Granulocyte colony-stimulating factor-producing squamous cell carcinoma of the lower gingiva: a case report.

The present study summarizes our experience in treating a patient with a suspected granulocyte colony-stimulating factor (G-CSF)-producing squamous cell carcinoma (SCC) of the lower gingiva, which is a rather rare entity. A 56-year-old woman underwent surgical excision of palate leukoplakia in 1996. In 2009, however, a leukoplakic superficial tumor was detected in the lower left gingiva, for which the patient underwent gingivectomy. This was subsequently diagnosed as SCC. The patient also underwent superselective arterial injection chemotherapy combined with radiotherapy, after local recurrenc

Vestibular unit transplantation in the coverage of gingival recession: a microsurgical approach.

The palate, maxillary tuberosity and edentulous ridges are generally used as the donor sites for connective tissue grafts. This article shows the use of vestibular soft tissue of a tooth in the coverage of localized gingival recession. The vestibular unit was harvested during crown lengthening procedures, sutured over the denuded root surface, and covered with a coronally advanced flap without vertical incisions. The healing was uneventful with complete coverage of the recession up to 18 months. The vestibular unit of a tooth can serve as the donor area for a connective tissue graft in the cov

Puberty-Associated Gingival Enlargement Exacerbated by Polycystic Ovary Syndrome: A Case Report.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrinopathy affecting reproductive-aged women, characterised by metabolic abnormalities, polycystic ovaries, hyperandrogenism, and chronic anovulation. This pathology has been attributed to hormonal imbalances, including elevated levels of insulin, growth hormone (GH), ghrelin, LEAP-2, GnRH, LH/FSH ratio (>2:1), androgenic hormones, and estrogen. While puberty-associated gingival enlargement is primarily triggered by dental plaque, systemic hormonal fluctuations can significantly exacerbate the tissue response. CASE PRESENTATION: A 1

Plasma Cell Gingivitis: A Clinicopathological Case Report Highlighting a Systematic Diagnostic Approach and Step-by-Step Surgical Management.

Plasma cell gingivitis (PCG) is a rare benign inflammatory condition characterized by diffuse erythematous gingival enlargement with dense plasma cell infiltration. Its clinical presentation overlaps with inflammatory, autoimmune, hematological, and plasma cell proliferative disorders, making accurate diagnosis challenging and necessitating a systematic clinicopathological evaluation. This report describes a 23-year-old male who presented with persistent gingival enlargement and bleeding involving the maxillary and mandibular anterior regions for two years despite repeated nonsurgical periodon

The diagnostic challenge of juvenile hyaline fibromatosis, a case report with literature reviews.

INTRODUCTION: Juvenile hyaline fibromatosis (JHF) is a rare genetic condition characterized by impaired collagen production or metabolism. This study aims to present a rare case of JHF. CASE REPORT: An 11-year-old boy presented with bilateral keloid-like lesions on his ears and admitted intermittent reappearance of such lesions since he was seven. He was born to second-degree relative consanguineous parents. Physical examination revealed bilateral soft pink masses on the ears, multiple scars on the scalp, severe gingival hypertrophy, multiple small soft white papules on the anterior neck, bro

Inactivation of the Odontogenic ameloblast-associated gene affects the integrity of the junctional epithelium and gingival healing.

Odontogenic ameloblast-associated (ODAM) belongs to the secretory calcium-binding phosphoprotein (SCPP) gene cluster. It is expressed by the epithelial ameloblasts during the accrued mineralisation of enamel and by cells of the junctional epithelium (JE), a specialised portion of the gingiva that plays a critical role in periodontal health. In both cases, ODAM localises at the interface between the cells and the tooth surface. It is also present among the cells of the JE, and is distinctively highly expressed in many epithelial tumours. ODAM has been proposed to be a matricellular protein impl

Expression pattern of odontogenic ameloblast-associated and amelotin during formation and regeneration of the junctional epithelium.

The junctional epithelium (JE) adheres to the tooth surface, and seals off periodontal tissues from the oral environment. This incompletely differentiated epithelium is formed initially by the fusion of the reduced enamel organ with the oral epithelium (OE). Two proteins, odontogenic ameloblast-associated (ODAM) and amelotin (AMTN), have been identified in the JE. The objective of this study was to evaluate their expression pattern during formation and regeneration of the JE. Cytokeratin 14 was used as a differentiation marker for oral epithelial cells, and Ki67 for cell proliferation. Immunoh

Mucogingival repairs in periodontal surgery.

The utilization of partial thickness apically repositioned flaps, laterally repositioned flaps, and free mucosal autografts enable the clinician to repair most mucogingival problems. These techniques may be used separately or in combination to increase the zone of attached gingiva, deepen the vestibular fold, and eliminate muscle and frenum pulls. If basic surgical principles are adhered to, the possibilities of repair of mucogingival problems utilizing these procedures are unlimited. The most important step in mucogingival surgery is planning the surgical procedure prior to beginning surgery.

Manipulation of the peri-implant tissue for better maintenance: a periodontal perspective.

Enhancement of peri-implant soft tissue is an essential factor in implant survival. As in periodontal tissue, the integrity of the attached gingiva, plus gingival contour, color, shape, size, consistency, and bleeding upon probing, is an indicator of bacterial activity that will eventually lead to gingivitis and periodontitis. The trajectory of peri-implant tissue is different from that of periodontal tissue because of periodontal ligament fibers, the absence of which makes the implant-bone interface weaker than that of natural dentition. The destruction of peri-implant tissue can be a faster

Clinical assessment of injuries in orthodontic movement of impacted teeth. II. Surgical recommendations.

Adverse effects on periodontal tissues have been associated with surgical techniques used to expose impacted teeth that are to undergo orthodontic tooth movement. The purpose of this study was to compare the effects on the periodontium of two of the more commonly used surgical techniques for labially impacted canines--(1) exposure of the entire labial aspect of the anatomic crown with total excision of all keratinized tissue (the window approach) and (2) a technique which exposes only 4 to 5 mm of the most superficial portion of the labial aspect of the cusp tip while maintaining 2 to 3 mm of

Recurrent gigantic pyogenic granuloma disturbing speech and mastication: a case report and literature review.

A case of gigantic pyogenic granuloma with three recurrences in the lower anterior gingiva is presented. Surgical wide excision of the lesion is the treatment of choice. The tumour must be excised down to the periosteum and the irritants around it removed to avoid recurrence. A contributing factor to the gigantic lesion is hormonal changes during pregnancy. Long-term review for 18 months after the third surgery showed no evidence of recurrence at the surgical site.

Forced eruption combined with gingival fiberotomy. A technique for clinical crown lengthening.

A new approach to clinical crown lengthening has been developed and described. The technique combines controlled eruptive tooth movement and incision of the supracrestal gingival attachment. The procedure was performed in patients with severe destruction of a tooth crown and in whom clinical crown lengthening procedures were essential before the teeth could be properly restored. Controlled eruptive forces were activated by simple orthodontic appliances. During the active phase of forced eruption, repeated intrasulcular incisions through the junctional epithelium and the supracrestal connective

Surgical Management of Hereditary Gingival Fibromatosis: Case Series.

Hereditary gingival fibromatosis (HGF) is an uncommon genetic condition marked by gradual and progressive overgrowth of fibrous tissue in the gums, which is benign in nature. It is a genetic disorder inherited in an autosomal dominant pattern, known for its considerable genetic diversity. The marginal, attached, and interdental gingivae are affected by this condition. The affected area appears pink, does not bleed easily, and exhibits a firm, fibrotic texture. Additionally, it displays a hard, widespread nodular growth that is smooth to stippled and has little bleeding tendency. Nevertheless,

Unusual gingival enlargement with aggressive periodontitis: a case report.

AIM: This article describes the surgical management of a young, female patient with severe gingival enlargement of unknown etiology. BACKGROUND: Gingival enlargement frequently occurs as gingival hyperplasia, representing a reaction to a known stimulus or agent and, histopathologically, implies an increase in both extracellular matrix and cell numbers. The enlargement may range from mild, in which gingival architecture is minimally affected, to severe, in which the gingiva becomes bulbous and covers the clinical crowns of teeth. A number of local and systemic factors, such as plaque, hormonal
