Vitamin D status in children with intestinal failure who have achieved enteral autonomy.

BACKGROUND: Vitamin D deficiency is highly prevalent in children with intestinal failure (IF) who receive parenteral nutrition (PN), but data on vitamin D status after achieving enteral autonomy (EA) are limited. We aimed to evaluate the prevalence of vitamin D deficiency in this population while exploring clinical variables that may be associated with its development. METHODS: A retrospective review was performed on 29 children with IF who had achieved EA. Deficiency was defined as a mean serum 25-hydroxyvitamin D <30 ng/ml. DATA RESULTS: Sixty-six percent of children had at least one defic

Neonatal Intestinal Failure Is Independently Associated With Impaired Cognitive Development Later in Childhood.

OBJECTIVE: The impact of pediatric intestinal failure (IF) on neurodevelopment beyond infancy has not been systematically studied. Our aim was to evaluate cognitive and motor impairment and to identify risk factors for adverse outcomes among children with IF. METHODS: We conducted a cross-sectional single-center study at the Helsinki University Children's Hospital. Patients with IF with >60 days of parental nutrition (PN) dependency aged between 3 and 16 years (n = 40) were invited to participate. The cognitive and motor skills were evaluated using validated tests: Wechsler Preschool and Prim

Intestinal failure-associated liver disease (IFALD): insights into pathogenesis and advances in management.

Premature infants and children with intestinal failure (IF) or short bowel syndrome are susceptible to intestinal failure-associated liver disease (IFALD, previously referred to as parenteral nutrition-associated liver disease, or PNALD). IFALD in children is characterized by progressive cholestasis and biliary fibrosis, and steatohepatitis in adults, and is seen in individuals dependent upon prolonged administration of PN. Many factors have been proposed as contributing to the pathogenesis of IFALD. In recent years, the focus has been on the potential synergistic roles of the intestinal micro

Clinical Application of Prophylactic Ethanol Lock Therapy in Pediatric Patients With Intestinal Failure.

Patients with intestinal failure have an increased risk for catheter-related bloodstream infections that can necessitate central venous line replacement and result in morbidity, prolonged hospitalization, or mortality. For pediatric patients with intestinal failure, the severe loss of intestinal absorptive ability leads to reliance on parenteral nutrition to meet minimal needs required for growth and development. Reliance on parenteral nutrition, in turn, forces dependency on central venous lines. Recent research concentrating on the pediatric population with intestinal failure indicates that

Intestinal failure and home parenteral nutrition: implications for oral health and dental care.

BACKGROUND & AIMS: Background disease processes, medication and therapies in people with intestinal failure receiving home parenteral nutrition may affect their oral health. To inform oral health advice for this group a study of their oral health status was carried out. METHODS: Fifty-two HPN outpatients recruited from specialised nutrition clinics at a national referral centre listed their medical and medication history, perceived oral health and dental treatment experience in a structured interview and underwent an oral health examination. Findings were compared with 2009 UK Adult Dental He

Hirschsprung's disease: a possible cause of anastomotic failure following repair of intestinal atresia.

Despite several reported cases, the association of Hirschsprung's disease and intestinal atresia is not widely recognized. We describe three patients with jejunoileal atresia who all had a preoperative barium enema that failed to show a transition zone. All three patients developed an anastomotic leak of the atresia repair, and required a temporary diverting enterostomy. Two patients had total colonic Hirschsprung's disease and in one, the transition zone was in the midtransverse colon. Histological examination of the appendix at the time of repair may be helpful in patients with small intesti

A multicentric observational retrospective study on patients with short bowel syndrome and chronic intestinal failure who underwent intestinal transplantation in Brazil.

INTRODUCTION: Short Bowel Syndrome (SBS) is a rare gastrointestinal disorder associated with Intestinal Failure (SBS-IF) that leads to morbidity, mortality, and a burden on healthcare costs. Intestine Transplantation (IT) is a treatment option for patients with SBS-IF as it replaces the missing or diseased intestine and offers the potential for return to normal activities and intestinal function. This study aims to describe the clinical course and demographical and clinical characteristics of subjects with SBS-IF who underwent IT in Brazil. METHODS: This retrospective observational study incl

Clinical approach to the management of Intestinal Failure Associated Liver Disease (IFALD) in adults: A position paper from the Home Artificial Nutrition and Chronic Intestinal Failure Special Interest Group of ESPEN.

We recommend that intestinal failure associated liver disease (IFALD) should be diagnosed by the presence of abnormal liver function tests and/or evidence of radiological and/or histological liver abnormalities occurring in an individual with IF, in the absence of another primary parenchymal liver pathology (e.g. viral or autoimmune hepatitis), other hepatotoxic factors (e.g. alcohol/medication) or biliary obstruction. The presence or absence of sepsis should be noted, along with the duration of PN administration. Abnormal liver histology is not mandatory for a diagnosis of IFALD and the decis

Vitamin D and infection risk in patients with short bowel syndrome and intestinal failure.

PURPOSE: Patients with short bowel syndrome (SBS) and intestinal failure (IF) are highly susceptible to micronutrient deficiencies and recurrent infections. Vitamin D plays an important role in immune modulation and has been linked to infection outcomes in other high-risk populations, yet its relevance in SBS/IF remains unclear. This study evaluated the effectiveness of vitamin D supplementation and the association between serum 25-hydroxyvitamin D [25(OH)D] levels and infection risk in SBS/IF patients. METHOD: In this single-center retrospective cohort study, 23 SBS/IF patients (8 pediatric,

Stomal Hyperflow and Intestinal Failure: A Case Report on the Use of a Chyme Reinstillation Device.

Stomal hyperflow, a frequent complication of ileostomy, is associated with high morbidity and mortality, particularly when linked to intestinal failure. This situation, especially when conventional treatments fail, often results in a poor prognosis for the patient. In this case report, we present the history of a patient suffering from stomal hyperflow with intestinal failure, in the context of short small bowel syndrome following multifocal venous mesenteric ischaemia. In a critical condition at the time of admission to our hospital, he was able to benefit from The Insides® System, a chyme re

A case of yogurt central line-associated bloodstream infection in a child with intestinal failure.

Central line-associated bloodstream infections (CLABSI) are a significant cause of morbidity in children with intestinal failure (IF). Probiotic therapy is discouraged in patients with IF because of the increased risk of bacteremia with the probiotic organism. We report an unusual, previously undescribed, clinical presentation of a Lactobacillus-species CLABSI linked to yogurt consumption in a toddler with IF secondary to Megacystis-microcolon-intestinal hypoperistalsis syndrome. Lactobacillus is abundant in fermented foods like yogurt and among the commensal constituents of the oral cavity mi

A comparison of catheter lock solutions and associated complications in pediatric intestinal failure patients.

BACKGROUND: Children with intestinal failure (IF) receiving parenteral nutrition (PN) are at risk for central venous catheter (CVC) associated complications including infection, occlusion, and breakage. A variety of catheter locks have been used to preserve patency and prevent infection when catheters are not in use. Taurolidine (T) and Ethylenediaminetetraacetic acid (EDTA) locks have antimicrobial and anticoagulant properties; however, superiority of one is unclear. MATERIAL AND METHODS: A multicenter, retrospective review of children with IF on PN and receiving care from an Intestinal Reha

Effects of Beraprost on Intestinal Microcirculation and Barrier Function in a Mouse Model of Renal Failure.

OBJECTIVE: Endothelial dysfunction plays an important role in the pathogenesis of chronic kidney disease. Prostacyclin (PGI2), an endothelial cell-produced endogenous prostaglandin, plays a crucial role in maintaining endothelial function. However, its effects on intestinal microcirculation and barrier function are not fully understood. We hypothesized that PGI2 improves intestinal microcirculation and barrier function via endothelial protective effects. METHODS: ICR and ICGN (a spontaneous nephrotic model) mice were used in this study. Intestinal microcirculation was visualized in vivo to in

Intestinal mucosal immunity is unimportant for polio eradication: the failure of oral polio vaccination.

AIMS: To explore if intestinal immunity induced by infection with live viruses in the oral poliovirus vaccine (OPV) is essential, necessary or even helpful in interrupting transmission of wild poliovirus (WPV) for global polio eradication. METHODS: We reviewed the biology of virus-host interactions in WPV infection and its alterations by OPV-induced immunity for direct evidence of the usefulness of intestinal immunity. We also explored indirect evidence by way of the effect of the inactivated poliovirus vaccine (IPV) on the biology and on transmission dynamics of WPV. RESULTS: Immunity, syst
