Modified hemihamate arthroplasty technique for treatment of acute proximal interphalangeal joint fracture-dislocations.

We retrospectively reviewed 11 patients who underwent modified hemihamate arthroplasty for the treatment of comminuted dorsal fracture-dislocation of the proximal interphalangeal (PIP) joint. This technique was used to minimize the potential risk of complications at the recipient site by volarly oblique osteotomy in the coronal plane at graft harvesting and anatomical repair of the detached flexor sheath. The average joint involvement of the fracture was 58.4%, and the mean duration of follow-up was 38 months. The active range of motion of the distal interphalangeal, PIP, and metacarpophalange

Minimally invasive endoscopic reconstruction technique of acute AC-joint dislocations: a cadaver study.

The treatment of acute Rockwood type III AC-joint dislocations is controversial. Problems related to open surgery are soft tissue healing, residual instability and the necessity of hardware removal. After non-operative therapy the cosmetic result may be problematic and in some cases symptomatic instabilities occur. The goal of the present cadaver study was to develop a new, minimally invasive technique for acute AC-joint reconstructions and to analyse its potential risk for neurovascular injuries. The surgical technique was based on an arthroscopically assisted reconstruction of the coracoclav

[Surgical treatment of type III acromioclavicular joint dislocations by a modified Bosworth technique].

BACKGROUND: We evaluated the results of surgical treatment of type III acromioclavicular joint dislocations by a modified Bosworth technique. METHODS: Thirty-four patients (9 females, 25 males; mean age 35 years; range 20 to 53 years) were treated with a modified Bosworth technique for type III acromioclavicular joint dislocations. Dislocations were caused by falls (n=18), traffic accidents (n=8), and during contact sports (n=8). Involvement was on the right side in 23 patients, and on the left side in 11 patients. The mean time to operation was two days (range 1 to 4 days). Functional evalua

Stabilization of acute, complete acromioclavicular joint dislocations with a new C hook implant.

Although coracoclavicular fixation is currently popular for type III acromioclavicular (AC) dislocations, a surgical gold standard is lacking. The purpose of this study was to evaluate the preliminary outcome of surgical treatment of complete AC dislocations with a new nitinol C hook implant. When the implant is cooled (<5 degrees C), it softens enough to be easily inserted under the coracoid process with a hole drilled in the clavicle. Cooling is induced with ice water. When the implant reaches body temperature, it hardens and anatomic reduction is achieved. Patients with acute type III AC di

[Diagnostic steps in disk dislocations and other frequent illnesses of the temporomandibular joint].

Modern diagnosis of the temporomandibular joint (TMJ) consists of multiple steps, based on each other and amplifying one another. The first step is the clinical functional analysis, respecting general medical as well as specific dental aspects. If need arises electronic axiography, recording the movement of TMJ hingeaxis, is being added as well as occlusal analysis on mounted plaster casts. In case of open questions at this point of analysis, magnetresonance- and computer-tomographic imaging is brought in. The present paper is an overview describing the current step by step diagnosis of TMJ di

Fracture-dislocations of the tarsometatarsal joint, a combined anatomical and computed tomographic study.

Based on anatomical and computed tomographic data as well as experience with the treatment of 30 patients with fracture-dislocation of the tarsometatarsal (Lisfranc) joint, a pathophysiological model is described in which the shape of the foot and ligamentous configuration in combination with applied forces are of pivotal importance. CT imaging helps to elucidate the extent of the lesions, easily overlooked in straight radiographs. In the transverse plane we discern three grades of dislocation. Grade 1, virtually no displacement; grade 2, dislocation of half of the shaft; grade 3, total displa

Double Dislocation of Interphalangeal Joints Accompanied with Contralateral Shoulder Dislocation: A Case Report.

Dislocation of any joint is an orthopaedic emergency and needs immediate attention by the attending physician. A delay in reducing a dislocated joint can lead to disastrous complications both immediately as well as in the long run. Although anterior dislocation of a shoulder joint is by far the commonest dislocation encountered by any emergency care physician, other joints may also get dislocated. In certain cases two joints may get dislocated simultaneously. Such dislocation is known as a double dislocation. Double dislocation of the proximal interphalangeal joint and the distal interphalange

[Traumatic Posterior Sternoclavicular Joint Dislocation in Adolescents].

Posterior sternoclavicular joint (SCJ) dislocation is a rare injury, usually caused by a direct blow to the medial chest wall. The SCJ is inherently unstable due to its bony components and most of its stability is derived from surrounding soft tissue. Among skeletally immature patients it is important to differentiate between true SCJ dislocation and fractures through the medial clavicle growth plate. There is a high anatomical proximity between the SCJ and mediastinal retrosternal structures, as the thoracic blood vessels, and traumatic posterior SCJ dislocation has the potential of harming t

Association between joint dislocation and malignant hyperthermia.

Malignant hyperthermia is a potentially fatal autosomal dominant hypermetabolic pharmacogenetic syndrome resulting from altered intracellular calcium dynamics in skeletal muscle, triggered by halogenated anaesthetics and suxamethonium. Current evidence suggests a degree of association between malignant hyperthermia and joint dislocations. We evaluated 162 patients with a personal or family history of malignant hyperthermia utilising a standardised protocol. We found a significantly higher incidence of joint dislocations in patients with malignant hyperthermia compared to non-susceptible patien

Results of the Treatment of Carpo-metacarpal Joints Dislocations.

BACKGROUND: Carpometacarpal (CMC) joint dislocations are rare, accounting for less than 1% of all hand injuries. They result from direct high-energy blows to the hand, i.e. falls on the outstretched hand, falls from bicycles or punching one's fist against an object. The objective of this case-series study was to analyse the results of treatment of these injuries in 6 patients who presented to the author's institution. MATERIAL AND METHODS: In the 3-year period between 2021 and 2023, a total of 6 male patients, mean age of 39 years, with CMC joints dislocations were treated. Three patients had

Double Dislocation of the Interphalangeal Joints of the Little Finger.

High-energy traumas may lead to the dislocation of joints with or without fractures. However, simultaneous double dislocation of the proximal and distal interphalangeal joints (PIP and DIP) in fingers shows up rarely. Although it can be inferred as simultaneous dislocation occurring during the same trauma, consecutive events should be taken into consideration. A 29-year-old, right-hand dominant male patient presented to the emergency room with a left little finger deformity after being hit by a ball while playing football. Despite the inability to move the little afteruent to this hyperextensi

Divergent dislocation of the carpometacarpal joints: a case report.

BACKGROUND: Divergent carpometacarpal joint dislocations of the fingers are very rare. Due to severe swelling and overlapping of bones on a radiograph of the wrist and hand, dislocations are missed. The purpose of this clinical case report is to highlight this unusual injury to avoid missing diagnosis. CASE PRESENTATION: We report a case of a 24-year-old Moroccan man, an athlete, who presented divergent carpometacarpal joint fracture-dislocations of the ulnar four fingers after a fall during a national cycling competition. Radiographs showed divergent dislocation and associated fractures. He

Isolated ulnopalmar dislocation of the fifth carpometacarpal joint.

Dislocations of the carpometacarpal (CMC) joints are uncommon and are frequently missed on standard radiographs of the hand. Dislocations could be dorsal or palmar; dorsal dislocations are seen more frequently. Palmar dislocations can be either ulnopalmar or radiopalmar. Stable CMC dislocations could be successfully treated conservatively, while unstable dislocations are mostly treated operatively. The purpose of this report is to present a patient with an isolated ulnopalmar dislocation of the fifth CMC joint, satisfactorily treated with closed reduction and casting.

Locked volar distal radioulnar joint dislocation.

INTRODUCTION: Volar dislocation of the distal radioulnar joint is a rare injury which is commonly missed in the emergency departments. A thorough review of literature showed very few reported cases and the cause for irreducibility varied in different cases, Lack of suspicion and improper X-ray can delay the diagnosis. CASE PRESENTATION: Our article discusses a case 40 year old construction worker, who presented to the Emergency with work-related injury, complaining of left wrist pain, deformity and inability to rotate his forearm. X-rays revealed a volar dislocation of distal ulna which was r

A rare case of spontaneous cricothyroid joint dislocation.

Post-traumatic cricothyroid joint dislocation is reported in the literature. Trauma of the laryngeal framework is the most common cause. As a result, spontaneous non-traumatic cricothyroid joint dislocation is very rarely described. We report the case of a patient with spontaneous cricothyroid joint dislocation with the aim of reporting the diagnostic and therapeutic findings related to this rare entity. History and physical examination may suggest diagnosis. Computed tomography scan can confirm diagnosis and detect coexistence of anatomic anomalies of the neck. Reduction of dislocation may be
