[Stepwise occupational reintegration after orthopedic rehabilitation--participants, realization, effectiveness, and optimization requirements].

This paper presents research results on stepwise occupational reintegration in patients with orthopaedic diseases, predominantly back pain. A retrospective questionnaire study was performed from January to May 2003. We questioned 534 insurants of the LVA (statutory pension insurance institute) in Schleswig-Holstein who had completed orthopaedic rehabilitation between 1999 and 2001 and had been recommended for stepwise occupational reintegration. The participant rate was 47% (n = 250 completed questionnaires). The standardized survey included demographic as well as illness-, rehabilitation- and

Welfare Regimes and Out-of-Pocket Healthcare Spending in the Last Year of Life Among Cancer Decedents in Europe and Israel.

Background/Objectives: Out-of-pocket (OOP) healthcare spending can remain substantial near the end of life even in countries with broad health coverage, yet less is known about whether these expenditures follow broader welfare-regime patterns. This study examined OOP healthcare expenditure among cancer decedents and assessed whether welfare-regime differences persisted after adjustment for individual demographic and socioeconomic characteristics. Methods: We analyzed Survey of Health, Ageing and Retirement in Europe (SHARE) End-of-Life data for 1950 cancer decedents aged 50 years and older fro

Quality of life and functioning of Hispanic patients with Major Depressive Disorder before and after treatment.

BACKGROUND: Similar rates of remission from Major Depressive Disorder (MDD) have been documented between ethnic groups in response to antidepressant treatment. However, ethnic differences in functional outcomes, including patient-reported quality of life (QOL) and functioning, have not been well-characterized. We compared symptomatic and functional outcomes of antidepressant treatment in Hispanic and non-Hispanic patients with MDD. METHODS: We analyzed 2280 nonpsychotic treatment-seeking adults with MDD who received citalopram monotherapy in Level 1 of the Sequenced Treatment Alternatives to

Does Volunteering Give an Extra Health Boost? Volunteering and High Blood Pressure Among Insured and Uninsured U.S. Adults in Late Midlife.

INTRODUCTION: High blood pressure (HBP) is a major predictor of cardiovascular disease, a leading cause of death in the U.S. Growing evidence suggests that formal volunteering in later life is associated with a lower prevalence of HBP; however, it remains unclear whether these benefits extend to adults in late midlife and whether they differ by health insurance status. METHODS: This prospective cohort study analyzed data from the 2012-2018 Health and Retirement Study. The analytic sample included 4,169 respondents between the ages of 50 and 64. Because blood pressure was measured in half the

Health Outcomes, Healthcare Resource Use, and Economic Burden of Invasive Meningococcal Disease in the United States Among Children Aged <11 Years.

BACKGROUND/OBJECTIVE: Although uncommon, invasive meningococcal disease (IMD) is a serious and life-threatening illness. In the United States (US), incidence is highest among infants aged <1 year, but research characterizing pediatric IMD burden is limited. This study aimed to evaluate health outcomes and economic burden (healthcare resource utilization [HCRU] and costs [associated with health outcomes and HCRU]) in US individuals aged <11 years with IMD and without IMD, and their caregivers, from the patient and payer (commercial and Medicaid insurance) perspective. METHODS: Retrospective cl

Cardiological rehabilitation, prehabilitation, and cardiovascular prevention in adults with congenital heart defects: tasks and services of the German Pension Insurance-part 1: preventive cardiology and prehabilitation.

Congenital heart defects (CHD) are the most common inborn cardiac anomalies, with approximately 1.35 million children born each year worldwide. Advances in medical treatment over recent decades have reduced mortality, yet morbidity remains high. Many patients now survive into adulthood but continue to have chronic heart disease and often develop complications such as heart failure, arrhythmias, pulmonary hypertension, and acquired cardiac and non-cardiac comorbidities, all of which require ongoing specialized care. Additionally, many adults with CHD (ACHD) lead a sedentary lifestyle, are overw

How is and how should healthcare for people with multiple sclerosis in Germany be designed?-The rationale and protocol for the mixed-methods study Multiple Sclerosis-Patient-Oriented Care in Lower Saxony (MS-PoV).

BackgroundMultiple sclerosis (MS) is the most common autoimmune inflammatory disease of the central nervous system in Europe, often causing severe physical, cognitive and emotional impairments. Currently, it is unclear whether the healthcare provisions of people with MS (PwMS) are in line with the recommendations for treatment based on guidelines or patients' needs. The main objectives of the study are as follows: (a) to investigate how well PwMS are treated; and (b) to develop a needs-oriented, patient-centred care model.MethodsThis mixed-methods study focuses on adult PwMS living in Lower Sa

Long-term impact of abusive head trauma in young children.

OBJECTIVE: Abusive head trauma is the leading cause of physical abuse deaths in children under the age of 5 and is associated with severe long-lasting health problems and developmental disabilities. This study evaluates the long-term impact of AHT and identifies factors associated with poor long-term outcomes (LTOs). METHODS: We used the Truven Health MarketScan Research Claims Database (2000-2015) to identify children diagnosed with AHT and follow them up until they turn 5. We identified the incidence of behavioral disorders, communication deficits, developmental delays, epilepsy, learning d

Fecal continence outcomes and potential disparities for patients with anorectal malformations treated at referral institutions for pediatric colorectal surgery.

PURPOSE: Fecal incontinence is a problem for many patients born with an anorectal malformation (ARM) that can impact quality of life. It is unknown if racial, ethnic, and socioeconomic disparities relate to fecal continence in these children. We sought to examine outcomes and potential disparities in care. METHODS: We performed a multicenter retrospective study of children > 3y with ARM evaluated at sites participating in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC). The primary outcome was fecal continence. We evaluated for associations between fecal continence and race, s

Beyond the Bedside: Addressing Structural Gaps in the Care of Undocumented, Seriously Ill Patients.

Undocumented patients face structural constraints to accessing life-sustaining treatments in the subacute setting due to gaps in health care coverage. We present the case of a 38-year-old Spanish-speaking woman with short bowel syndrome, dependent on prolonged parenteral nutrition after critical illness, who faced additional challenges as an undocumented and uninsured patient. Despite medical stabilization and a consistent desire to pursue life-prolonging care, discharge planning was complicated by a health care system not designed to support individuals without legal status or insurance. Home-visit nursing service use by older adults in Japan using insurance claims data.

This study aimed to investigate the proportion of home-visit nursing (HVN) used, the timing of initiating these services, and the medical conditions of older adults in the last two years of life based on their place of death. The study used insurance claims data of people aged 75 or older from 32 Japanese municipalities who passed away between March 2017 and September 2018. Of the 15,124 older adults analyzed, the results revealed that 14 % died at home, and among them, 68 % had used HVN services. In the home or non-home groups, the most common timing for initiating the services was in the las

Facial Feminization Surgery: The Ethics of Gatekeeping in Transgender Health.

The lack of access to gender-affirming surgery represents a significant unmet health care need within the transgender community, frequently resulting in depression and self-destructive behavior. While some transgender people may have access to gender reassignment surgery (GRS), an overwhelming majority cannot afford facial feminization surgery (FFS). The former may be covered as a "medical necessity," but FFS is considered "cosmetic" and excluded from insurance coverage. This demarcation between "necessity" and "cosmetic" in transgender health care based on specific body parts is in direct opp

Current status and challenges of biologic targeted therapy for myasthenia gravis in China.

Myasthenia gravis (MG) is an autoimmune neuromuscular disorder posing substantial disease burden in China, with significant impacts on patient quality of life. While conventional immunosuppressants remain fundamental therapy, biologic targeted agents-including B-cell targeting drugs, complement C5 inhibitors, FcRn antagonists, and IL-6R inhibitors-have brought major advances, especially for refractory and MuSK-antibody-positive MG. Increasing availability in China has demonstrated promising clinical outcomes, yet substantial barriers remain. Challenges include high drug costs, limited insuranc

The cancer survivor.

Patients who have completed therapy for malignant disease face many problems, including the possibility and persistent fear of recurrent disease or a new malignancy. The side effects of therapy, including surgical scars, radiation effects or damage to physiological function may strongly influence patients' complete recovery. Depletion of financial resources or a lengthy interruption of employment may present serious difficulties, particularly if patients cannot function as well as before. They may also find it difficult to buy life insurance, or mistakenly believe that they are not eligible to

Estimates of healthcare spending for preterm and low-birthweight infants in a commercially insured population: 2008-2016.

The growth in healthcare spending is an important topic in the United States, and preterm and low-birthweight infants have some of the highest healthcare expenditures of any patient population. We performed a retrospective cohort study of spending in this population using a large, national claims database of commercially insured individuals. A total of 763,566 infants with insurance coverage through Aetna, Inc. for the first 6 months of post-natal life were included, and received approximately $8.4 billion (2016 USD) in healthcare services. Infants with billing codes indicating preterm status
