Reducing Low-Value Delirium Care Using Behavioral Design in the Electronic Medical Record.

Delirium is associated with increased morbidity, mortality, and healthcare costs. Despite best practice guidelines recommending non-pharmacological treatment strategies, delirium is frequently sub-optimally managed with reliance on antipsychotics or benzodiazepines. To optimize delirium management at our institution, we implemented a clinical decision support tool embedded within our Epic electronic medical record (EMR). Our team developed an Our Practice Advisory (OPA) triggered by a positive delirium screening result to alert hospitalists to patients at high risk for delirium. An accompanyin

Gastrointestinal tract and viral pathogens.

Viral gastroenteritis is the most common viral illness that affects the gastrointestinal (GI) tract, causing inflammation and irritation of the lining of the stomach and intestines. Common signs and symptoms associated with this condition include abdominal pain, diarrhea, and dehydration. The infections commonly involved in viral gastroenteritis are rotavirus, norovirus, and adenovirus, which spread through the fecal-oral and contact routes and cause non-bloody diarrhea. These infections can affect both immunocompetent and immunocompromised individuals. Since the pandemic in 2019, coronavirus

The Impact Of The COVID-19 Pandemic On Hospital Admissions In The United States.

Hospital admissions in the US fell dramatically with the onset of the coronavirus disease 2019 (COVID-19) pandemic. However, little is known about differences in admissions patterns among patient groups or the extent of the rebound. In this study of approximately one million medical admissions from a large, nationally representative hospitalist group, we found that declines in non-COVID-19 admissions from February to April 2020 were generally similar across patient demographic subgroups and exceeded 20 percent for all primary admission diagnoses. By late June/early July 2020, overall non-COVID

Placement of a Magnetic Small Bowel Feeding Tube at the Bedside.

BACKGROUND: Current methods of achieving postpyloric enteral access for feeding are fraught with difficulties, which can markedly delay enteral feeding and cause complications. Bedside tube placement has a low success rate, often requires several radiographs to confirm position, and delays feeding by many hours. Although postpyloric enteral tubes can reliably be placed in interventional radiology (IR), this involves greater resource utilization, delays, cost, and inconvenience. We assessed the utility of bedside enteral tube placement using a magnetic feeding tube (Syncro-BlueTube; Syncro Medi

The Clue is in the Eyes. A Case Report of Internuclear Ophthalmoplegia.

UNLABELLED: The chief complaint of vertiginous symptoms can be daunting, and the differential is quite long. Approximately 15% of patients presenting to the emergency department (ED) with dizziness have a dangerous underlying cause.1 We present a case of a 40-year-old female with a sudden onset of what she describes as vertigo, in the setting of intermittent diplopia. The patient was found to have a left medial rectus palsy consistent with a left internuclear ophthalmoplegia. Internuclear ophthalmoplegia (INO) is an abnormal gaze that is characterized by the weakness or inability to adduct the

Newborn Care Curriculum: The Late Preterm Infant in the Level II Neonatal Intensive Care Unit.

INTRODUCTION: Late preterm births represent the largest category of preterm deliveries. Although these infants are frequently managed in normal newborn nurseries, they have an increased risk of readmissions and mortality. Thus, we designed this PowerPoint-based learning module to provide learners with a computer-based educational resource on late preterm infants in the level II neonatal intensive care unit. METHODS: This module can be completed using the included pretest and posttest to assess for change in knowledge. The module should take approximately 20 to 30 minutes to complete. RESULTS

Understanding Local Consultation Patterns of Inpatient Geriatric Medicine Teams: a Cross-Sectional Study.

BACKGROUND: Geriatric consultation for Comprehensive Geriatric Assessment (CGA) improves outcomes of older adults living with frailty who are hospitalized, but consultation patterns and utilization of inpatient geriatric consultation teams by other hospital-based services are poorly understood. METHODS: We conducted a cross-sectional study using linked health administrative data to describe characteristics of older adults (≥ 65 years) who received a CGA while hospitalized between January 1, and December 31, 2019. We identified hospital-based services requesting CGA and the frequency and reaso

Inpatient Low-dose Transitions From Full Agonist Opioids Including Methadone Onto Long-acting Depot Buprenorphine: Case Series From a Multicenter Clinical Trial.

OBJECTIVES: Persons with opioid use disorder (OUD) suffer disproportionately from morbidity and mortality related to serious addiction-related infections requiring hospitalization. Long-acting buprenorphine (LAB) is an underused medication for OUD that may facilitate linkage to care and treatment retention when administered before hospital discharge. Transition onto buprenorphine in the inpatient setting is often complicated by pain, active infection management, potential surgical interventions, and risk of opioid withdrawal in transition from full agonists to a partial agonist. METHODS: The

The tectonic plates are shifting: cultural change vs. mural dyslexia.

In response to a rapidly changing healthcare marketplace, a variety of new business models have arisen, including new specialties (hospitalists), selective care (concierge medicine), and joint ventures (ambulatory surgical centers, specialty hospitals), some with hospitals and others with independent vendors. Since both hospitals and physicians are feeling the squeeze of rising expenses, burdensome regulations, heightened consumer expectations, and stagnant or decreasing reimbursement, the response to global economic competition and the need to improve clinical and financial outcomes can bring

A Quality Improvement Project to Increase Discharges before Noon at a Pediatric Hospital.

INTRODUCTION: Delays in patient discharges lead to inefficient hospital throughput and reduced access to care. The aim of this quality improvement (QI) project was to increase the percentage of discharge orders placed before noon (DOBN) by the pediatric hospital medicine (PHM) service from 27% to 32% between January 1, 2021, and December 31, 2022. METHODS: We convened a multidisciplinary team of hospitalists, residents, nurses, and bed management staff and surveyed key stakeholders to identify the most important barriers. Key interventions included the implementation of a multidisciplinary hu

Hospital Care for Pregnant Persons With OUD Review.

Opioid use disorder is increasingly prevalent among pregnant persons. Hospitalists and general obstetricians are uniquely positioned to treat pregnant persons with OUD, who may not otherwise be engaged in care. Pregnant people with OUD face stigma, discrimination, and fear of losing custody of their children. Providers should offer supportive and nonjudgmental care that is guided by the principles of harm reduction. All pregnant persons should be screened for OUD, diagnosed and treated accordingly. Medications for opioid use disorder (MOUD), methadone and buprenorphine, are safe in pregnancy a

Point-of-care ultrasound in skin and soft tissue infections.

BACKGROUND: Skin and soft tissue infections (SSTIs) are commonly encountered in clinical practice. Point-of-care ultrasound (POCUS) is becoming an increasingly valuable tool in hospital medicine, especially with advancements in ultrasound technology that make it easier to perform. POCUS can augment the history and physical exam in patients with suspected SSTIs. POCUS can detect deeper infections, such as abscesses, and expedite time to surgical debridement for life-threatening infections such as necrotizing fasciitis (NF). OBJECTIVES: This review provides an introduction on how to perform and

Expert perspectives for the pharmacist on facilitating and improving the use of albumin in cirrhosis.

PURPOSE: Albumin, the most abundant and arguably most important protein in the human body, plays a unique role in decompensated cirrhosis because its structure and function are quantitatively and qualitatively affected. A literature review was performed to provide insights into albumin use. The manuscript was developed using a multidisciplinary approach; 2 hepatologists, a nephrologist, a hospitalist, and a pharmacist, who are all members of or work closely with the Chronic Liver Disease Foundation, collaborated to write this expert perspective review. SUMMARY: Cirrhosis represents the potent

A Case of Multi-Organ Failure Secondary to Malabsorption Occurring in the Presence of Acute-on-Chronic Pancreatitis.

This case report highlights a concerning and complex case of a middle-aged female presenting with severe malabsorption, diarrhea, and subsequent malnutrition. The patient's weight dramatically dropped from 85 lbs to 50 lbs over the course of two to four months. The medical history included ongoing pancreatitis, esophageal ulcers, and previous surgeries for a dermoid cyst in the brain and cervical neoplasia. Upon admission to the hospital, the patient received total parenteral nutrition (TPN) on the first day, but this led to delirium due to refeeding syndrome. Refeeding syndrome is a well-know

Murine Typhus in 5 Children Hospitalized for Multisystem Inflammatory Syndrome in Children.

Multisystem inflammatory syndrome in children (MIS-C) is an emerging disease described in children in association with infection or epidemiological link to severe acute respiratory syndrome coronavirus 2. Signs and symptoms include fever, rash, and cardiac dysfunction; US Centers for Disease Control and Prevention have put forth broad criteria for diagnosis. The illness is serious and can progress rapidly to heart failure and death. However, findings in MIS-C are nonspecific, and there is significant overlap with other systemic illnesses, including Kawasaki disease and several viral and bacter

Clinician satisfaction with hospital at home: A multilevel framework.

BACKGROUND: Hospital at Home (HaH) programs provide acute inpatient-level care, can expand the capacity of traditional hospitals, and help address ongoing inpatient bed capacity issues while maintaining quality of care. While prior studies have explored patient satisfaction with HaH, scholars highlight a need for research into clinician satisfaction with HaH. PURPOSE: This study identifies factors that influence clinician satisfaction with the HaH model and suggests an approach for effective program design and implementation. APPROACH: This study takes a qualitative approach, examining the p

Is hospitalization a missed opportunity to intervene on tobacco cessation?

BACKGROUND: Although tobacco use is associated with elevated morbidity and mortality, its use remains widespread among adults within the United States. Nicotine Replacement Therapy (NRT) products are effective aids that improve rates of tobacco cessation. Many smokers interact with the medical system, such as during hospitalization, without their tobacco use addressed. Hospitalization is a teachable moment for patients to make health-related changes, including tobacco cessation. METHODS: Retrospective cohort study of adult patients in a university-based patient-centered medical home from 2012

Improving the Safety of Opioid Use for Acute Noncancer Pain in Hospitalized Adults: A Consensus Statement From the Society of Hospital Medicine.

Hospital-based clinicians frequently treat acute, noncancer pain. Although opioids may be beneficial in this setting, the benefits must be balanced with the risks of adverse events, including inadvertent overdose and prolonged opioid use, physical dependence, or development of opioid use disorder. In an era of epidemic opioid use and related harms, the Society of Hospital Medicine (SHM) convened a working group to develop a consensus statement on opioid use for adults hospitalized with acute, noncancer pain, outside of the palliative, end-of-life, and intensive care settings. The guidance is i
