Percutaneous probe reduction of frontal fractures of the humeral capitellum.

Nine frontal fractures of the humeral capitellum treated by percutaneous probe reduction are reported. Most of the fractures were percutaneously reduced with a Steinmann pin inserted along the lateral border of the biceps tendon. To prevent redisplacement of the fragment, the elbow joint was first flexed more than 90 degrees with plaster immobilization for two weeks. The favorable results obtained with this method are due to reduced surgical trauma to local tissues and good reduction of the fracture. The percutaneous probe reduction should be considered for frontal fracture of the humeral capi

Humeral shaft split fracture around proximal humeral locking plates: a report of two cases.

Locking plates increasingly are being used in the treatment of osteoporotic fractures. Such devices provide multiple fixed- angle anchorage points and improve fracture fixation stability in weak bone. We report two cases of early proximal humeral fracture fixation failure in osteoporotic bone by humeral shaft "fissure" or split fracture after open reduction and internal fixation with locking proximal humeral plates and screws. Each patient failed early in the postoperative period and was revised to fixation with compression plating techniques, with uneventful union.

The Humeral Head Push-Pull Plate Technique: An Alternative Way to Stabilize the Humeral Head in Varus Angulated Proximal Humeral Fractures in the Elderly.

Despite the introduction of different surgical techniques and implants, osteoporotic varus displaced fractures of the proximal humerus treated with an angular stable plate have still a high risk of recurrence and/or screw penetration leading to revision surgery. Here, we describe a technique where we use an alternative approach to stabilize the humeral head by the use of apical subchondral bone anchors tightened to an angular stable plate. This technique eliminated the need for screws in the upper part of the humeral head, allowing the upper angular stable screws to be redirected to the infero

Decreased Hip, Lower Leg, and Humeral Fractures but Increased Forearm Fractures in Highly Active Individuals.

It is not known how physical exercise affects the risk of different types of fractures, especially in highly active individuals. To investigate this association, we studied a cohort of 118,204 men and 71,757 women who from 1991 to 2009 participated in Vasaloppet, a long-distance cross-country skiing race in Sweden, and 505,194 nonparticipants frequency-matched on sex, age, and county of residence from the Swedish population. Participants ranged from recreational exercisers to world-class skiers. Race participation, distance of race run, number of races participated in, and finishing time were

Anterior minimally invasive plating osteosynthesis using reversed proximal humeral internal locking system plate for distal humeral shaft fractures.

OBJECTIVE: To describe the surgical application of anterior minimally invasive plating osteosynthesis (MIPO) using reversed proximal humerus internal locking system (PHILOS) plate for multifragmentary distal humeral shaft fractures. METHODS: Twelve patients with distal humeral shaft fractures (type 12B, n = 6 and type 12C, n = 6) were operated on by anterior MIPO and reversed PHILOS plate fixation. The amount of intact bone in the distal fragment was measured by fracture-to-coronoid distance (FCD). Data of the postoperative alignment, complications, union time, and clinical outcomes were coll

Importance of anterior humeral line for successful anatomical reduction in the surgical treatment of pediatric supracondylar humeral fractures.

Anterior humeral line (AHL) and tilting angle (TA) are used for the assessment of pediatric elbow sagittal plane alignment in surgical treatment of pediatric supracondylar humeral fracture. However, few studies exist that compare the reliabilities between these parameters. The purpose of this study is to determine whether measurements of radiographic parameters are reliable and useful for achieving anatomical reduction. In the current study, we demonstrated that the identifying the AHL location involves a simple and reliable measurement compared with TA. The intraoperative AHL identification i

[A comparative study between humeral head prosthesis replacement and internal fixation for treatment of comminuted proximal humeral fractures].

OBJECTIVE: To compare the clinical efficacies of humeral head prosthesis and internal fixation in the treatment of comminuted proximal humeral fractures. METHODS: The clinical data were analyzed for the patients with comminuted proximal humeral fractures undergoing surgeries for humeral head replacement or open reduction plus internal fixation in our hospital between January 2002 and January 2009. Constant scores were used to determine the excellent clinical outcome rates in the two groups, and the operating time, blood loss and postoperative motor scores of the shoulder were compared. RESUL

Fracture obliquity is a predictor for loss of reduction in supracondylar humeral fractures in older children.

Supracondylar humeral fractures in older children have different biomechanical characteristics and surgical outcomes when compared to the same fractures in younger children. We aimed to analyze the fracture's architecture in a large group of older children and investigate the correlation between patients' variables, fracture patterns, fixation techniques and the rate of loss of reduction (LOR). A retrospective review study was conducted. We collected the records of 240 consecutive patients aged 8-14 years that sustained Gartland type 2/3 supracondylar humeral fractures between 2004 and 2014 an

Fractures of the proximal- and middle-thirds of the humeral shaft should be considered as fragility fractures.

AIMS: The aim of this study was to determine the current incidence and epidemiology of humeral diaphyseal fractures. The secondary aim was to explore variation in patient and injury characteristics by fracture location within the humeral diaphysis. METHODS: Over ten years (2008 to 2017), all adult patients (aged ≥ 16 years) sustaining an acute fracture of the humeral diaphysis managed at the study centre were retrospectively identified from a trauma database. Patient age, sex, medical/social background, injury mechanism, fracture classification, and associated injuries were recorded and analy

Strangulation of Radial Nerve Within Nondisplaced Fracture Component of Humeral Shaft Fracture.

Rates of radial nerve injury, which is commonly associated with humeral shaft fractures, range from 8% to 12%. This neurapraxia typically recovers with nonoperative management. In some conservatively treated cases, the radial nerve is lacerated or entrapped. Patients with a lacerated or entrapped nerve may have better outcomes with early operative management. We report on a rare case of the radial nerve entrapped within a nondisplaced segment of a closed humeral shaft fracture and describe the clinical outcome of early operative management.

[Old and young-Differences in the treatment of proximal humeral fractures?].

The rising incidence of proximal humeral fractures has made this an important topic in the treatment reality of trauma surgery. Given the variety of treatment options available, the question arises as to what extent patient age influences the choice of treatment and, consequently, the functional outcome. In our approach the morphological features of the fracture and individual patient characteristics form the foundation for determining whether surgical or conservative treatment should be pursued. Whenever possible, joint-preserving reconstruction should be the goal in younger patients. In olde

Olecranon Osteotomy Exposure for Distal Humeral Fracture Treatment.

BACKGROUND: Olecranon osteotomy (OO) is commonly utilized to improve exposure when treating intra-articular distal humeral fractures. A chevron-shaped osteotomy facilitates reduction and increases surface area for healing1. Following distal humeral fracture reduction and fixation, the OO fragment is fixed with a precontoured plate. The OO technique yields comparable outcomes to alternative techniques1,2. DESCRIPTION: The technique is performed as follows. (1) Imaging is reviewed and preoperative planning is performed. (2) The patient is positioned in the lateral decubitus position with the op

Pediatric Humeral Shaft Fracture Nonunion: A Case Report.

Humeral shaft fractures in the pediatric population are a commonly encountered injury in everyday practice. Most patients with these injuries are treated without surgery and go on to have an uneventful recovery. Nonunion of these injuries in the pediatric population has been reported only once in the literature. This case report follows a 13-year-old female after a seemingly standard transverse humeral shaft fracture. The patient was treated with a fracture brace initially. No signs of healing were noted at the eight-week post-injury follow-up. The family elected for continued conservative man

Altered Glenohumeral Biomechanics in Proximal Humeral Fracture Malunion.

INTRODUCTION: There is little biomechanical evidence to support the traditional guideline that 45° represents acceptable proximal humerus deformity. We evaluated glenohumeral contact pressure (GP) and area, subacromial contact pressure, and joint abduction to assess biomechanical changes with different proximal humerus deformities. METHODS: Fifteen fresh-frozen cadaver shoulders were used. Intact specimens were tested on a custom dynamic shoulder frame. Subsequently, a surgical neck fracture was made in each specimen and fixed using a custom dual hinge plate for fixation of 15°, 30°, and 45°

Humeral Shaft Fracture Healing Rates in Older Patients.

Nonoperative treatment has become the standard of care for the majority of humeral shaft fractures. Published studies have mainly come from trauma centers with a young cohort of patients. The purpose of this study was to determine the nonunion rate of humeral shaft fractures in patients older than 55 years. A retrospective study was performed on a group of orthopedic trauma group treated at a level I trauma center during a 10-year period (2007-2017). Patients 55 years or older and treated for a humeral shaft fracture nonoperatively, with or without manipulation, were identified. Nonunion was d
