Open Versus Percutaneous Stabilization of Thoracolumbar Fractures: A Large Retrospective Analysis of Safety and Reoperation Rates.

BACKGROUND: Thoracolumbar fractures (TLF) requiring surgical intervention can be treated with either open or percutaneous stabilization, each with some distinct risks and benefits. There is insufficient evidence available to support one approach as superior. METHODS: Patients who underwent spinal fixation for TLF between 2008 and 2020 were reviewed. Patients with one or two levels of fracture treated with either open or percutaneous stabilization were included. Exclusion criteria were more than two levels of fracture, patients requiring corpectomy, stabilization constructs that crossed the ce

Anatomic reconstruction of the medial collateral ligament in multi-ligaments knee injury using achilles allograft : a modification of Marx's technique.

Medial Collateral Ligament (MCL) injury may require operative treatment. Marx et al. described the latest technique for reconstruction of MCL. While good results have been reported using the Marx technique, some issues have been observed. To address the mentioned issues, a modification to the Marx technique has been devised. Eleven patients were enrolled and their ligaments were repaired by the fixation of allograft on the proximal and distal attachment footprints of the superficial MCL. For preventing loss of knee ROM, MCL and other ligaments were reconstructed in 2 separate stages. At the la

Use of locking plates for fixation of the greater trochanter in patients with hip replacement.

BACKGROUND: Fixation of the greater trochanter with total hip replacement is challenging and associated with short- and long-term complications. Locking plate technology has been used for fixation of other bones and may be applied successfully in trochanteric fixation. The purpose of this retrospective study was to analyze the utility of the use of trochanteric locking plates in total hip arthroplasty (THA) patients. METHODS: From 2004 to 2014, 32 procedures were performed to fix the greater trochanter in patients with trochanteric fracture, osteotomy, or nonunion in the setting of THA. The m

Resumption of complete weight bearing after osteosynthesis of bimalleolar fractures using locking plates.

INTRODUCTION: Bimalleolar fractures represent 9% of fractures and affect 187/100,000 people per year. The gold standard for osteosynthesis is a one-third tubular or lateral locking plate and compression screwing or use of tension band wiring, with complete weight bearing planned around the 6th week. The development of locking plates seems to allow earlier resumption of weight bearing. The objective of our study was to evaluate the clinical and radiological results after internal fixation of bimalleolar fractures using locking plates with complete weight bearing authorized from the outset. The

Union Rates and Complications of Lateral Column Lengthening Using the Interposition Plating Technique: A Radiographic and Medical Record Review.

Lateral column lengthening is a common procedure for correction of pes planovalgus. A tricortical bone graft has been a standard among foot and ankle surgeons. The purpose of the present study was to compare the union rates and complications between the 2 forms of fixation for lateral column lengthening. The present study was a retrospective medical record and radiograph review of 52 patients divided into 2 equal groups, allograft (group A) and opening wedge plate (group B). The radiographic analyses compared the preoperative, postoperative and long-term measurements of cuboid abduction and ta

Comparative Analysis of Secondary Outcomes in Treatment of Charcot Neuropathy: A Systematic Review.

BACKGROUND: Charcot neuroarthropathy (CN) is commonly associated with diabetic neuropathy and can predispose patients to amputations. Management remains a challenge, with no definitive treatment. This study examines major postoperative complications associated with open reduction with internal (intramedullary) nails, fixation, external fixation, or combined internal and external fixation for correction of the deformity. METHODS: We reviewed available literature using PubMed, OvidSP, Scopus, and Cochrane and searched bibliographies of the included studies to identify additional pertinent refer

Rare Systemic Response to Titanium Spinal Fusion Implant: Case Report.

Neurosurgical patients with titanium spinal implant hypersensitivity can be difficult to diagnosis due to its rarity. Suspicion for titanium allergy is generally localized to the hardware site and may initially be thought to be an infectious process. Patients who report anorexia and fatigue over a long duration after the initial post-operative period may be diagnosed with depression rather than a systemic response to spinal metallic instrumentation. To our knowledge, a systemic titanium hypersensitivity reaction to spinal fixation devices has not been reported in the literature. We offer this

Partial Extensor Hallucis Longus Injury Following Lisfranc Fixation Loosening.

Lisfranc injuries are typically treated in the acute setting with open reduction and internal fixation (ORIF). The type of hardware that provides the best fixation for these injuries has not been definitively determined. Recently, dorsal bridge plating has increased in popularity. We report a case of partial extensor hallucis longus (EHL) injury after dorsal bridge plate fixation of a Lisfranc injury. The patient was successfully treated with hardware removal, tendon debridement, and tubularization. This case highlights a potential complication of dorsal bridge plating in the treatment of Lisf

Application of axial load-share ratio testing technology in the bone transport treatment of large segmental tibial bone defects.

BACKGROUND: Bone transport is a widely established method for treating tibial bone defects. However, safely removing the external fixator as early as possible to shorten treatment duration remains a clinical challenge. This study aimed to investigate whether axial load-share ratio (ALSR) testing can safely and effectively assess callus healing strength at the mineralization zone and docking site, and thereby guide the precise timing of external fixator removal during tibial bone transport. METHODS: This retrospective study included 42 adult patients with tibial bone defects treated by bone tr

Vertical incomplete humerotomy with suture loop cerclages for humeral stem extraction and reimplantation in revision of shoulder arthroplasty.

BACKGROUND: Extraction of well-fixed humeral implants during revision shoulder arthroplasty is difficult and prone to complications, especially humeral fracture (up to 12% in anatomic stem revision and 30% in reverse stem revision). To prevent this complication, we perform a vertical incomplete humerotomy (VIH) and use cerclages with suture loops and Nice knots for fixation. The purpose of this study is to describe the VIH technique with suture cerclage fixation, report the perioperative complications, and evaluate the radiological and clinical results outcomes. METHODS: We performed a retros

The arthroscopic-assisted removal of a distal femoral condylar locking plate--a case report.

We describe a minimally invasive procedure for removal of a distal femur locking plate using arthroscopic assistance. Using lateral accessory portals, we performed arthroscopically-assisted removal of distal locking screws. Under fluoroscopic image, proximal screws were removed from the diaphysis. The accessory lateral portal sites used to remove the distal screws were connected. Through this arthrotomy, the plate was elevated from the distal femur and removed without difficulty. With this method we were able to concomitantly visualize the intra-articular regions while accomplishing the hardwa

Free flap reconstruction for infection of ankle fracture hardware: case report and review of the literature.

BACKGROUND: Clinical management of orthopedic hardware infections related to ankle fracture fixation may present difficult therapeutic dilemmas. Typically, management includes removal of the hardware, debridement of necrotic tissue, and eventual placement of an alternative method of stabilization, usually, an external fixator or cast. However, problems arise when the fracture cannot be managed adequately with an external method. Such is the case with supination external rotation (SER) fractures, yet maintaining the hardware in the setting of infection typically is not considered an option. ME

Maxillary Sinusitis Following Orthognathic Surgery: Should It Be Considered Odontogenic Sinusitis?

Maxillary sinusitis is a recognized complication following dental procedures, but its occurrence after orthognathic surgery, such as Le Fort osteotomies, remains less documented. This case report presents a 58-year-old female who developed unilateral maxillary sinusitis 23 years post-orthognathic surgery. The patient was asymptomatic, aside from occasional cacosmia, and was incidentally found to have sinus opacification on a computed tomography (CT) scan performed for implant-prosthetic rehabilitation. Nasal endoscopy revealed purulence and mucosal edema, prompting endoscopic sinus surgery (ES

Spinal Cord Stimulator Explantation: Motives for Removal of Surgically Placed Paddle Systems.

BACKGROUND: Despite requiring successful trials prior to implantation, spinal cord stimulation (SCS) systems for pain are often later removed. Removing surgically implanted hardware subjects patients to the risks and discomfort of a second surgery, threatens the cost-effectiveness of SCS, and limits the perceived durability of SCS technology for pain problems. OBJECTIVE: To investigate patterns of reasons given among patients who underwent SCS explant surgery (SCSES). METHODS: Retrospective review of SCSES cases over 17 years at Allegheny General Hospital, Pittsburgh, PA. RESULTS: 165 patie

Staged Intracranial Free Tissue Transfer and Cranioplasty for a Refractory Nasal-cranial Base Fistula.

After a transnasal endoscopic resection of a high-grade adenoid cystic carcinoma that underwent adjuvant chemoradiation, there was delayed recurrence managed by en bloc resection through an open craniofacial approach. Subsequently, the patient developed a chronic nasocranial fistula with secondary infection and bone flap resorption. This resulted in infectious episodes with secondary scalp incisional dehiscence and hardware exposure which required multiple bone debridement procedures, hardware removal, prolonged IV antibiotics, and hyperbaric oxygen treatment. The nasocranial fistula and chron

Transverse femoral implant prominence: four cases demonstrating a preventable complication for ACL reconstruction.

Anterior cruciate ligament (ACL) tear is a commonly occurring injury that often demands surgical reconstruction. Although the utility of this operation is widely accepted, many specific components, including graft fixation technique, remain controversial. Many clinicians favor transverse femoral implant fixation for soft tissue ACL grafts. This technique can be accomplished successfully; however, in a minority of the cases, the femoral implant can be excessively prominent, leading to iatrogenic postoperative iliotibial band syndrome. This article presents 4 patients that developed postoperativ
