Outcome evaluation in patients with elbow pathology: issues in instrument development and evaluation.

Measurement of outcome after treatment of elbow pathology has tended to rely on nonstandardized scales and isolated impairment measures, particularly range of motion. This study evaluated the reliability and validity of patient questionnaires with patients with elbow pathology. Patients with a variety of complaints affecting the elbow (n = 70) completed four questionnaires-the American Shoulder and Elbow Surgeons Elbow Form; the Patient-rated Elbow Evaluation; the Disabilities of Arm, Shoulder and Hand questionnaire; and Short Form 36--during clinic attendance and returned repeat questionnaire

Time trends in cluster profiles of psychosomatic problems, self-rated poor health, and low life satisfaction from 2002 to 2022 among adolescents in five Nordic countries.

BACKGROUND: The aim was to perform person-oriented analyses among adolescents in five Nordic countries to estimate changes from 2002 to 2022 based on three key indicators of poor health status: psychosomatic complaints, self-rated poor health, and poor life satisfaction. METHODS: Data were employed from the HBSC survey of 15-year-olds (2002 to 2022) from five Nordic countries (N = 48,320): Denmark, Finland, Iceland, Norway, and Sweden. Cluster analyses were performed. RESULTS: Seven cluster profiles were identified. There were small differences between countries based on these profiles. Subs

Phenotyping malignant pleural effusions.

PURPOSE OF REVIEW: Patients with malignant pleural effusions (MPEs) are heterogenous in their disease course, symptom severity, responses to cancer therapies, fluid recurrence rates, and thus need for definitive fluid control measures. To tailor the most appropriate treatment for individual patients, clinicians need to 'phenotype' the patients and predict their clinical course. This review highlights the recent efforts to develop better predictive tools and knowledge gaps for further research. RECENT FINDINGS: The LENT scoring system, which includes pleural fluid lactate dehydrogenase, perfor

The income and health effects of tribal casino gaming on American Indians.

The legalization of American Indian casino gaming in the late 1980s allows examination of the relationship between income and health in a quasi-experimental way. Revenue from gaming accrues to individual tribes and has been used both to supplement tribe members' income and to finance tribal infrastructure. We assembled annual data from 1988-2003 on tribal gaming, health care access (from the Area Resource File), and individual health and socioeconomic characteristics data (from the Behavioral Risk Factors Surveillance System). We use this information within a structural, difference-in-differen

[Implementation and recording of child health and obesity indicators in Primary Care Child Health Programmes across Spain's autonomous communities: a descriptive key-informant survey].

OBJECTIVE: Child Health Programmes (CHPs) are widely implemented in Primary Care, but the territorial consistency of records on childhood obesity and social determinants is unknown. This study aimed to describe CHP implementation and digitalisation and compare indicators, resources and recording procedures across Spain's autonomous communities (ACs). METHODS: A cross-sectional descriptive study using an ad hoc online questionnaire with ten main questions and thirty-nine items was carried out, completed by one key informant from the Spanish Association of Primary Care Paediatrics in each of th

The ideological construction of risk: an analysis of corporate health promotion programs in the 1980s.

The nature of theoretical knowledge about health promotion and disease prevention programs at the worksite is examined in the context of knowledge as ideological construction. The emergence of the discourse about health promotion is linked to corporate attempts to reduce their share of medical expenditures and solve problems of productivity and absenteeism which plague the American workforce. Although not always evident, corporate managers choose selectively from a body of knowledge about medical costs and illness and disease in contemporary America. That knowledge reinforces the notion of ris

Child health in the United States: recent trends in racial/ethnic disparities.

In the United States, race and ethnicity are considered key social determinants of health because of their enduring association with social and economic opportunities and resources. An important policy and research concern is whether the U.S. is making progress toward reducing racial/ethnic inequalities in health. While race/ethnic disparities in infant and adult outcomes are well documented, less is known about patterns and trends by race/ethnicity among children. Our objective was to determine the patterns of and progress toward reducing racial/ethnic disparities in child health. Using natio

Health trends in Jamaica. Significant progress and a vision for the 21st century.

Health trends in Jamaica have improved considerably during the past century. Life expectancy at birth increased from 38 years in 1900 to 72 years in 2000. The crude death and infant mortality rates declined significantly from 35.7 and 174.3 deaths to 5.1 and 24.4 deaths, respectively, in the same period. The seven leading causes of death in 1945 were infectious diseases while the main causes of mortality and morbidity are now the chronic non-communicable lifestyle diseases, and injuries. Over the past few decades, considerable progress has been made in controlling vaccine preventable diseases

Pre-operative clinical assessment for anaesthesia and the effect of HIV infection.

BACKGROUND: HIV infection is common in South Africa, often remaining clinically latent and liable to be missed during clinical pre-operative assessment, despite the patient having a severe degree of immune compromise. OBJECTIVES: The primary objective was to determine the pre-operative physical status of patients presenting for anaesthesia, and to compare this with subsequent HIV tests and the CD4 counts of the HIV-positive patients. The secondary objective was to determine the prevalence of HIV infection in this group and in selected subgroups. METHOD: A sample of 350 adult patients present

Health and health perceptions among Kenyan grandparents.

The dramatic increase in the aging population in developed countries has led to an explosion of research on health and aging in the United States. Few studies, however, have been conducted in developing countries, even though many of these populations are experiencing a faster rate of growth in the 65+ population. Thus, although our knowledge of health and aging has increased, our knowledge of the variation in health as people age is limited. While the numbers of older adults is increasing in Africa, very little is known about the health and well-being of African elders. Recently, a growing nu

BACKGROUND: There are no studies on the health-related quality of life (HRQoL) in German children with a myelomeningocele (MMC). This study aims to obtain generalizable epidemiological data on the HRQoL of such children. PATIENTS AND METHODS: KINDL-R questionnaires were filled out and clinical findings on typical MMC disabilities were also documented. Of the 115 families contacted, 70 MMC families responded (response rate 61%). Normative KINDL-R data from a sample of healthy children served as reference. RESULTS: No differences in clinical data were found when comparing responders and non-re

Trends in inequality in maternal and child health and health care in Uganda: Analysis of the Uganda demographic and health surveys.

BACKGROUND: Uganda has made great strides in improving maternal and child health. However, little is known about how this improvement has been distributed across different socioeconomic categories, and how the health inequalities have changed over time. This study analyses data from Demographic and Health Surveys (DHS) conducted in 2006, 2011, and 2016 in Uganda, to assess trends in inequality for a variety of mother and child health and health care indicators. METHODS: The indicators studied are acknowledged as critical for monitoring and evaluating maternal and child health status. These in

Anthropometric Reference Data for Children and Adults: United States, 2011-2014.

Anthropometry or human body measurements provide important indicators of nutritional status in children and adults (1). In children, measurements reflect general health status, dietary adequacy, and growth and development over time. In adults, body measurements are used to evaluate health and dietary status, disease risk, and body composition. This report presents anthropometric reference data for 2011-2014 for U.S. children and adults.

Use of the Short Form-36 to detect the influence of upper gastrointestinal disease on self-reported health status.

Patient-centred outcome measures such as the Short Form-36 (SF-36) have been developed to assess the impact of ill health and medical interventions on self-reported health status. The objective of the study was to assess the impact of gastrointestinal disease upon health status as measured by the SF-36 physical and mental health component scores (PCS and MCS) and to assess whether these component scores might be an appropriate outcome measure for use in clinical research in gastroenterology. The subjects were 364 patients aged between 18 and 64 years who had been prescribed proton pump inhibit

Disparities in Preconception Health Indicators -
Behavioral Risk Factor Surveillance System, 2013-2015, and Pregnancy Risk Assessment Monitoring System, 2013-2014.

PROBLEM/CONDITION: Preconception health is a broad term that encompasses the overall health of nonpregnant women during their reproductive years (defined here as aged 18-44 years). Improvement of both birth outcomes and the woman's health occurs when preconception health is optimized. Improving preconception health before and between pregnancies is critical for reducing maternal and infant mortality and pregnancy-related complications. The National Preconception Health and Health Care Initiative's Surveillance and Research work group suggests ten prioritized indicators that states can use to m
