BACKGROUND: Outpatient hysteroscopy has become well-established for the investigation abnormal uterine bleeding. Although "See and Treat" clinics have been widely introduced, the types of procedures offered are limited, and many patients with intrauterine pathology continue to be admitted as in-patients for hysteroscopic surgery. We wanted to investigate the feasibility and acceptability of surgery for small intrauterine lesions without the need for general anesthesia by using a miniature resectoscope.

Hysteroscopy is frequently used to assist the diagnosis and treatment of a series uterine diseases. However, complications associated with the procedure including fluid overload, hyponatremia, hypo-osmolality, hemorrhage, uterine perforation, and air embolism have been reported. OBJECTIVE: To evaluate hysteroscopic metroplasty and its effect on reproductive outcome.
STUDY DESIGN: Seventy-five patients consulted for uterine septum between January 1996 to December 2000 at "La Sapienza" University in Rome were included in the study.
RESULTS: Forty patients attempted pregnancy and a total of 30 spontaneous pregnancies resulted (75% pregnancy rate). The uterine cavity was normal at hysteroscopic follow-up in 68% of cases.

We describe a case of osseous metaplasia into the cervical canal in a 41-year-old woman. The patient had a history of primary infertility, pelvic pain, chronic endocervicitis, and bone formation in the upper third of the cervical canal. A case of uterine rupture at 32 weeks' gestation, one year after hysteroscopic resection of uterine septa, is reported. A review of the literature shows only 16 other reported cases of uterine rupture following hysteroscopic surgery. The physiopathology and the prevention of these adverse effects are discussed.

Ten women with submucous myomas that could not be completely resected hysteroscopically were treated with intracervical carboprost. This caused uterine contraction and extrusion of the unresectable intramural component into the endometrial cavity. Eleven of 13 myomas in these patients could then be completely resected. One intramural myoma extruded into the endometrial cavity after injection of carboprost and was also completely resected.

STUDY AIM: To evaluate the follow-up after operative hysteroscopic resection of submucous leiomyomas.
PATIENTS AND METHODS: Between January 1990 and December 1996, 200 patients underwent operative hysteroscopic resection of 289 uterine leiomyomas. Indications were: menometrorrhagia (n = 159), postmenopausal metrorrhagia (n = 22), infertility (n = 19) as sole etiology. Sixteen patients had infertility and menometrorrhagia.

In a retrospective case report series, we evaluated the efficacy of hysteroscopic adhesiolysis in patients with severe Asherman's syndrome. In 31 patients with permanent severe adhesions, hysteroscopic treatment was performed. In all patients, uterine cavity with at least one free ostial area was restored after one (n = 16), two (n = 7), three (n = 7), and four (n = 1) surgical procedures.
