Hypertriglyceridaemia is a well-documented cause of pancreatitis. Although rare, pancreatitis can result in life-threatening complications such as atypical haemolytic uremic syndrome (aHUS) and secondary haemophagocytic lymphohistiocytosis (sHLH). Non-traumatic small bowel perforation is rare in adults but carries a high morbidity and mortality. The diagnosis is made on clinical suspicion, and the most common causes in developing countries are infectious diseases, being cytomegalovirus infection in immunocompromised patients the main etiology. In primary biliary cirrhosis, the primary immune response to the bacteriophage phi X 174 is normal but the secondary response is significantly reduced. The reduction is primarily of IgG antibody, while IgM is proportionately less affected. These changes may be the result of a reduction in helper T lymphocyte function and may contribute to the increase in the ratio of serum concentration of IgM to IgG.
