Innovations in the Treatment of Antibiotic-Resistant Infections in Internal Medicine.

Antimicrobial resistance (AMR) is one of the major contemporary challenges in internal medicine, impairing the effectiveness of empirical antibiotic regimens and increasing mortality, hospital length of stay, and healthcare costs. The rising prevalence of multidrug-resistant Gram-negative organisms calls for a clinical response grounded in therapeutic innovation, antimicrobial rationalisation, and institutional reorganisation. We conducted a narrative review of the literature from 2020 to 2025, integrating recommendations from international guidelines, including the Infectious Diseases Society

Diabetic foot infections in Internal Medicine services in Spain (2018-2022).

INTRODUCTION: Diabetic foot infections represent a common and serious complication of diabetes mellitus, with a wide range of clinical presentations. Despite their significance, uncertainties persist regarding their management and impact on Internal Medicine services. MATERIALS AND METHODS: A retrospective cohort study was conducted using data from the Registry of Specialized Healthcare Activity (RAE-CMBD) over a five-year period (2018-22). Patients discharged from Internal Medicine with a diagnosis of diabetic foot were included. Demographic, clinical, and activity data were collected, and t

Standardizing Point-of-Care Ultrasound (POCUS) Credentialing in Internal Medicine Training.

Point-of-care ultrasound (POCUS) is increasingly recognized as a valuable tool in internal medicine for enhancing physical examination and improving diagnostic accuracy. Growing evidence shows that integrating POCUS into routine care improves diagnostic accuracy, shortens time to diagnosis, increases resident confidence, and enhances patient safety by reducing procedure-related complications. However, POCUS training remains inconsistent across internal medicine residency programs, with no national requirement for education or competency - unlike emergency medicine, which has established minimu

A Curriculum on Advanced Topics in Hypertension for Internal Medicine Residents.

OBJECTIVES: Hypertension (HTN) affects nearly half of US adults. Our multi-institutional survey revealed that Internal Medicine residents lack proficiency in advanced HTN topics. We developed a curriculum to address knowledge gaps in these topics and aimed to assess the effects of the curriculum on residents' confidence, desire for future training, and knowledge in advanced HTN topics. METHODS: HTN experts taught four advanced topics in HTN: conducting a workup for secondary HTN, managing HTN in chronic kidney disease, managing HTN in patients who are or may become pregnant, and managing hype

Albumin administration in internal medicine: A journey between effectiveness and futility.

Albumin is the most abundant circulating protein and provides about 70% of the plasma oncotic power. The molecule also carries many other biological functions (binding, transport and detoxification of endogenous and exogenous compounds, antioxidation, and modulation of inflammatory and immune responses). Hypoalbuminemia is a frequent finding in many diseases, representing usually only a biomarker of poor prognosis rather than a primary pathophysiological event. Despite that, albumin is prescribed in many conditions based on the assumption that correction of hypoalbuminemia would lead to clinic

Extremely Old Patients Hospitalized in Internal Medicine: What about Their Anemia?

In western countries, about half of the hospitalized patients are anemic. Generally, these patients are old, often with multiple diseases, and anemia worsens the prognosis, finally increasing the risk of death. We describe a monocentric observational study that evaluates 249 consecutive adult patients (160 women and 89 men) with anemia admitted in the internal medicine department over five months. They represent 71.5% of all patients admitted in the study period. Demographic, historical, and clinical data, laboratory tests, duration of hospitalization, readmission at 30 days, and death were re

Hospital readmissions in Internal Medicine Specialty: Frequency, associated factors and outcomes.

OBJECTIVES: Hospital readmission has become a focus of national attention as a potential indicator of healthcare quality and has a significant financial impact on healthcare system. Limited data is available regarding readmissions to Internal Medicine specialty from our sub-continent. It is, therefore, essential to determine the frequency and factors leading to readmissions, in order to avoid preventable readmissions and improve quality of healthcare provision. METHODS: This retrospective study reviewed adult discharges from Internal Medicine specialty between October 2018 and February 2019 a

[Albumin substitution : A review of the indications in general internal medicine].

Albumin has multiple physiological functions such as maintenance of oncotic pressure, binding of plasmatic molecule, immunomodulation and endothelial stabilization. Considering potential negative effects of hypoalbuminemia, the question of albumin substitution arises. For cirrhotic patients, indications for substitution are well established with a scientific consensus for spontaneous bacterial peritonitis, paracentesis-induced circulatory dysfunction and hepatorenal syndrome. However, in the case of hepatic encephalopathy or of hypoalbuminemia in non-cirrhotic patients, the scientific consensu

New cephalosporins for the treatment of pneumonia in internal medicine wards.

The burden of hospital admission for pneumonia in internal medicine wards may not be underestimated; otherwise, cases of pneumonia are a frequent indication for antimicrobial prescriptions. Community- and hospital-acquired pneumonia are characterized by high healthcare costs, morbidity and non-negligible rates of fatality. The overcoming prevalence of resistant gram-negative and positive bacteria (e.g., methicillin-resistant Staphylococcus aureus, penicillin and ceftriaxone-resistant Streptococcus pneumoniae, extended-spectrum β-lactamases and carbapenemases producing Enterobacteriaceae) has m

Optimal Management of Cirrhotic Ascites: A Review for Internal Medicine Physicians.

Clinical history of liver cirrhosis is characterised by two phases: the asymptomatic phase, also termed 'compensated cirrhosis', and the phase of complications due to the development of portal hypertension and liver dysfunction, also termed 'decompensated cirrhosis', in which patients may develop ascites, the most frequent and clinically relevant complication of liver cirrhosis. Ascites can be classified into uncomplicated and complicated according to the development of refractoriness, spontaneous bacterial peritonitis (SBP) or the association with hepatorenal syndrome (HRS). In this narrative

[Reducing urinary catheter use in patients hospitalized in internal medicine departments].

Clinical utility exists in certain situations for introducing a urinary catheter, but its use is the major cause of in-hospitaL acquired urinary tract infections. Furthermore, there are other complications of urinary catheterization, including urethral injury, macroscopic hematuria, and the inability to remove the catheter once introduced. Also, the in-hospital use of an indwelling urinary catheter in the elderly patient is associated with prolonged hospitalizations and an increased risk of in-hospital mortality. Although there are clinical criteria for the use of an indwelling urinary cathete

Childhood trauma and sexual behavior in adulthood among internal medicine outpatients.

Abuse in childhood may be associated with a variety of sexual behaviors in adulthood. However, previous studies have been limited by population type, number of traumas inquired about, and number of sexual behaviors explored. In this study, we examined five forms of childhood abuse or trauma (i.e. sexual abuse, physical abuse, emotional abuse, physical neglect, and the witnessing of violence) and 13 sexuality variables (e.g., age at first intercourse, number of different sexual partners) among 76 women in an outpatient resident-provider internal medicine setting. Participants who had experience
