A quasi-experimental study estimating the impact of long-lasting insecticidal nets with and without piperonyl butoxide on pregnancy outcomes.

BACKGROUND: Long-lasting insecticidal nets (LLINs) are the main vector control tool for pregnant women, but their efficacy may be compromised, in part, due to pyrethroid resistance. In 2017, the Ugandan Ministry of Health embedded a cluster randomized controlled trial into the national LLIN campaign, where a random subset of health subdistricts (HSDs) received LLINs treated with piperonyl butoxide (PBO), a chemical synergist known to partially restore pyrethroid sensitivity. Using data from a small, non-randomly selected subset of HSDs, this secondary analysis used quasi-experimental methods t

Rolling back malaria in Africa - challenges and opportunities to winning the elimination battle.

A high-level review was conducted of the literature pertaining to the challenges and opportunities for eliminating malaria on the African continent. Although malaria mortality and morbidity are on the decline, the disease remains one of public health importance. Africa has invariably borne the brunt of the disease, recording the highest number of cases and deaths. However, with greater emphasis being placed on the disease by the international community, partnerships have developed to boost malaria elimination efforts on the continent. One such initiative is the Roll Back Malaria (RBM) partners

Malaria.

Following unsuccessful eradication attempts there was a resurgence of malaria towards the end of the 20th century. Renewed control efforts using a range of improved tools, such as long-lasting insecticide-treated bednets and artemisinin-based combination therapies, have more than halved the global burden of disease, but it remains high with 445 000 deaths and more than 200 million cases in 2016. Pitfalls in individual patient management are delayed diagnosis and overzealous fluid resuscitation in severe malaria. Even in the absence of drug resistance, parasite recurrence can occur, owing to hi

Impact of Malaria Vector Control Interventions at the Beginning of a Malaria Elimination Stage in a Dominant Area of Anopheles anthropophagus, Hubei Province, China.

Three towns with similar socio-ecological characteristics, malaria morbidities, and populations were selected for this study to explore economic and effective malaria control measures. The sources of infection were controlled in each town. Impregnated mosquito nets with 2.5% deltamethrin (15 mg/m(2)) combined with residual spraying of 5% cypermethrin (25 mg/m(2)) was implemented in cattle and pig pens, as well as in crowded sites in Chenji, whereas the mosquito nets were treated with 2.5% deltamethrin only in Guanqiao Town. All the control measures implemented in Fengling (control town) were th

EMIRA: Ecologic Malaria Reduction for Africa--innovative tools for integrated malaria control.

BACKGROUND: Malaria control is based on early treatment of cases and on vector control. The current measures for malaria vector control in Africa are mainly based on long-lasting insecticide treated nets (LLINs) and to a much smaller extent on indoor residual spraying (IRS). A third pillar in the fight against the malaria vector, larval source management (LSM), has virtually not been used in Africa since the ban of DDT in the 1960s. Within the light of recent WHO recommendations for Bacillus thuringiensis israelensis (Bti) use against malaria and other vector species, larval source management

Community-wide distribution of long-lasting insecticidal nets can halt transmission of lymphatic filariasis in southeastern Nigeria.

Lymphatic filariasis (LF) in rural southeastern Nigeria is transmitted mainly by Anopheles spp. mosquitoes. Potential coinfection with Loa loa in this area has prevented use of ivermectin in the mass drug administration (MDA) strategy for LF elimination because of potential severe adverse L. loa-related reactions. This study determined if long-lasting insecticidal net (LLIN) distribution programs for malaria would interrupt LF transmission in such areas, without need for MDA. Monthly entomologic monitoring was conducted in sentinel villages before and after LLIN distribution to all households

Insecticidal bed nets and filariasis transmission in Papua New Guinea.

BACKGROUND: Global efforts to eliminate lymphatic filariasis are based on the annual mass administration of antifilarial drugs to reduce the microfilaria reservoir available to the mosquito vector. Insecticide-treated bed nets are being widely used in areas in which filariasis and malaria are coendemic. METHODS: We studied five villages in which five annual mass administrations of antifilarial drugs, which were completed in 1998, reduced the transmission of Wuchereria bancrofti, one of the nematodes that cause lymphatic filariasis. A total of 21,899 anopheles mosquitoes were collected for 26

Deltamethrin and permethrin residue on long-lasting insecticidal nets after 18 months of use in a visceral leishmaniasis-endemic area in Nepal.

The insecticide residue on two types of long-lasting insecticidal nets (LLIN), Olyset Net and PermaNet 2.0, used in a visceral leishmaniasis-endemic village in eastern Nepal was quantified using HPLC. After two washes during 18 months of use the mean insecticide residues on PermaNet 2.0 and Olyset Net were 53.5 mg/m(2) (97.3% of the target dose) of deltamethrin and 911.8 mg/m(2) (91.2% of the target dose) of permethrin, respectively. These residues were close to the insecticide loads specified by the manufacturers of the two LLINs. The use of LLINs has been postulated as an alternative or comp

Know your foe: lessons from the analysis of tsetse fly behaviour.

The emergence of new vector-borne diseases requires new methods of vector control. These diseases are often zoonoses associated with wilderness areas, and established methods of vector control used in domestic settings (e.g., indoor-residual spraying, insecticide-treated bednets) are therefore inappropriate. Similar difficulties are also emerging with the control of 'old' vector-borne diseases such as malaria. Understanding the host-finding behaviour of vectors assists the development and application of control methods and aids the understanding of epidemiology. Some general lessons are illust

Integrated housing quality and insecticide-treated net use: propensity score methods for childhood anemia prevention in The Gambia.

BACKGROUND: Childhood anemia remains a critical public health challenge in sub-Saharan Africa, with malaria being a major contributing factor to its burden. Although insecticide-treated nets (ITNs) and improved housing are established malaria prevention strategies, their combined associations with childhood anemia are poorly understood. We examined the independent and joint associations of housing quality and ITN use with childhood anemia in The Gambia, using propensity score-weighted methods. METHODS: We analyzed data from 4155 children aged 6-59 months in the Gambia Demographic and Health S

Comparative analysis of Egypt's malaria elimination strategies and implementation science: Pathways to achieve malaria-free status for other African countries.

BACKGROUND: Malaria elimination is a critical public health goal, particularly in Africa, where the disease disproportionately affects vulnerable populations. Egypt's success in achieving World Health Organization (WHO) malaria-free certification on October 20, 2024, through an evidence-based malaria elimination program, offers a valuable model for replication. Our study is identified as an implementation study, evaluating the evidence-based interventions deployed, the implementation strategy tested, and its outcomes to provide insights for scaling similar programs across Africa. Assessing the external validity of mobile phone surveys for monitoring ITN coverage indicators: a comparison with household surveys in Tanzania.

BACKGROUND: Insecticide-treated net (ITN) coverage indicators allow country malaria programmes to understand the overall coverage of their ITN distribution activities and can be used to forecast the need for additional ITN procurement and to plan future distribution campaigns. As a result, more frequent data collection could better guide programme strategies, particularly for those strategies operating on an annual framework; however, the high costs and infrequency of national-scale, household surveys limit their practicality for ongoing monitoring. Due to the rapid growth trend of mobile phon

Seasonal climate patterns and their impact on mosquito net use in Nigeria: evidence from repeated surveys.

Consistent use of insecticide-treated nets (ITNs) for malaria prevention has been a challenge undermining the intervention's effectiveness. Perceptions of heat and absence of mosquitoes are among the reasons cited for non-use of ITN in populations with access. We conducted repeated household surveys across seasons in Ondo State of Nigeria to investigate seasonal changes in ITN use rates and potential determinant factors. Five household surveys were conducted in different months before an ITN distribution campaign in December 2021 followed by post-campaign surveys in April 2022, and in June, Au

Asymptomatic Plasmodium falciparum malaria prevalence among adolescents and adults in Malawi, 2015-2016.

Malaria remains a significant cause of morbidity and mortality in Malawi, with an estimated 18-19% prevalence of Plasmodium falciparum in children 2-10 years in 2015-2016. While children report the highest rates of clinical disease, adults are thought to be an important reservoir to sustained transmission due to persistent asymptomatic infection. The 2015-2016 Malawi Demographic and Health Survey was a nationally representative household survey which collected dried blood spots from 15,125 asymptomatic individuals ages 15-54 between October 2015 and February 2016. We performed quantitative pol

Impact of insecticide treated mosquito nets and low dose monthly diethylcarbamazine on lymphatic filariasis infection between 1999 and 2004 in two endemic communities of north-eastern Tanzania.

Lymphatic filariasis (LF) is among the poverty related neglected tropical diseases earmarked for elimination using mass drug administration (MDA) strategy. Additional use of insecticide treated mosquito nets (ITNs) might enhance elimination of LF infection. Between August 1998 and July 1999, all individuals aged 8 months from Magoda and Mpapayu villages in north-eastern Tanzania, were administered with monthly low dose diethylcarbamazine (DEC) at a dosage of 50mg in children aged < 15 years and 100mg in adults aged ≥ 15 years. ITNs were also distributed to Magoda in December 1998 and to Mpapay

Characterization of Anopheles gambiae D7 salivary proteins as markers of human-mosquito bite contact.

BACKGROUND: Malaria is transmitted when infected Anopheles mosquitoes take a blood meal. During this process, the mosquitoes inject a cocktail of bioactive proteins that elicit antibody responses in humans and could be used as biomarkers of exposure to mosquito bites. This study evaluated the utility of IgG responses to members of the Anopheles gambiae D7 protein family as serological markers of human-vector contact. METHODS: The D7L2, D7r1, D7r2, D7r3, D7r4 and SG6 salivary proteins from An. gambiae were expressed as recombinant antigens in Escherichia coli. Antibody responses to the salivar

Use of insecticide-treated nets (ITN) and intermittent preventive treatment (IPTp) for malaria prevention and its associated factors among pregnant women in Sub-Sahara Africa.

BACKGROUND: Malaria is a significant public health issue in sub-Saharan Africa, accounting for 96% of global malaria-related deaths. Malaria during pregnancy can lead to maternal morbidity and death, stillbirths, and newborn deaths. Prevention and control policies in areas of stable transmission emphasize a combination of intermittent preventive treatment during pregnancy (IPTp) and use of insecticide-treated nets (ITNs), along with effective case management. Neither ITNs nor IPTp provide complete protection alone, and since IPTp and ITNs work synergistically, combining these interventions cou

Insecticide-treated nets distribution campaign: Physical integrity, usage and sustainable disposal of end-of-life insecticide-treated nets under operational settings in Odisha, India.

OBJECTIVES: Prior to 2017, Odisha accounted for 50% of all Plasmodium falciparum cases in India. The 'National Strategic Plan' for malaria elimination had distributed 11.3 million insecticidal treated nets (ITNs) to 23 million individuals in Odisha's 17 malaria-endemic districts by 2017. In 2021, the National Centre for Vector-Borne Disease Control replaced end-of-life ITNs. India needs official regulations on end-of-life ITNs collection and disposal mechanisms. A pilot study was undertaken to understand community practices and perceptions on end-of-life ITNs. METHODS: The 2021 study was cond

RTS,S/AS01E malaria vaccine (MoSQUIRIX*) Children living in malaria-endemic regions: little efficacy, poorly documented harms.

Malaria remains a major public health problem in most tropical coun- tries. Plasmodium falciparum infection can be life-threatening, especially in children. Insecticide-treated bed nets have been shown to reduce deaths due to malaria among young children. A malaria vaccine (RTS,S/AS01E) containing two adjuvants has been assessed for its ability to prevent P. falciparum malaria among young children living in endemic areas. The clinical data have been analysed by the European Medicines Agency (EMA) in conjunction with the World Health Organization (WHO). Efficacy has been evaluated in sub-Sahara
