An ontology of randomized controlled trials for evidence-based practice: content specification and evaluation using the competency decomposition method.

Randomized controlled trials (RCTs) are one of the least biased sources of clinical research evidence, and are therefore a critical resource for the practice of evidence-based medicine. With over 10,000 new RCTs indexed in Medline each year, knowledge systems are needed to help clinicians translate evidence into practice. Common ontologies for RCTs and other domains would facilitate the development of these knowledge systems. However, no standard method exists for developing domain ontologies. In this paper, we describe a new systematic approach to specifying and evaluating the conceptual cont

Accuracy of Retrieval-Augmented Large Language Model-Generated Preconsult Summaries in Breast Surgical Oncology.

PURPOSE: Radiology and pathology reports are important in breast surgical oncology planning, but are often unstructured and variable, requiring time-intensive previsit review and preparation. Here we evaluate the accuracy, completeness, and safety of retrieval-augmented large language model (LLM)-generated structured preconsult summaries compared with clinical staff-authored summaries. METHODS: In this single-institution retrospective validation study, 200 randomly sampled new breast surgical oncology consultations were included. LLM summaries were generated and compared with human summaries

Hospital Length of Stay and Surgery among European Children with Rare Structural Congenital Anomalies-A Population-Based Data Linkage Study.

Little is known about morbidity for children with rare structural congenital anomalies. This European, population-based data-linkage cohort study analysed data on hospitalisations and surgical procedures for 5948 children born 1995-2014 with 18 rare structural congenital anomalies from nine EUROCAT registries in five countries. In the first year of life, the median length of stay (LOS) ranged from 3.5 days (anotia) to 53.8 days (atresia of bile ducts). Generally, children with gastrointestinal anomalies, bladder anomalies and Prune-Belly had the longest LOS. At ages 1-4, the median LOS per yea

Different Approaches to requesting Consent for Routine data linkage in Neonatal follow-up (ACORN): protocol for a 2×2 factorial randomised trial.

INTRODUCTION: Routinely collected data can be linked to research data to create a rich dataset and inform practice. However, consent is normally required to link identifiable data. Reported rates of consent to data linkage for children ranged from 21% to 96%, but no studies have investigated different approaches to seeking consent for data linkage for school-age children. METHODS AND ANALYSIS: The Approaches to Consent for Routine Data Linkage in Neonatal Follow-up (ACORN) trial is a 2×2 factorial randomised trial to assess whether, for children who participated in neonatal randomised trials

Replicating medication trend studies using ad hoc information extraction in a clinical data warehouse.

BACKGROUND: Medication trend studies show the changes of medication over the years and may be replicated using a clinical Data Warehouse (CDW). Even nowadays, a lot of the patient information, like medication data, in the EHR is stored in the format of free text. As the conventional approach of information extraction (IE) demands a high developmental effort, we used ad hoc IE instead. This technique queries information and extracts it on the fly from texts contained in the CDW. METHODS: We present a generalizable approach of ad hoc IE for pharmacotherapy (medications and their daily dosage) p

Integration of Large-Scale Genomic Data Sources With Evolutionary History Reveals Novel Genetic Loci for Congenital Heart Disease.

BACKGROUND: Most cases of congenital heart disease (CHD) are sporadic and nonsyndromic, with poorly understood etiology. Rare genetic variants have been found to affect the risk of sporadic, nonsyndromic CHD, but individual studies to date are of only moderate sizes, and none to date has incorporated the ohnolog status of candidate genes in the analysis. Ohnologs are genes retained from ancestral whole-genome duplications during evolution; multiple lines of evidence suggest ohnologs are overrepresented among dosage-sensitive genes. We integrated large-scale data on rare variants with evolution

Assessing Information Congruence of Documented Cardiovascular Disease between Electronic Dental and Medical Records.

Dentists are more often treating patients with Cardiovascular Diseases (CVD) in their clinics; therefore, dentists may need to alter treatment plans in the presence of CVD. However, it's unclear to what extent patient-reported CVD information is accurately captured in Electronic Dental Records (EDRs). In this pilot study, we aimed to measure the reliability of patient-reported CVD conditions in EDRs. We assessed information congruence by comparing patients' self-reported dental histories to their original diagnosis assigned by their medical providers in the Electronic Medical Record (EMR). To

KaBOB: ontology-based semantic integration of biomedical databases.

BACKGROUND: The ability to query many independent biological databases using a common ontology-based semantic model would facilitate deeper integration and more effective utilization of these diverse and rapidly growing resources. Despite ongoing work moving toward shared data formats and linked identifiers, significant problems persist in semantic data integration in order to establish shared identity and shared meaning across heterogeneous biomedical data sources. RESULTS: We present five processes for semantic data integration that, when applied collectively, solve seven key problems. Thes

Optimal management of the critically ill: anaesthesia, monitoring, data capture, and point-of-care technological practices in ovine models of critical care.

Animal models of critical illness are vital in biomedical research. They provide possibilities for the investigation of pathophysiological processes that may not otherwise be possible in humans. In order to be clinically applicable, the model should simulate the critical care situation realistically, including anaesthesia, monitoring, sampling, utilising appropriate personnel skill mix, and therapeutic interventions. There are limited data documenting the constitution of ideal technologically advanced large animal critical care practices and all the processes of the animal model. In this paper

An efficient fractal method for detection and diagnosis of breast masses in mammograms.

In this paper, we present an efficient fractal method for detection and diagnosis of mass lesion in mammogram which is one of the abnormalities in mammographic images. We used 110 images that were carefully selected by a radiologist, and their abnormalities were also confirmed by biopsy. These images included circumscribed benign, ill-defined, and spiculated malignant masses. Firstly, we discriminated lesions automatically using new fractal dimensions. The results which were examined by different types of breast density showed that the proposed method was able to yield quite satisfactory detec

Combining joint models for biomedical event extraction.

BACKGROUND: We explore techniques for performing model combination between the UMass and Stanford biomedical event extraction systems. Both sub-components address event extraction as a structured prediction problem, and use dual decomposition (UMass) and parsing algorithms (Stanford) to find the best scoring event structure. Our primary focus is on stacking where the predictions from the Stanford system are used as features in the UMass system. For comparison, we look at simpler model combination techniques such as intersection and union which require only the outputs from each system and comb

The dual role of implantable loop recorder in patients with potentially arrhythmic symptoms: a retrospective single-center study.

BACKGROUND: Unexplained and potentially arrhythmic symptoms often lead to electrophysiology referral for evaluation. Implantable loop recorder (ILR) correlation of the symptom to the rhythm can secure a definitive arrhythmic diagnosis after a standard, yet nondiagnostic workup. METHODS: This large single-center retrospective study sought to assess the role of ILR in the evaluation of potentially arrhythmic symptoms, both in terms of diagnosis of an arrhythmia as well as to rule out an arrhythmic cause. Clinical data, indications for ILR, interrogation reports, and further management strategie

An expectation-maximization method for spatio-temporal blind source separation using an AR-MOG source model.

In this paper, we develop a maximum-likelihood (ML) spatio-temporal blind source separation (BSS) algorithm, where the temporal dependencies are explained by assuming that each source is an autoregressive (AR) process and the distribution of the associated independent identically distributed (i.i.d.) innovations process is described using a mixture of Gaussians. Unlike most ML methods, the proposed algorithm takes into account both spatial and temporal information, optimization is performed using the expectation-maximization (EM) method, the source model is adapted to maximize the likelihood,

Distinct and convergent visual processing of high and low spatial frequency information in faces.

We tested for differential brain response to distinct spatial frequency (SF) components in faces. During a functional magnetic resonance imaging experiment, participants were presented with "hybrid" faces containing superimposed low and high SF information from different identities. We used a repetition paradigm where faces at either SF range were independently repeated or changed across consecutive trials. In addition, we manipulated which SF band was attended. Our results suggest that repetition and attention affected partly overlapping occipitotemporal regions but did not interact. Changes

Joint blind restoration and surface recovery in photometric stereo.

We address the problem of simultaneous estimation of scene structure and restoration of images from blurred photometric measurements. In photometric stereo, the structure of an object is determined by using a particular reflectance model (the image irradiance equation) without considering the blurring effect. What we show is that, given arbitrarily blurred observations of a static scene captured with a stationary camera under different illuminant directions, we still can obtain the structure represented by the surface gradients and the albedo and also perform a blind image restoration. The sur

Evaluating the medical literature in reproductive medicine: questions concerning prognosis.

To illustrate the recommended approach to questions concerning prognosis and the formulation of clinical recommendations, the principles of evidence-based medicine were applied to questions regarding the cardiovascular health risks associated with polycystic ovary syndrome (PCOS) and the effects of treatment on risk. A critical examination of the published literature reveals that the clinical diagnosis of PCOS does not, by itself, increase the risk of developing coronary heart disease (CHD) or adversely affect the prognosis in women with CHD. Rather, any increased risks can be attributed to ot
