Imperforate hymen is the most frequent cause of haematocolpos, although it is a rare malformation (1:2000). We present two cases of young girls with cyclic abdominal pain and urinary symptoms. At gynaecological examination, they all presented imperforate hymen and ultrasound revealed significant vaginal distension. X-shaped hymenectomy was performed in all patients. The management of the obstructed hemivagina and ipsilateral renal anomaly, or OHVIRA syndrome, requires vaginal access compromising sometimes the hymenal integrity. A vaginoplasty is the most commonly performed procedure in such scenarios. However, the necessity to maintain the hymen integrity may limit this technique. In this case report, we describe a safe and successful laparoscopy-assisted resection for the management of a high vaginal septum while maintaining an intact hymen.

INTRODUCTION: Hymeneal imperforation is a rare genital malformation often discovered during abdominopelvic emergencies. Hematocolpos typically presents with pelvic pain, a palpable mass, and primary amenorrhoea. The diagnosis is confirmed by clinical evaluation and imaging studies such as ultrasound.
CASE PRESENTATION: A 13-year-old girl presented a four-week history of abdominal distension and pain, which worsened over time, along with constipation, but without vomiting or fever. Examination revealed stable vital signs and abdominal guarding. A gynecological exam showed an imperforate hymen.

Various organic lesions can give rise to superficial dyspareunia. Acute urinary retention is a symptom that often accompanies hematocolpometra. The imperforate hymen is a rare gynecologic abnormality that can be easily diagnosed on initial presentation. Evidence describing age-related differences among children with suspected physical and sexual child abuse is lacking. We describe findings in severe cases of suspected abuse. The diagnostic circumstances, clinical features and complementary explorations in hematocolpos are discussed. Hematocolpos due to imperforate hymen is treated by simple star incision protecting the Bartholin gland orifices to separate the hymen and the meatus. Hematocolpos resulting from duplex uterus with a blind hemivagina is more difficult to treat.
