In no other area of veterinary surgery is the selection and performance of the appropriate surgical approach more important than neurosurgery. An adequate, atraumatic, and anatomically sound exposure is an important part of the battle in most orthopaedic procedures and is an even more important consideration when related to neurosurgery. The delicate and often unforgiving nature of the tissue involved dictates that careful planning and attention to detail become high priority concerns for the veterinary neurosurgeon. Most veterinarians recommend surgery for dogs with severe impairment, recurre

A light microscopic investigation of the anulus fibrosus in cervical intervertebral discs of the dog was conducted to ascertain the arrangement and distribution of elastic fibers. Elastic fibers were observed in all lamellae of the anulus fibrosus. However, collagenous fibers were the predominant type of connective tissue fiber, and elastic fibers were randomly dispersed among them. Intralamellar (collagenous and elastic) fibers were vertically and obliquely oriented in both superficial and deep lamellae of the anulus fibrosus. All intralamellar fibers were densely and regularly arranged in su

In an epidemiological study of acute herniated lumbar intervertebral discs in the New Haven, Connecticut (U.S.A.), area, it was found that this condition was most likely to be diagnosed among persons in the age group 30-39 years, and that the most important risk factors among the variable considered in this study were driving of motor vehicles at or away from work, sedentary occupations, suburban residence, and previous full-term pregnancies. Variables for which there was some suggestion of an association but for which the evidence was inconclusive were the male sex, high social class among fe

BACKGROUND: Disc height (DH) change is considered one of the most critical factors in assessing intervertebral disc degeneration (IVD). Pfirrmann et al. developed a scoring system for disc degeneration evaluation based on changes in DH in magnetic resonance imaging (MRI). While the relationship between DH measurements and Pfirrmann scores for disc degeneration has been explored, the validity of different DH measuring techniques or their connection with disc degeneration is yet uncertain. The present study investigates intra-rater and inter-rater agreement and reliability of different DH measur

The most common reason that adults in the United States see their physician is lower back or neck pain secondary to degenerative disc disease. To date, approaches to treat degenerative disc disease are confined to purely mechanical devices designed to either eliminate or enable flexibility of the diseased motion segment. Tissue engineered intervertebral discs (TE-IVDs) have been proposed as an alternative approach and have shown promise in replacing native IVD in the rodent tail spine. Here we demonstrate the efficacy of our TE-IVDs in the canine cervical spine. TE-IVD components were construc

The intervertebral disc (IVD) is defined by a uniquely avascular niche characterized by constitutive hypoxia, limited nutrient diffusion, acidic pH, hyperosmolarity, and repetitive mechanical loading. These stressors interact with each other rather than acting in isolation. Reduced endplate transport exacerbates hypoxia and glucose deprivation, driving glycolytic lactate accumulation and acidification. In parallel, acid-osmotic stress perturbs ion homeostasis and mitochondrial membrane potential, while mechanical loading promotes microdamage and inflammatory mediator release. Together they con

We describe a surgical technique for a vertebral column autograft using the intervertebral disc for cervical disc disease for patients whose major problem is not spinal instability. Of a total of 41 patients with cervical disc disease suffering from cervical spondylotic radiculomyelopathy, 33 patients were operated on at one level and 8 patients were operated on at two levels. Postoperative X-ray film showed some movement at the "operated" disc level in all patients (average postoperative follow-up period was 43 months, range two years to 5 years). A significant decrease in motion in the exten

Chronic low back pain due to intervertebral disc (IVD) degeneration (IVDD) is a major global health burden. Interactions between IVD tissues and surrounding structures are important for spinal health and pathology, yet many studies focus on structures within the IVD and neglect a deeper investigation of adjacent tissues. This study describes a newly identified intervertebral fat pad (IVFP) in rat lumbar spines, its changes following IVD injury, and similar structures in mice and humans. IVFPs were analyzed histologically using naive and injured IVDs from a rat model of IVDD, in which 5-month-o

A 5-year-old male neutered miniature dachshund presented with a 24-hour history of nonambulatory pelvic limb paraparesis. The dog was diagnosed with grade III myelopathy, Th3-L3 neurolocalisation. Computed tomography (CT) was consistent with intervertebral disc extrusion between the second and third lumbar vertebrae (L2/3) on the left side. Left-sided hemilaminectomy was performed to remove the compressive intervertebral disc material. Intervertebral disc fenestration was performed at L2/3, prophylactic intervertebral disc fenestration was also performed at L3/4. After surgery, persistent spin

OBJECTIVE: During degeneration of the intervertebral disc ingrowth of blood vessels and nerves into the disc are associated with back pain. Vascular endothelial growth factors promote vasculogenesis by binding to the membrane vascular endothelial growth factor receptor 1, while shorter soluble forms of this receptor can inhibit vascularization. We hypothesized that membrane and soluble receptor forms might change between stages of intervertebral disc degeneration.
RESULTS: Expression of soluble and membrane forms of vascular endothelial growth factor receptor 1 in human degenerated interverte

ObjectiveAs therapeutic options are rapidly expanding to treat pain of discogenic and vertebrogenic origin, it is important for the clinician to have an evidence-based understanding of the latest information regarding the normal and pathological anatomy of the intervertebral disc as well as imaging characteristics and treatment strategies. The American Society of Pain and Neuroscience (ASPN) commissioned the DISC project to inform and update the interventional spine community about anterior column pain syndromes.MethodsUsing established literature review methods a clinical oversight group was

Back pain is a common and debilitating disorder with largely unknown underlying biology. Here we report a genome-wide association study of back pain using diagnoses assigned in clinical practice; dorsalgia (119,100 cases, 909,847 controls) and intervertebral disc disorder (IDD) (58,854 cases, 922,958 controls). We identify 41 variants at 33 loci. The most significant association (ORIDD = 0.92, P = 1.6 × 10-39; ORdorsalgia = 0.92, P = 7.2 × 10-15) is with a 3'UTR variant (rs1871452-T) in CHST3, encoding a sulfotransferase enzyme expressed in intervertebral discs. The largest effects on IDD are

BACKGROUND: Inherent complications associated with surgery and limited success of percutaneous minimally invasive procedures make researches wanting for an ideal treatment for cervical disc herniation.
OBJECTIVE: We prospectively study the role of ozone disc nucleolysis in cervical intervertebral disc herniation.
PATIENTS AND METHODS: From January 2008 to December 2020, we prospectively study 246 consecutive patients of cervical disc herniation treated by a single session of intradiscal injection of ozone-oxygen mixture (ozone disc nucleolysis). There were 55% of females and 45% of males. Ag

BACKGROUND: The lack of a comprehensive understanding regarding the pathological mechanisms of intervertebral disc degeneration (IDD) impedes the development of effective therapeutic strategies.
OBJECTIVES: To investigate the role of Twist1 in IDD and its interplay with glycolysis METHODS: Twist1 expression was evaluated in primary nucleus pulposus (NP) cells and clinical tissues. NP cell apoptosis and extracellular matrix (ECM) metabolism were detected to assess the impact of Twist1 on IDD, which was further demonstrated via in vivo assay. Glycolytic activity was estimated by measuring the e

BACKGROUND: Intervertebral disc (IVD) degeneration is associated with severe clinical symptoms including chronic back pain. Galectins are a family of carbohydrate-binding proteins, some of which can induce functional disease markers in IVD cells and other musculoskeletal tissues. Galectin-4 and -8 were shown to trigger disease-promoting activity in chondrocytes, but their effects on IVD cells have not been investigated yet.
METHODS: IVD specimens from 36 patients with spondylosis, spondylolisthesis, and scoliosis were assessed immunohistochemically for the presence of galectin-4 and -8. The d

Aging is a risk factor for several chronic conditions, including intervertebral disc degeneration and associated back pain. Disc pathologies include loss of reticular-shaped nucleus pulposus cells, disorganization of annulus fibrosus lamellae, reduced disc height, and increased disc bulging. Sonic hedgehog, cytokeratin 19, and extracellular matrix proteins are markers of healthy disc. Preclinical murine models help understand cellular and molecular mechanisms of disc pathologies and the associated neurological symptoms. Klotho (Kl) is a known anti-aging gene. Kl-deficient mice (KlKl/KlKl) have

An 18-year-old female patient presented with a 1-month history of low back pain, which had worsened and was accompanied by radiating pain in the right lower limb for half a month. She was admitted to our hospital with computed tomography and magnetic resonance imaging findings suggesting calcification of the L3/4 disc and a large intraspinal mass at the L2-4 level. The patient's symptoms did not improve with conservative treatment, and her muscle strength rapidly declined. Therefore, a right-sided L3 laminotomy was performed for decompression. Intraoperatively, a cystic intraspinal mass with c

BACKGROUND AND OBJECTIVES: Intervertebral disc degeneration (IVDD) is a leading cause of chronic low back pain, characterized by extracellular matrix (ECM) degradation, excessive inflammation activation, and increased cell apoptosis. LGR6, a receptor known for its role in tissue regeneration, has recently been implicated in modulating macrophage efferocytosis, a process critical for clearing apoptotic cells and maintaining tissue homeostasis. This study aimed to investigate the role of LGR6 in regulating IVDD progression and to focus on its impact on macrophage efferocytosis, ECM regulation, a
