Association of a Perioperative Multicomponent Fall Prevention Intervention With Falls and Quality of Life After Elective Inpatient Surgical Procedures.

IMPORTANCE: Falls after elective inpatient surgical procedures are common and have physical, emotional, and financial consequences. Close interactions between patients and health care teams before and after surgical procedures may offer opportunities to address modifiable risk factors associated with falls. OBJECTIVE: To assess whether a multicomponent intervention that incorporates education, home medication review, and home safety assessment is associated with reductions in the incidence of falls after elective inpatient surgical procedures. DESIGN, SETTING, AND PARTICIPANTS: This prospect

The INTUIT Study: Investigating Neuroinflammation Underlying Postoperative Cognitive Dysfunction.

BACKGROUND/OBJECTIVES: Every year, up to 40% of the more than 16 million older Americans who undergo anesthesia/surgery develop postoperative cognitive dysfunction (POCD) or delirium. Each of these distinct syndromes is associated with decreased quality of life, increased mortality, and a possible increased risk of Alzheimer's disease. One pathologic process hypothesized to underlie both delirium and POCD is neuroinflammation. The INTUIT study described here will determine the extent to which postoperative increases in cerebrospinal fluid (CSF) monocyte chemoattractant protein 1 (MCP-1) levels

Geriatric comanagement reduces perioperative complications and shortens duration of hospital stay after lumbar spine surgery: a prospective single-institution experience.

OBJECTIVE Geriatric patients undergoing lumbar spine surgery have unique needs due to the physiological changes of aging. They are at risk for adverse outcomes such as delirium, infection, and iatrogenic complications, and these complications, in turn, contribute to the risk of functional decline, nursing home admission, and death. Whether preoperative and perioperative comanagement by a geriatrician reduces the incidence of in-hospital complications and length of in-hospital stay after elective lumbar spine surgery remains unknown. METHODS A unique model of comanagement for elderly patients u

Effect of adding phenylephrine infusion to goal-directed fluid therapy on postoperative quality of recovery in elderly patients undergoing laparoscopic gastrointestinal surgery: a randomized controlled trial.

BACKGROUND: Goal-directed fluid therapy (GDFT) guided by pulse pressure variation (PPV) is an established strategy to optimize perioperative fluid management in elderly surgical patients. However, its efficacy may be limited by age-related decline in vascular compliance. This study aimed to investigate whether supplementing PPV-guided GDFT with a continuous low-dose phenylephrine infusion could improve the quality of postoperative recovery in this population. METHODS: Ninety-four elderly patients scheduled for elective laparoscopic gastrointestinal surgery were randomized to either PPV-guided

[Continuous spinal anesthesia in very elderly patients with high anesthesia risk in traumatologic-orthopedic and general surgery interventions].

Continuous spinal anaesthesia (CSA) was carried out via a 28-gauge spinal catheter in 154 surgical patients at the Department of Anaesthesiology and Critical Care at Radeberg Asklepios-ASB Hospital between May 1992 and March 1999. The method was used preferably in patients aged over 70 (mean age 82.3 years) with high general risk during anaesthesia (ASA III-IV) who underwent orthopaedic or general surgery of the lower limb and hypogastrium. Remarkably, an anaesthetic level of between Th 8 and Th 10 was achieved with the low initial dose of 7.5 mg of 0.5% hyperbaric bupivacaine. Only minimal ca

Comparative analysis of six large language models in perioperative decision support for geriatric patients with multimorbidity: a three-dimensional evaluation framework.

BACKGROUND: While large language models (LLMs) show promise in healthcare, their reliability in high-stakes perioperative management for elderly patients with multimorbidity remains critically underexplored. METHODS: This benchmarking study evaluated five general-purpose LLMs (ChatGPT, Gemini, DeepSeek, Claude, Kimi) and one domain-optimized model (New Youth Anesthesia Artificial Intelligence Assistant, NYAAI) using a novel three-dimensional framework assessing guideline compliance, clinical applicability, and safety redundancy. A simulated case of an 84-year-old male with femoral fracture and

Ephedrine for Reducing Onset Time of Rocuronium in Elderly Patients: A Randomised Study.

INTRODUCTION: Onset time of rocuronium is prolonged in elderly patients compared to younger patients. Ephedrine reduces onset time of rocuronium in younger adults, but in elderly patients, the effect of ephedrine on onset time of rocuronium has not been investigated. The aim of this study was to determine the onset time of rocuronium 0.6 mg kg-1 after administration of ephedrine 0.15 mg kg-1 in patients aged 80 years or above. The hypothesis was that ephedrine reduces onset time compared to placebo (saline). METHODS: We included patients aged ≥ 80 years with American Society of Anesthesiologi

Postoperative confusion in older adults.

The development of postoperative confusion in older patients is increasingly being recognized as clinically significant because it is becoming more common as the population ages. Postoperative delirium and postoperative cognitive dysfunction differ in time course of development. Risk factors other than age include certain medications as well as anesthesia (both the type and amount used). Postoperative delirium and postoperative cognitive dysfunction appear to increase a patient's risk for developing dementia. Routine preoperative screening may help to identify patients with preexisting cogniti

[Expert consensus on the clinical application of continuous glucose monitoring in adult patients during the perioperative period (2026 edition)].

Perioperative glycemic abnormalities, including hyperglycemia, hypoglycemia, and glycemic variability, are important determinants of postoperative outcomes. Conventional glucose monitoring has inherent temporal limitations and fails to capture dynamic glycemic fluctuations in a timely manner. Continuous glucose monitoring (CGM) enables real-time, continuous assessment of glucose levels and trends, thereby facilitating more precise glycemic management. To standardize its clinical application, the Chinese Society of Anesthesiology, in collaboration with the National Alliance of Geriatric Anesthe

Simultaneous bilateral femoral neck fracture and end-stage renal disease in a 76-year-old woman: a case report.

Hip fracture is a common occurrence in the elderly. Due to the growing demand for the specific care of these patients, we established the Orthogeriatric Unit (OGU) at San Gerardo University Hospital (Italy) in 2007. However, simultaneous bilateral femoral neck fractures among the geriatric population (those aged ≥65 years) are rarely reported in the literature. Reporting the rare case of a frail 76-year-old woman admitted with bilateral hip fracture and end-stage renal disease, we explain the important role played by the OGU and its flexible multidisciplinary approach for providing comprehensi

Strategies to prevent postoperative delirium: a comprehensive evaluation of anesthesia selection and drug intervention.

Postoperative delirium (POD) represents a common neurological complication encountered predominantly among the elderly cohort undergoing surgical intervention for hip fractures. This phenomenon, particularly commonplace in geriatric populations with heightened preoperative risk profiles, pronounced comorbidities, and later stages of lifespan, poses complex clinical challenges. The impact of perioperative pharmacological interventions and anesthetic strategies on POD's emergence cannot be understated, as it may profoundly affect the length of hospital stays, rehabilitation milestones, and the o

[Experience with the use of clopheline in geriatric anesthesiology].

The new aspects of the use of the central adrenopositive agent clopheline as a component of combined endotracheal anesthesia were studied. In distinction to the recommendation of some authors, clopheline was used in the preanesthesia period in an average dose of 2.5-3.0 mcg/kg (in debilitated patients in a dose of 1.8-2.2 mcg/kg). The authors examined 86 patients 60 to 85 years of age during planned and emergency operations on the biliary tract. All of them suffered from arterial hypertension: 70% had three and more concomitant diseases; 20% of patients were related to IB category of anesthesi

An Oxycodone-Based Conscious Analgesia Strategy with Supplemental Low-Dose Propofol Improves Safety in Elderly Obese Patients Undergoing Gastroscopy: A Prospective Randomized Controlled Trial.

BACKGROUND: During diagnostic gastroscopy, intolerance is usually manifested as retching, coughing, and involuntary movement. Conscious analgesia relieves discomfort while patients continue to breathe spontaneously and remain partly responsive. Oxycodone, a semi-synthetic opioid, may reduce the need for deep propofol sedation. We compared an oxycodone-based conscious analgesia strategy with supplemental low-dose propofol with propofol-based deep sedation in elderly obese patients. PATIENTS AND METHODS: In this prospective, single-center, open-label randomized controlled trial, 128 patients ag
