

Flap decisions and options in soft tissue coverage of the upper limb.

Soft tissue deficiency in the upper limb is a common presentation following trauma, burns infection and tumour removal. Soft tissue coverage of the upper limb is a challenging problem for reconstructive surgeons to manage. The ultimate choice of soft tissue coverage will depend on the size and site of the wound, complexity of the injury, status of surrounding tissue, exposure of the vital structures and health status of the patient. There are several local cutaneous flaps that provide adequate soft tissue coverage for small sized defects of the hand, forearm and arm. When these flaps are limit

Lip and perioral reconstruction.

For defects up to approximately 80% of either upper or lower lip, reconstructions that use remaining lip and cheek can function and look reasonably well. Free tissue transfers, such as the free radial forearm flap, are useful for larger defects as they import additional tissue in one step and reduce microstomia, which is more likely to result from local tissue repairs. At best, free flaps provide a static dam or curtain that functions as a lip; at worst, they deliver a large amount of composite tissue to allow for primary healing. Satisfactory outcomes after free flap reconstructions for lip a

Free versus Pedicled Perforator Flaps for Lower Extremity Reconstruction: A Multicenter Comparison of Institutional Practices and Outcomes.

BACKGROUND: Of all body regions, lower extremity wounds have been and remain the greatest challenge. Perforator free flaps have been accepted as a reasonable option to solve this dilemma but require the complexity of microsurgery. As a consequence, the possibility that pedicled perforator flaps could supplant even perforator free flaps has recently gained intense enthusiasm. METHODS: A retrospective investigation was undertaken to compare the validity for the use of perforator flaps of all types at three dissimilar institutions, that is, a university, a regional center, and a community hospit

The different concepts of surgical managements between anterior and lateral skull base reconstructions based on surgical purposes.

OBJECTIVE: Anterior and middle cranial fossa defects require different strategies, depending on their anatomical location. The aim of this study was to elucidate the surgical managements for the defects based on surgical purposes and to clarify the conceptional differences relating to anterior and lateral skull base reconstructions. METHODS: This study included patients who had undergone reconstructive surgery for anterior or middle cranial fossa defects in our institution between July 2008 and June 2019. It consisted of 66 patients who had been subject to anterior skull base reconstructions,

Thoracodorsal artery perforator flap: a versatile alternative for various soft tissue defects.

Thoracodorsal artery perforator (TDAP) flap is a relatively new member of the perforator flap family. The objective of this study is to describe the use of pedicled and free TDAP flaps for various soft tissue defects. Fifteen patients underwent soft tissue reconstruction using 16 TDAP flaps. Twelve pedicled flaps were used for axillary, breast, and shoulder regions. Four free flaps were used for cheek, popliteal, hand, and foot reconstruction. The flaps were harvested based on the perforators, which were preoperatively located at or close to a point 8 cm below the posterior axillary fold and 2

Other flaps for head and neck use: temporoparietal fascial free flap, lateral arm free flap, omental free flap.

Advances in microvascular surgery have led to the development of new donor sites for head and neck reconstruction. The superficial temporoparietal, lateral arm, and omental free flaps represent three flaps with primarily soft tissue uses. Their reliable blood supply and pliability make them well suited for a variety of head and neck defects. The superficial temporoparietal and lateral arm free flaps may be used as osteocutaneous flaps, but more rarely is this the case. Minimal donor site morbidity is an advantage with the superficial temporoparietal and lateral arm flaps. The morbidity of a la

Gracilis Free Flap in a Patient with Sickle Cell Disease.

INTRODUCTION: Free tissue transfer in the sickle cell population presents many challenges to the reconstructive surgeon. There are few reported cases of successful free tissue transfers within the sickle cell population. The majority of successful cases involve fasciocutaneous free flaps with few successful muscle flaps. This case report describes the successful utilization of a gracillis free flap to reconstruct a multifocal soft tissue defect following a closed distal tibia fracture in a patient with sickle cell disease (SCD). CASE REPORT: This is a 20-year-old female with past medical hist

Flap Impaling: A Simple and Effective Technique in Soft Tissue Reconstruction of Complex Extremity Wounds.

Composite defects due to trauma present with compound comminuted fractures with soft tissue loss. Stabilization requires multiple Schanz pins across the fracture segments and flap cover for soft tissue reconstruction. These pins hinder flap inset and may require complex planning with multiple flaps. A simple approach is to impale the flaps over these pins to achieve flap inset without disturbing the skeletal fixation. This method of impaling was utilized for free as well as pedicled fasciocutaneous and muscle flaps. A stab incision is made and careful dissection is done to create a passage for

Syndactylized glabrous flaps for multiple finger palmar defects.

BACKGROUND: Palmar finger or pulp defects require coverage with glabrous tissue to achieve a good match with the lost tissue. The management of multiple finger palmar or pulp defects is challenging because these defects may not always be suitable for local or pedicled flaps. In such situations, syndactylizing free or pedicled flaps can be used. PATIENTS AND METHODS: We evaluated the results of free glabrous flaps syndactylizing across multiple finger defects. The two flaps used were the superficial branch of the radial artery (SUPBRA) flap and hypothenar free flap. Seven syndactylized glabrou

[Fresh post-traumatic soft tissue defects of the wrist joint and hand and their closure].

As a universal human tool the hand often suffers injuries of its soft tissue. Different localization of defects (palmar and dorsal) and different injuries of functional structures (tendon, nerve, vessel, bone) also need different soft tissue cover and wound closure. The possibilities in plastic surgery of the hand include free transplantation of skin, local and distant flaps, free flaps and neurovascular flaps.
