Decreased exchangeable body sodium might be a consequence of proctocolectomy and construction of an ileal reservoir. To investigate this, 35 patients with ulcerative colitis and conventional ileostomy were studied before and after conversion to continent ileostomy and compared with 25 unoperated subjects as reference. The sodium urinary excretion varied between 7 and 229 and 1 and 217 mmol/24 h in patients with conventional and reservoir ileostomy, respectively. The total exchangeable sodium was measured and related to body composition estimated from body weight, total body water, and total bo

BACKGROUND: Protective ileostomy is often performed following anterior resection for rectal cancer to minimize the clinical consequences of anastomotic leakage. The actual operation to create the stoma is usually straightforward, although ileostomy management can result in considerable patient morbidity. The timing of ileostomy closure continues to be a subject of debate. While early closure of the stoma may alleviate an individual's experience of prolonged skin irritation or persistent concerns regarding hydration, it still raises concerns about postoperative safety.
METHODS: We performed a

BACKGROUND: Temporary loop ileostomy formation is a routine procedure performed to protect distal colorectal anastomosis. Traditionally, the ideal timing of ileostomy reversal is believed to fall between 3 and 6 months. It has been suggested that delayed reversal increases the risk of complications. Our study looks to demonstrate the relationship between delayed reversal and its associated risks in a real-world setting.
METHODS: This is a retrospective cohort study conducted at Monash Health, analysing patients who underwent loop ileostomy reversal from May 2016 to December 2023. Main outcome

Surgical site infection (SSI) remains a major concern after ileostomy closure, limiting the adoption of primary closure despite the cosmetic and practical advantages of primary closure. A 0.1% betaine-0.1% polyhexanide solution, a surfactant-disinfectant solution with biofilm-disrupting properties, has been shown to significantly reduce SSI rates in various acute and chronic wounds throughout the body. This single-institution case series is the first in the literature to evaluate SSI rates with the use of betaine-polyhexanide irrigation and primary closure of ileostomy sites. All patients demo

BACKGROUND: A significant number of patients experience complications of the Kock pouch (KP) warranting revision or excision. This systematic review aimed to assess the pooled prevalence and risk factors for complications and failure of the KP.
METHODS: This Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA)-compliant systematic review (CRD42023416961) searched PubMed, Scopus, and Web of Science for studies on adult patients with Kock continent ileostomy published after the year 2000. The main outcome measures were revision, complications, and failure of the KP. Risk

PURPOSE: Young children with medically refractory very early-onset inflammatory bowel disease (VEO-IBD) sometimes benefit from ileostomy diversion alone or may be offered subtotal colectomy with ileostomy. Though generally well-tolerated, ileostomy complications are frequent. Prolapse is particularly frustrating as it can be difficult and painful to reduce, becomes a recurring problem is some patients, and often requires ostomy revision or bowel resection.
METHODS: Over the course of the past 6 months, eight consecutive children with VEO-IBD underwent 10 creation or revision of a diverting il

An anastomotic leak (AL) is the most serious complication observed in laparoscopic anterior resection of rectal cancer (LARRC). In order to protect anastomosis from AL and avoid stoma reversal surgery in patients with ileostomy, spontaneously closing cannula ileostomy (SCCI) was used in LARRC and its safety and feasibility were assessed in the present study. To the best of our knowledge, this is the first time that SCCI has been used in such a case. A total of 41 patients who underwent LARRC with SCCI or ileostomy procedures between November 2013 and August 2014 were retrospectively analyzed.

INTRODUCTION: Total abdominal colectomy with ileal pouch-anal anastomosis is the intervention of choice for patients with medically uncontrolled ulcerative colitis. A three-stage approach is preferred in particularly debilitated patients. In this setting, laparoscopic surgery has shown to be safe, offering several advantages over the open approach. Single incision laparoscopic surgery is a new minimally invasive approach which represents a true scarless procedure for the first step of the restorative proctocolectomy. In this article, we describe our technique in performing the single-incision

High-dose methotrexate (HD-MTX) is an important treatment for Burkitt lymphoma, but can cause hepatic and renal toxicity when its clearance is delayed. We report a case of acute renal failure after HD-MTX therapy in a patient with ileostomy, The patient was a 3-year-old boy who had received a living-related liver transplantation for congenital biliary atresia. At day 833 after the transplantation, he was diagnosed with PTLD (post-transplantation lymphoproliferative disorder, Burkitt-type malignant lymphoma). During induction therapy, he suffered ileal perforation and ileostomy was performed. S

BACKGROUND: Intestinal transplantation (ITx) is a life-saving procedure for patients with intestinal failure. The poorer outcome of ITx than of other organ transplantation, however, warrants more clinical and basic research on ITx. Herein, we developed a modified Paul-Mikulicz ileostomy procedure in a swine model of ITx, and investigated its feasibility for obtaining intestinal samples of both the graft and the recipient.
MATERIALS AND METHODS: We performed ITx in 10 pairs of piglets, each weighing 15-20 kg. Procurement included an isolated segment of the small bowel, constituting a model of

BACKGROUND & AIMS: The gut-liver axis is believed to be crucial in the pathogenesis of primary sclerosing cholangitis (PSC). However, the impact of colectomy on liver disease progression is unclear. Our study estimated the effect of colectomy on PSC progression with correction for time dependency and established risk factors by pooling data from several cohorts across different countries.
METHODS: We analysed data from the International PSC Registry (IPSCR), comprising patients from Finland, The Netherlands, Norway, and Sweden. Primary endpoint was defined as liver transplantation (LT) or PSC

Ascariasis, caused by Ascaris lumbricoides, is the most common helminthic infection worldwide, mainly in developing countries. Although intestinal obstruction is its most frequent complication, intestinal perforation with peritonitis is rare and life-threatening. A 45-year-old woman from a rural area presented with cramping abdominal pain, intermittent fever, and asthenia for 3 months. Emergency laparotomy revealed generalized peritonitis due to ileal perforation, managed with ileal resection and ileostomy. Postoperative complications included respiratory failure, septic shock, and three reope

A high volume of ileostomy output in patients with extensive bowel resection can be hard to manage. This leads to extensive loss of fluids and electrolytes along with malabsorption. Medications have traditionally controlled it by delaying intestinal transit and decreasing intestinal and gastric secretion using opiates, loperamide, diphenoxylate, omeprazole, somatostatin, and octreotide. However, many patients depend on parenteral nutrition and fluid and electrolyte infusions, even with optimal drug therapy. Despite the best possible care, they may develop renal failure. Teduglutide is a glucag

RATIONALE: Venous thromboembolism is one of the common complications of inflammatory bowel disease (IBD), which is a significant health problem worldwide. Factors such as inflammation, medications, and abdominal surgery, contribute to the increased risk of venous thromboembolism in patients with IBD. Cavernous transformation of the portal vein (CTPV) is a relatively rare complications of IBD. Subsequent portal hypertension could be fatal and the clinical treatment is difficult. Thus, early prevention is very crucial.

OBJECTIVES: Pediatric functional constipation (FC) may require invasive evaluations [like colon manometry (CM)] and surgical interventions [including diverting ostomy (DO)]. We evaluated the utility of CM in guiding surgery after DO.
METHODS: Children with medically refractory FC undergoing an ostomy were included. Institutional Review Board approval was obtained for this retrospective study. Demographics and CM variables [high amplitude propagating contractions (HAPCs)] were recorded. Outcome measures: response to ostomy closure defined as successful if no need for further surgery after osto
