Hormone producing pancreatic islet cell carcinoma: changing clinical presentation.

A patient with pancreatic islet cell carcinoma demonstrated spontaneous remission and recurrence of hyperinsulinism and disappearance of elevated plasma motilin levels. Despite evidence for gastrin production by the tumor initially, the Zollinger-Ellison syndrome was not diagnosed until three years after initial presentation. Diarrhea and steatorrhea could be attributed to hyperchlorhydria rather than to direct intestinal effects of elevated cirulating gastrin, gastric inhibitory peptide or motilin. Pancreatic islet cell carcinomas, considered as a type a APUD cell proliferation, frequently pr

Regulatory peptides in Barrett's oesophagus.

Barrett's epithelium refers to the presence of ectopic mucosal types in the squamous-lined oesophagus. Previous studies have documented argentaffin and argyrophil-positive cells as well as gastrin-like immunoreactivity in oesophageal tissue extracts from patients with Barrett's mucosa. In the present study, 125 oesophageal biopsies obtained under direct vision at endoscopy from 22 patients with Barrett's oesophagus were systematically studied using fluorescence and peroxidase antiperoxidase single and double-staining immunocytochemical methods employing highly specific antibodies to localize t

GLP-1 and GIP may play a role in long-term weight trajectories after gastric bypass.

INTRODUCTION: Suboptimal clinical responses to metabolic and bariatric surgery include insufficient weight loss (WL), weight regain (WR), and/or comorbidity remission failure or relapse. Gut hormones' role in WR and Type 2 diabetes (T2D) relapse is not fully established. So, our aim was to evaluate the hormone profiles of patients with long-term optimal and suboptimal response after gastric bypass (RYGB). METHODS: This cross-sectional study included 43 individuals who underwent RYGB surgery over 10 years ago, divided into two groups: 23 participants with no T2D history but different WR trajec

Effects of intraduodenal glutamine on incretin hormone and insulin release, the glycemic response to an intraduodenal glucose infusion, and antropyloroduodenal motility in health and type 2 diabetes.

OBJECTIVE: Glutamine reduces postprandial glycemia when given before oral glucose. We evaluated whether this is mediated by stimulation of insulin and/or slowing of gastric emptying. RESEARCH DESIGN AND METHODS: Ten healthy subjects were studied during intraduodenal (ID) infusion of glutamine (7.5 or 15 g) or saline over 30 min, followed by glucose (75 g over 100 min), while recording antropyloroduodenal pressures. Ten patients with type 2 diabetes mellitus (T2DM) were also studied with 15 g glutamine or saline. RESULTS: ID glutamine stimulated glucagon-like peptide 1 (GLP-1; healthy: P < 0.

Interrelation of nerves and hormones in stomach and pancreas.

Recent investigations have shown that the functions of the gastrointestinal organs may be regulated by a complex system of paracrine cells and peptidergic nerves, in addition to the established humoral and autonomic nervous systems. In this article we discuss the possible paracrine regulation of the secretory functions of the stomach and the pancreas by somatostatin-producing D-cells. Secondly, we discuss the distribution, localization, effects and secretion of the neuropeptides VIP and 'gastrin-releasing polypeptide' (GRP or 'mammalian bombesin') applied to the stomach (GRP) and pancreas (GRP

Appetite, Glycemia, and Entero-Insular Hormone Responses Differ Between Oral, Gastric-Remnant, and Duodenal Administration of a Mixed-Meal Test After Roux-en-Y Gastric Bypass.

OBJECTIVE: To examine the effect of different feeding routes on appetite and metabolic responses after Roux-en-Y gastric bypass (RYGB). RESEARCH DESIGN AND METHODS: A standard liquid meal was administered either orally, into the gastric remnant, or intraduodenally 6 months after RYGB. Changes in plasma glucose, insulin, glucagon-like peptide 1 (GLP-1), glucose-dependent insulinotropic peptide (GIP), peptide YY (PYY), and appetite were measured pre- and postprandially. RESULTS: Postprandial GLP-1 and PYY responses were similar, whereas glucose, insulin, and GIP levels differed markedly after

Polysaccharide extracted from peony seed meal preventive effect of on loperamide-induced constipation in rats.

In the present study, one polysaccharide (named PSP-1) was purified from peony (Paeonia suffruticosa Andr.) seed meal, and its structural characteristics, as well as preventive constipation activity, were investigated. The weight average molecular weight of PSP-1 was 15.3 kDa and was composed of arabinose, galactose, glucose, xylose, and mannose in the molar ratio of 1:4.8:5.1:0.4:0.2. The primary structure of PSP-1 was elucidated through Fourier-transform infrared spectroscopy (FT-IR) and methylation analysis, revealing its glycosidic linkages and branching pattern. In the loperamide-induced

Optimizing treatment of cardiovascular risk factors in cerebral small vessel disease using genetics.

Cerebral small vessel disease (cSVD) causes lacunar stroke (LS) and intracerebral haemorrhage and is the most common pathology underlying vascular dementia. However, there are few trials examining whether treatment of conventional cardiovascular risk factors reduces stroke risk in cSVD, as opposed to stroke as a whole. We used Mendelian randomization techniques to investigate which risk factors are causally related to cSVD and to evaluate whether specific drugs might be beneficial in cSVD prevention. We identified genetic proxies for blood pressure traits, lipids, glycaemic markers, anthropome

Identification of glicentin as a low-potency glucose-dependent insulinotropic polypeptide receptor agonist.

The glucose-dependent insulinotropic polypeptide receptor (GIPR) is a class B1 G protein-coupled receptor (GPCR) that promotes glucose-dependent insulin secretion upon activation by GIP. Dual agonism of GIPR and glucagon-like peptide-1 receptor (GLP-1R) has emerged as a breakthrough therapeutic strategy for type 2 diabetes and obesity, improving glycaemic control and promoting weight loss. Although GIPR is widely expressed in the central nervous system, endogenous GIP expression in the brain is controversial, and peripherally-administered fluorescent GIPR/GLP-1R agonists predominantly localise

Glucagon-like peptide-1 (GLP-1) levels are associated with acute kidney injury after cardiac surgery.

Acute kidney injury (AKI) is a frequent and clinically relevant complication after cardiac surgery. Early prediction remains challenging because serum creatinine is a delayed and insensitive marker of renal dysfunction. Glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) are incretin hormones with established metabolic functions and emerging roles in cardiovascular, renal, and critical illness–related pathophysiology, and their perioperative association with AKI remains incompletely characterised. In this prospective single-centre observational study, 107 adu

Low discontinuation rate of tirzepatide treatment in Japanese patients with diabetes mellitus; importance of traditional Japanese diet.

Tirzepatide, a novel dual GIP and GLP-1 receptor agonist, has demonstrated robust efficacy in clinical trials; however, discontinuation rates due to gastrointestinal adverse events have been reported at 6-10% in Western populations. In contrast, our real-world study of 219 Japanese patients revealed a markedly lower discontinuation rate of approximately 1.3%. Dietary questionnaires indicated that patients experienced reduced appetite for high-fat and high-calorie foods while maintaining consumption of low-carbohydrate traditional Japanese foods such as fish and meat, which may have contributed

Dynamic multi-omics profiling of islet and gut hormonal secretion and peripheral crosstalk in response to various nutrient loads.

Postprandial metabolism is a complex and dynamic process involving diverse biomolecules, with islet and gut hormones playing crucial roles. However, how these hormones interact with biomolecules after nutrient intake and coordinate with peripheral insulin resistance (IR) remains elusive. This study characterizes postprandial multi-omics dynamics under mixed meals and four distinct macronutrient loads, investigating hormone secretion patterns, associated responsive molecules, and their relationships with IR. Postprandial multi-omics data significantly elucidate insulin and glucagon secretion, h

Altered Glucagon and GLP-1 Responses to Oral Glucose in Children and Adolescents With Obesity and Insulin Resistance.

CONTEXT: Pediatric obesity is characterized by insulin resistance, yet it remains unclear whether insulin resistance contributes to abnormalities in glucagon and incretin secretion. OBJECTIVE: To examine whether fasting and stimulated glucagon, glucagon-like peptide-1 (GLP-1), and glucose-dependent insulinotropic polypeptide (GIP) concentrations differ between children and adolescents with obesity and insulin resistance (OIR), obesity and normal insulin sensitivity (OIS), and controls with normal weight (NW). METHODS: 80 (34 boys) children and adolescents, aged 7-17 years with OIR (n = 22),

The Role of Incretins on Insulin Function and Glucose Homeostasis.

The incretin effect-the amplification of insulin secretion after oral vs intravenous administration of glucose as a mean to improve glucose tolerance-was suspected even before insulin was discovered, and today we know that the effect is due to the secretion of 2 insulinotropic peptides, glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1). But how important is it? Physiological experiments have shown that, because of the incretin effect, we can ingest increasing amounts of amounts of glucose (carbohydrates) without increasing postprandial glucose excursions, w

The role of GIP and pancreatic GLP-1 in the glucoregulatory effect of DPP-4 inhibition in mice.

AIMS/HYPOTHESIS: Glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) are two peptides that function to promote insulin secretion. Dipeptidyl peptidase-4 (DPP-4) inhibitors increase the bioavailability of both GLP-1 and GIP but the dogma continues to be that it is the increase in GLP-1 that contributes to the improved glucose homeostasis. We have previously demonstrated that pancreatic rather than intestinal GLP-1 is necessary for improvements in glucose homeostasis in mice. Therefore, we hypothesise that a combination of pancreatic GLP-1 and GIP is necessary

An analysis of cosecretion and coexpression of gut hormones from male rat proximal and distal small intestine.

Gut endocrine cells are generally thought to have distinct localization and secretory products. Recent studies suggested that the cells are highly related and have potential to express more than one hormone. We studied the coexpression and cosecretion of gut hormones in separate segments of rat small intestine. We measured secretion of glucagon-like peptide-1 (GLP-1), peptide YY (PYY), neurotensin, glucose-dependent insulinotropic polypeptide (GIP), and cholecystokinin (CCK) from proximal and distal half of the small intestine, isolated from male rats and perfused ex vivo. Modified sham feeding of foods with different macronutrient compositions differentially influences cephalic change of insulin, ghrelin, and NMR-based metabolomic profiles.

Little is known about the effect of the macronutrient composition of foods on cephalic phase response of gastrointestinal hormones. In addition, the metabolomics of cephalic phase response has not been studied before. The objective of the present study was to assess cephalic phase endocrinological and metabolomic responses following modified sham feeding (MSF) of foods with different macronutrient compositions. Ten healthy males attended four separate test sessions after overnight fasting, for a 3-min MSF of water, high-fat, high-carbohydrate or high-protein food, respectively, in a randomized
