Tobacco-Free Nicotine Pouches and Their Potential Contribution to Tobacco Harm Reduction: A Scoping Review.

Tobacco harm reduction (THR) refers to strategies designed to reduce the health risks associated with tobacco smoking but may involve continued use of nicotine and/or tobacco. Next-generation products (NGPs) are a THR alternative as they do not burn tobacco or produce smoke and deliver nicotine and have fewer and substantially lower levels of harmful chemicals compared to cigarettes. Tobacco‑free nicotine pouches (TFNPs) are an emerging category of nicotine‑containing oral products that do not combust or contain tobacco leaf. Similar to Swedish snus, TFNPs are placed between a user's lip and g

The Need to Rethink Harm Reduction for People Using Drugs Alone to Reduce Overdose Fatalities.

Background: During the ongoing opioid epidemic, Cuyahoga County (second largest in Ohio) reported overdose mortality rates (54/per 100,000) higher than the national average. Prior research demonstrates that people who use drugs often use alone but there is minimal research on people who died of overdose while using alone. The objective of this study is to examine sociodemographic, toxicologic, and injury characteristics, and emergency medical response to overdose decedents who died using drugs alone. Method: Data from the Cuyahoga County Medical Examiner's Office (2016-2020, N = 2944) on unint

A lot testing protocol for quality assurance of fentanyl test strips for harm reduction applications.

Fentanyl test strips (FTS) are lateral flow immunoassays that were originally designed and validated for detecting low concentrations of fentanyl in urine. Some FTS are now being marketed for the harm reduction purpose of testing street drugs for the presence of fentanyl. This manuscript provides a simple protocol to assess whether different brands and lots of fentanyl test strips perform adequately for use in drug checking. The results gathered from this protocol will document problems with particular lots or brands of FTS, help buyers choose from among the array of products, provide feedback

Identifying Provider Barriers to the Implementation of Opioid Harm Reduction Methods: An Initial Narrative Review.

According to the Centers for Disease Control and Prevention, the opioid epidemic remains a major issue in the United States, with over 80,000 deaths attributed to opioids in 2021. This public health crisis continues to impact communities across the country, highlighting the need for intervention and reflecting the nation's failed attempts at prohibition through criminalization to reduce opioid use. Harm reduction methodshave proven to be effective in preventing adverse health outcomes and promoting the overall well-being of individuals with opioid use disorders. However, significant gaps remai

Substance use, harm reduction attitudes and behaviors among attendees of nature rave parties in Israel.

BACKGROUND: Few studies have analyzed harm reduction behaviors and attitudes among rave party attendees. Since the late 1980s, there has been a large Israeli rave scene, also known as 'Nature Parties'. However, only a few studies have been conducted among nature party attendees and almost all of them are from a qualitative perspective. This study's aim was to fill the gap and conduct quantitative research to investigate the patterns of substance use, harm reduction attitudes and behaviors among Israeli nature rave party attendees. METHODS: A cross-sectional online survey recruited 1,206 peopl

Harm Reduction, Solidarity, and Social Mobility as Target Functions: A Rortian Approach to Stakeholder Theory.

Instrumental Stakeholder Theory has begun to suffer from what might be termed "mission drift." Despite its initial success in creating a foothold for ethics in managerial decision-making, the efficiency arguments which now dominate this research stream have become counterproductive to the original goal of connecting ethics and capitalism. We argue in this paper that the way forward is by re-centering contingency, conversation, and inefficiency in stakeholder theory. To start this process, there needs to be a reckoning of some unintended impacts of the success of the instrumental stream of stak

Hepatitis C treatment outcomes among people who inject drugs accessing harm reduction settings in Kenya.

Data are limited on HCV treatment outcomes among people who inject drugs (PWID) in low- and middle-income countries (LMICs) and particularly sub-Saharan Africa. We provided ledipasvir/sofosbuvir under directly observed therapy (DOT) to 95 PWID accessing medication-assisted treatment (MAT) and needle and syringe programs (NSP) in Nairobi and Coastal Kenya. Participants were predominantly male (n=81, 85.3%), mean age of 36.5 years (SD=±6.5); 38 (40%) were HIV-positive, 12 (12.6%) were cirrhotic, and 87 (91.6%) reported injecting drugs in the last 30 days. Genotypes were 53 (55.8%) 1a, 39 (41.1%)

Harm Reduction Practices and Needs in a Belgian Chemsex Context: Findings from a Qualitative Study.

Chemsex is a growing public health concern, with little evidence-based care and support available. The aim of this study is to understand current risk reduction practices, and the information and care needs of gay, bisexual, and other men who have sex with men (GBMSM) who engage in chemsex. Between January and March 2020, semi structured in-depth interviews with drug-using GBMSM (n = 20) were conducted. Data were analyzed thematically. The reported preparatory measures were: deliberately scheduling chemsex sessions, and discussing preferences regarding setting and attendees. During the event,

Drug consumption rooms in Catalonia: A comprehensive evaluation of social, health and harm reduction benefits.

BACKGROUND AND AIMS: Despite the availability of several drug consumption rooms (DCR) in different European countries few epidemiological studies have evaluated their benefits. A network of DCR for people who inject drugs (PWID) has existed in Catalonia since 2000. We aimed to study the impact of frequently attending DCR on injecting in public, infectious risk (disposal of used syringes in safe places, sharing needles and/or injecting equipment), accessing drug dependence services and non-fatal overdoses. METHODS: In 2014-2015, we performed the cross-sectional study REDAN in Catalonia's netwo

Qualitatively and quantitatively evaluating harm-reduction goal setting among chronically homeless individuals with alcohol dependence.

INTRODUCTION: Most treatment programs for alcohol dependence have prioritized alcohol abstinence as the primary treatment goal. However, abstinence-based goals are not always considered desirable or attainable by more severely affected populations, such as chronically homeless people with alcohol dependence. Because these individuals comprise a multimorbid and high-utilizing population, they are in need of more focused research attention that elucidates their preferred treatment goals. The aim of this secondary study was therefore to qualitatively and quantitatively document participant-genera

A drug use survey among clients of harm reduction sites across British Columbia, Canada, 2012.

BACKGROUND: In British Columbia (BC), understanding of high-risk drug use trends is largely based on survey and cohort study data from two major cities, which may not be representative of persons who use drugs in other regions. Harm reduction stakeholders, representing each of the five geographic health regions in BC, identified a need for data on drug use to inform local and regional harm reduction activities across the province. The aims of this project were to (1) develop a drug use survey that could be feasibly administered at harm reduction (HR) sites across all health regions and (2) ass

A harm reduction paradox: comparing China's policies on needle and syringe exchange and methadone maintenance.

BACKGROUND: China has launched methadone maintenance treatment (MMT) and needle and syringe exchange programmes (NSEP) as part of the country's HIV prevention strategy amongst injection drug users. MMT is expanding, with backing from multiple government ministries, however, NSEP have received less political support and funding. METHODS: Semi-structured, serial interviews were conducted with key informants, knowledgeable about China's harm reduction policies. Concurrent content analysis allowed for revision of the interview guide throughout the data collection process. This was combined with a

A new type of harm reduction: creating arteriovenous fistula for a compulsively injecting opiate user.

Compulsive intravenous opiate injectors often cause themselves recurrent physical damage, which sometimes threatens life or limb. Unsuccessful attempts to find a vein can occupy several hours of each day, during which blood may clot in the syringe, making injection even more difficult. Adding small amounts of heparin to the opiate in the syringe before injecting prevents clotting but may be only partially helpful. The authors describe the first reported case in which an arteriovenous fistula was created specifically to enable a compulsive injector to inject quickly, easily, and safely.

Quality Improvement Initiative to Prepare Medical Students With Harm Reduction Principles for Substance Use Care.

BACKGROUND: Undergraduate medical education is increasingly incorporating harm reduction (HR). Assessments of HR curricula have focused on skills like overdose reversal. The literature lacks evaluations of curricula that more broadly teach HR-based physician-patient communication. METHODS: From academic years 2022 to 2024, a Harvard Medical School student working group collaborated with faculty to take a quality improvement approach to incorporating HR into the substance use interview curriculum. We implemented two Plan-Do-Study-Act cycles. We measured the effectiveness of HR education in the

Harm reduction engagement and ongoing opioid use among adults receiving methadone or buprenorphine in Philadelphia.

BACKGROUND: Opioid Use Disorder (OUD), a chronic condition with significant health and social consequences, has been intensified by synthetic opioids such as fentanyl. Although medications for OUD (MOUD) reduce mortality and improve treatment retention, many individuals continue to engage in non-prescribed opioid use while in treatment. Harm reduction services (HRSs), including syringe exchange sites, supervised injection facilities, or sites that distribute fentanyl test strips or naloxone, seek to mitigate the risks associated with substance use. This study explores engagement with harm redu

Community readiness for harm reduction approaches to drug use: A qualitative Pilot study in Nigeria.

In Nigeria, drug abuse presents mounting public health challenges, understanding community readiness for harm reduction strategies is crucial yet understudied. This qualitative study examined community readiness to adopt harm reduction approaches and support the reintegration of individuals with drug use disorders. Through in-depth interviews with 18 participants, including parents of children with and without drug use disorders and healthcare providers from the Neuro-Psychiatric Hospital in Akure; the study revealed a complex interplay of factors affecting drug use and recovery. Navigating US illicit drug market volatility: harm reduction strategies employed by people who inject drugs.

The US illicit drug market is increasingly unreliable, with new and often harmful adulterants contributing to overdose risk among people who use drugs. In this context, it is crucial to examine whether people who inject drugs (PWID) are aware of these changes and if so, how they are adapting to inform harm reduction interventions and programs. In-depth interviews were conducted with 23 PWID between July 2022 and January 2023. Interviews explored participants' substance use behaviors and perspectives on harm reduction interventions. A combined deductive and inductive thematic analysis was condu
