Structural Syndemics and Antiretroviral Medication Adherence Among Black Sexual Minority Men Living With HIV.

BACKGROUND: Although HIV antiretroviral treatment (ART) access and uptake have increased among racial/ethnic minority individuals, lower rates of ART adherence and viral suppression persist, especially among Black men who have sex with men (BMSM) compared with their White counterparts. SETTING: Black men who have sex with men living with HIV (BMSM+) residing in Los Angeles County (N = 124) were recruited in-person (eg, clinic) and online (eg, social networking apps). METHODS: Participants completed a cross-sectional survey measuring demographic characteristics, structural syndemics (poverty,

Impact of the Domestic Violence Housing First Model on Survivors' Safety and Housing Stability: 12-Month Findings.

Intimate partner violence (IPV) is a widespread and devastating phenomenon resulting in a myriad of long-term consequences for survivors and their children. IPV victimization not only has negative health and economic consequences, it has also been linked to homelessness and housing instability. In response, the Domestic Violence Housing First (DVHF) model is being used in some domestic violence (DV) agencies to help survivors attain safe and stable housing. The model includes using individualized advocacy and/or flexible funding to help survivors meet these goals. Using a longitudinal, quasi-e

Performance of 2 Single-Item Screening Questions to Identify Future Homelessness Among Emergency Department Patients.

IMPORTANCE: Despite increasing interest in assessing patient social needs in health care settings, there has been little research examining the performance of housing-related screening questions. OBJECTIVE: To examine the performance of 2 single-item screening questions assessing emergency department (ED) patients' self-perceived risk of future homelessness. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study was conducted among a randomly selected sample of adult ED patients from 2016 to 2017 in a public hospital ED in New York City. Data were analyzed from September 2019 throu

Longitudinal high-frequency ethnographic interviewing to simulate and prepare for intensive smartphone data collection among veterans with homeless experience.

OBJECTIVE: While Veteran homelessness has steadily declined over the last decade, those who continue to be unhoused have complex health and social concerns. Housing instability interferes with access to healthcare, social services, and treatment adherence. Preventing unwanted housing transitions is a public health priority. This study is the first phase of a larger research agenda that aims to test the acceptability and feasibility of smartphone-enabled data collection with veterans experiencing homelessness. In preparation for the development of the smartphone data collection application, we

Health-related social needs and distress in cancer patients and survivors: A global scoping review of food, housing, and transportation insecurity.

BACKGROUND: Health-related social needs (HRSNs) such as food insecurity, housing instability, and transportation barriers affect cancer outcomes, yet their relationship with patient distress remains unclear. This scoping review evaluates how these HRSNs contribute to psychological, emotional, and psychosocial distress among cancer patients and survivors. METHODS: Six databases (Medline, Embase, PsycInfo, CINAHL, Cochrane Central, Sociological Abstracts) were searched. Of 840 screened records, 11 studies (2010-2024) from the United States (7), Australia (2), Mexico (1), and the United Kingdom

Chronic social isolation, crowding, and instability distinctly impact reproductive function in adult female Wistar rats.

Social stressors represent highly relevant stress models which remain understudied in females. Concordantly, little is known of the impact of different types of social stress on female reproductive functioning, despite close interactions between the hypothalamic-pituitary-adrenal (HPA) axis and hypothalamic-pituitary-gonadal (HPG) axis. This study provides a comprehensive assessment of reproductive functioning in female rats following exposure to three social stress paradigms. 112 regularly cycling adult female Wistar rats were exposed to 21 days of isolation, crowding, social instability, con

An Exploratory Study to Assess Individual and Structural Level Barriers Associated With Poor Retention and Re-engagement in Care Among Persons Living With HIV/AIDS.

BACKGROUND: Retention in care is the most challenging step along the HIV care continuum. Many patients who engage in care and achieve viral suppression have care interruptions, characterized by moving in and out of care ("churn"). Poor retention has clinical consequences and contributes to new HIV transmissions, but how to predict or prevent it remains elusive. This study sought to understand the relationship between individual- and structural-level barriers, and poor retention for persons living with HIV/AIDS in Atlanta, GA. METHODS: We administered a survey, through interviews, with HIV-inf

Characteristics and health of homeless families: the ENFAMS survey in the Paris region, France 2013.

BACKGROUND: The objectives were to estimate the size of homeless family population in Paris region, to describe their living conditions and health and to analyse the impact of homelessness on children's growth and development, which was never investigated in France. METHODS: A cross-sectional survey was conducted on a random sample of homeless sheltered families in 2013. Families were interviewed in 17 languages and a nurse took anthropometric measures, blood samples and collected health data from child health reports. RESULTS: The population size was estimated at 10 280 families. Half were

Beyond individual factors: a critical ethnographic account of urban residential fire risks, experiences, and responses in single-room occupancy (SRO) housing.

BACKGROUND: Burn injuries are a significant public health concern, closely linked to housing conditions and socioeconomic status. Residents in socioeconomically deprived neighbourhoods are at increased risk of exposure to hazards due to older and poorer housing conditions and limited access to fire protection measures. Individual behaviours such as substance use, smoking, and hoarding are often highlighted as primary causes of residential fires, overshadowing the broader socioeconomic and structural factors that also play a significant role in housing safety. This paper explores the correlatio

Families' Experiences of Doubling Up After Homelessness.

This study examined experiences of doubling up among families after episodes of homelessness. Doubling up refers to two or more adults or families residing in the same housing unit, which has been an increasing trend in the United States in recent decades. Within the past 14 years, the number of households containing more than one family, related or unrelated, has more than tripled. Although doubling up is increasingly common among families at all income levels, this study seeks to understand the experiences of doubling up among families who have been homeless. Through qualitative interviews w

Participant Needs, Service Utilization, and Costs in a Medicaid Housing Pilot Program.

IMPORTANCE: Housing instability and homelessness have wide-ranging health and social effects. Health systems increasingly recognize the importance of addressing these needs to make progress toward health equity goals and reduce health care costs. State Medicaid programs are uniquely positioned to support the housing needs of their members but are relatively new to the housing space and need to understand how to most effectively partner with the housing sector to improve health outcomes. OBJECTIVE: To evaluate the implementation and initial outcomes of a housing benefit pilot program, includin

Housing and displacement as risk factors for negative health outcomes among people who inject drugs in Los Angeles, CA, and Denver, CO, USA.

BACKGROUND: The United States is currently experiencing a housing and homelessness crisis. In response, many cities have adopted policies of displacement that move unhoused people from place to place. Recent research indicates that these policies may have negative health impacts on unhoused people who use drugs. We sought to examine health risks associated with forced displacement among unhoused people who inject drugs (PWID). METHODS: We interviewed a community-recruited sample of opioid-using PWID in Los Angeles, CA and Denver, CO between April 2021 and November 2022 (N = 472) about their de

Variation in Health Among Unstably Housed Youth From Cities, Suburbs, Towns, and Rural Areas.

PURPOSE: Youth face similar rates of homelessness across rural and urban areas, yet little is known about how the health of unstably housed youth varies by location. We assessed differences in health by location (city, suburb, town, and rural) and housing status among youth facing a range of unstable housing experiences. METHODS: This secondary data analysis from 8th, 9th, and 11th graders completing the 2019 Minnesota Student Survey examined youth who had experienced housing instability in the prior year (n = 10,757), including running away (48%) or experiencing homelessness (staying in shel

When Policy Is Not Enough: Intimate Partner Violence Survivors' Search for Housing Assistance in Washington, D.C.

Policies throughout the U.S. respond to the well-documented relationship between housing instability and intimate partner violence (IPV), but in practice survivors' needs often remain unmet. In this community-based participatory study, an interdisciplinary research team interviewed 41 survivors seeking emergency housing assistance at the sole public gateway for homeless families in Washington, D.C. Only four survivors received immediate assistance, and half were determined ineligible for services. While some felt genuinely supported, many survivors faced long wait times and experienced shaming

A Sort of Homecoming: Incarceration and the housing security of urban men.

While individuals returning from prison face many barriers to successful re-entry, among the most serious are the challenges they face in securing housing. Housing has long been recognized as a prerequisite for stable employment, access to social services, and other aspects of individual and family functioning. The formerly incarcerated face several administrative and de facto restrictions on their housing options; however, little is known about the unique instabilities that they face. We use a longitudinal survey of urban families to examine housing insecurity among nearly 3,000 urban men, in

Bridges to Elders: A Program to Improve Outcomes for Older Women Experiencing Homelessness.

Homelessness among older individuals is increasing and women experiencing homelessness have been previously shown to have poorer health outcomes than their male counterparts. To address these concerns, the Bridges to Elders (BTE) program was developed to improve health and social outcomes for older women experiencing or at risk for homelessness. BTE consisted of a nurse practitioner (NP) and community health worker (CHW) dyad who provided intensive case management services for women 55+ with housing instability. This evaluation used a pretest/posttest design to examine three main outcomes from

Cost burden among the CF population in the United States: A focus on debt, food insecurity, housing and health services.

BACKGROUND: Advancements in the cystic fibrosis (CF) field have resulted in longer lifespans for individuals with CF. This has led to more responsibility for complex care regimens, frequent health care, and prescription medication utilization that are costly and may not be fully covered by health insurance. There are outstanding questions about unmet medical needs among the U.S. population with CF and how the financial burden of CF is associated with debt, housing instability, and food insecurity. METHODS: Researchers developed the CF Health Insurance Survey (CF HIS) to survey a convenience s

Patient Characteristics Associated With Group Prenatal Care Attendance in a Randomized Controlled Trial.

INTRODUCTION: Group prenatal care (GPNC) represents a promising alternative to individual prenatal care (IPNC) for better perinatal outcomes. Some studies suggest better perinatal outcomes with more GPNC sessions attended. The present study sought to examine patient characteristics associated with higher or lower GPNC session attendance. METHODS: We conducted a secondary analysis of data collected in a single-site randomized controlled trial comparing GPNC to IPNC, focusing solely on patients assigned to GPNC. The outcome of interest was GPNC session attendance. Predictors evaluated included
